Duties/Responsibilities will include:
Incumbent is responsible for clinical and administrative coordination of services and oversight provided to veteran's enrolled in Homeless Grant and Per Diem (GPD) Programs as described in the Homeless GPD program rules, regulations, and directives. This includes verification of veteran status and eligibility for participation in the Homeless GPD Program and other homeless programs including HUD-V ASH, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting participant information as outlined by NEPEC evaluations procedures. The incumbent develops and implements information systems to track services and is responsible for data collection, analysis and reporting of program outcomes.
The Incumbent may develop quality monitors and data points that will provide a periodic snapshot of program utilization, barriers to delivery of service, performance measurement of program, and all National/VISN quality performance goals. The incumbent may identify best practice programs for consideration of implementation. Incumbent may support research to identify local veteran needs. Incumbent assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application. Incumbent is responsible for overseeing GPD providers' billing payment and funding. Incumbent is responsible for ensuring that the monthly billing for all GPD providers is accurate and completed in a timely manner. When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction.
Incumbent coordinates and participates in program site inspections utilizing the Grant and Per Diem Program rules, regulations, and directives as guidance as provided by National GPD Program Office and Homeless Program Office. The incumbent is responsible to forward completed inspection packages to the appropriate offices. When necessary, the Incumbent notes inspection deficiencies and establishes methods to monitor corrective actions and develops time lines for consequences as a result of inaction or insufficient actions related to deficiencies. Incumbent monitors the clinical care of program participants, and utilizes various treatment modalities of individual and group work, case management, community organization, and community outreach activities to alleviate or remove problems that interfere with the Homeless Grant and Per Diem Program participants' ability to achieve independence.
Incumbent coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the medical and mental health care at VA Medical Center, as well as referrals to VA Regional Offices and other non-VA entitlement programs as well as community-based social services and agencies. Incumbent participates in HCHV Team events within the VA and the community. Incumbent may serve on appropriate community boards, committees and councils. Incumbent performs accountability functions: Documenting all contacts with patients/families in the medical charts; Completing statistical, administrative, and all other reports in accordance with Mental Health Service policies, VA Medical Center, VISN and VA Central Offices.
The Incumbent may develop education and training programs and provide technical assistance in support of the GPD Program including interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Training may be provided to staff on a variety of Homeless Veteran healthcare topics that is consistent with program needs. Incumbent actively participates in the Homeless Programs including GPD Program conference calls and attends appropriate training offered by the National Homeless Programs Office, National GPD Office and VISN training or calls. Incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of Interest while serving as the Homeless Grant and Per Diem Program Liaison. Incumbent performs other duties as assigned.
Work Schedule: 8:00am - 4:30pm, Monday - Friday, (Subject to change based upon the needs of the agency.)
