The Polytrauma System of Care programs focus on active duty service members who are being treated under the sharing agreement with the Department of Defense, and veterans who are enrolled in the VA.

The PSC care management practice, which includes psychosocial assessment, diagnosis, and treatment, is focused on helping service members and/or Veterans and their families maximize rehabilitation and treatment potential and achieve more adequate, satisfying, and productive emotional and social functioning.

The PNS team will coordinate the care of returning service members and veterans who have experienced traumatic brain injury. The care may be received at any of the Health Care facilities and Clinic Based Outpatient Clinics {CBOCs} located within VISN 21, including VAPAHCS. The PNS team identifies, screens, assesses, evaluates and triages eligible returning combat service members/veterans to receive appropriate care throughout all the facilities within VISN 21 (major medical centers and the respective CBOCs). Duties/Responsibilities will include the following:

As an ACC PCMHI coordinator in the Integrated Mental Health in Primary Care Program, incumbent manages the walk-in (warm handoft) schedule and crisis response availability for PCMHI providers in the ACC. Incumbent is responsible for such administrative tasks as monitoring consults, coordinating program schedule, facilitating team meetings updating and revising the literature provided to veterans entering the program, ensuring implementation of outcome measures and training staff in how to properly use the tools with veterans entering the services. Assists colleagues in preparing for Joint Commission survey by a review of patient records in CPRS and assisting in completion of necessary and required documentation.
