During the interview process all candidates will be required to undertake an initial driving assessment (at normal road speed) with a qualified Advanced Driving Instructor (ADI) who will assess each candidates suitability for progressing to completing a Standard Response Driving Qualification. The outcome of the initial assessment, which will assess attitude, awareness around vehicle preparation, vehicle control and driving safely, will form part of the overall interview criteria. In order to be certified as a Blue Light Response Driver, the successful candidate will have to agree to the terms and conditions set out in the NWSSP Study Leave Procedure, participate in a 2 week intensive driving course funded by NWSSP and pass the final assessment. Multi-Disciplinary Teams Overall responsibility for arranging the daily and weekly PCN and Community team led MDT meetings to ensure smooth running of integrated care within the team setting. Key roles include searches and discharge data analysis to identify patients, managing the meeting agenda items; ensuring that all new referrals are identified, and information circulated to team members of the meeting as required. Coordinate and manage the administrative functions of MDT meetings. Liaise with all clinical and non-clinical members in the MDT to ensure effective MDT function. Take minutes of MDT meetings and disseminate; chase progress against actions identified in these meetings and ensure follow up where necessary. Manage reporting required and associated within the DES specifications for required services. Patient Identification Receive and collate information from transfers of care (including hospital admissions and discharges) plus out of hours calls and present this information to the MDT as required. Liaise with service providers and clinicians to identify frequent flyers, and new service users utilising risk stratification tools provided and present this information to the weekly MDT meetings. Support the completion of new referrals by checking criteria, and where criteria have been met, direct referral to the MDT. Signpost team members, service users and carers to relevant services Maintenance of IT based information systems and responsibility for key performance data: To ensure the IT requirements for recording activity are adhered to in collaboration with other team members Accurate update and maintenance of GP systems within the MDT. To provide agreed performance/activity data within the MDT and PCN. Communication and collaborative working relationships Demonstrates ability to work as a member of a team. Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary. Actively work toward developing and maintaining effective working relationships both within and outside the PCN or group of PCNs. Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to Patients GP, Nurses, other practice staff and other healthcare professionals including pharmacists and pharmacy technicians from provider and commissioning organisations. Work with service users, PCN practices and partners Develop excellent working relationships with all partners, wider service networks including the voluntary sector, GP practices, adult social care, hospitals, community pharmacists and other members of the MDT Acting as a point of contact for residents, families and professionals who visit the care home, such as MDT members and in-reach specialists. Meet regularly with the clinical lead and review case load and MDT function. Keep the MDT and OHP organisation abreast of good news stories. About Us

Teaching Assistants and Communication Support Workers (CSW) are an essential part of the educational team working with children and young people who are deaf in Oxfordshire educational settings. TA/CSWs work in the Deaf and Hearing Support Resource Bases at New Marston Primary School and The Cherwell School (secondary), and with post-16 students who have a sensory impairment (deafness or vision impairment) in the Colleges of Further Education in Oxfordshire with a base at City of Oxford College.

About the Role

We are seeking Teaching Assistants or Communication Support Workers with a calm, encouraging and positive manner to work with deaf children and young people in the Deaf and Hearing Support Resource Bases at New Marston Primary School, Cherwell School in Oxford or City of Oxford College. No previous knowledge of working with deaf pupils is required.

The deaf pupils supported by the base have a range of communication modes and learning styles.Some speak and listen as their primary mode of communication, some use sign language together with speaking and listening, and some are British Sign Language users.

There are good public transport links and parking is also available.

The role is permanent and is term time only (39 weeks per year).

About you

You will have the ability to communicate sensitively and appropriately with children, young people, families, and colleagues.You work positively as part of a team but also feel confident to work independently and take initiative when needed.

You have a good level of basic education (e.g. Grade 4 or above in GCSE Maths and English or equivalent) or equivalent professional experience.

Working closely with specialist Teachers of Deaf Children and Young People (ToDs), mainstream staff and other professionals to improve outcomes, your aim will be to enable access to the curriculum and to facilitate full inclusion for the pupils you work with.

Ideally, you will have experience of working with children and young people with SEN or disability, but training will be given.We are particularly interested if you have a qualification in communicating with deaf people (preferably Level 2 CACDP or above, or a level of fluency in BSL at a comparable stage). If you have an interest in learning sign language, this role offers opportunities to develop these skills.

You will have excellent communication and interpersonal skills and be resilient, creative, and adaptable.

No previous knowledge of working with deaf pupils is required but would be an advantage.A passion for working with children and young people to make a difference and improve their access to learning is essential.
