You’ll work with a caseload of adult males within a custodial or community environment. In partnership with probation practitioners, you’ll complete initial assessments to understand their specific needs, and then create tailored accommodation plans to meet this through an agreed number of interventions. CMHN within an integrated Community Mental Health Team, older age adults (65+)

To hold a caseload; assess, monitor and offer advice, support and guidance to our patients and their carers and families.

The Service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic / functional mental illness in an inpatient or community setting would be an advantage. The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to the older adult and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the wider team.
