The incumbent serves as a Program Analyst (Health Informatics) located in Charleston, SC. JOB DUTIES
Independent practice level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans and/or family members/caregivers in the PACT/Primary Care and SCI&D Clinic. Able to work independently with Veterans and family members experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and other psychosocial problems. Effective social work often occurs over a series of several visits. Must independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and their family members. Able to assess functional acuity of the patient and provide appropriate interventions.

Must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence possible.

Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as team members in the development and implementation of treatment goals and interventions.

Possess ability to coordinate community placements through collaboration with Veterans and family members as well as team members to ensure that appropriate community placements are completed in a timely manner. Community placements can involve but are not limited to referrals to group and family care homes, assisted living facilities, adult day healthcare programs, VA contract and private nursing home placements, Community Living Centers, residential care homes, inpatient and outpatient hospice services.

Serve as a liaison between Veterans and/or family members, VA and community resources, and local hospital case management staff in order to ensure thorough coordination of services.

Must possess knowledge and ability at the advance practice level to independently implement treatment modalities and evidence-based practices, provide educational classes, and/or supportive groups for Veterans and families, including shared medical appointments. The Social Worker must be able to use and teach Veterans in the effective use of My HealtheVet (MHV) system.

Must possess skill in using computer systems, especially for the internet for Veterans who choose this modality to communicate with the team.

Provide consultation and education to Veterans and family members regarding community resources, VA benefits and specialty programs, and Advance Directives. This includes knowledge of the process for accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs.

Must be available to provide consultation to other treatment team and staff members regarding psychosocial needs of the Veteran and/or families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment.

Serves as Occupational Therapist at a Medical Referral Center providing inpatient and outpatient services to Federal prisoners referred by physicians, mid-level practitioners or dentists.

• Provides education when dealing directly with physicians, nurses and other clinical providers requesting completion or adjustment of pertinent parts of the health records, which are found to be inconsistent or incomplete upon analysis. Assists and provides guidance to supporting clinical services to capture required medical information on patients for computer database input.
• Selects, compiles and trends medical, surgical, and statistical data from numerous components of the health record. Retrieves diagnostic and statistical data for various reports required locally or by VISN or VHA Central Office.
• May assign User Classes to users at the request of the Chief HIM; responds to questions or issues related to that user class. Reviews requests for new templates or additions/correction to existing templates (e.g., clinical reminder dialogs, documentation templates, Power Forms). Reviews requests for new paper forms and assists in the creation and implementation of electronic versions of these forms so that they are always available to EHR users.

The duties of the Medical Technician Phlebotomist position include, but are not limited to:

• Obtain and maintain a current Basic Life Support (BLS) certification to enhance professional growth and development through participation in educational programs such as BLS, in-service meetings, and workshops.
• Performs phlebotomy using appropriate and established venipuncture techniques and universal precautions, being careful to avoid areas close to nerve bundles and arteries.
• Conducts any or all of the following tasks related to specimens; performs collection, ensures accurate and complete labeling and accessioning, distributes to appropriate areas of the lab, runs through centrifuge and de-capping machines, and stores analyzed specimens in racks according to day and lab number.
• Accurately evaluates and interprets patient orders in the computer to determine which testing may be accessioned and performed at the time, if fasting or other specific preparation is required, and if any specimens must be collected and returned by the patient.
• Collects samples with knowledge of the many special anticoagulants and testing requirements for routine and complex testing both pre- and post- collection, being careful to choose the correct sample and tube types.
• Performs positive (active) identification of patients and specimens, including full name and full social security number through entire process of specimen collection, accessioning, processing, and storage/distribution of patient specimens to clinical laboratory departments. Coordinates with all other clinical departments, to include medicine, surgery, dialysis, dermatology, to complete order entry and accessioning of test orders in the computer, correcting duplications or entry errors.
• Maintains laboratory records of all tests results, while strictly adhering to Clinical Laboratory Improvement Act (CLIA) and local/agency requirements.
• Observes patient at all times during and after collection for any adverse responses. Handles unusual patient situations appropriately (blind, deaf, suicidal). Properly applies pressure to the puncture site, evaluates for bruising or excessive bleeding, and bandages appropriately.
• Performs lactose and glucose tolerance tests using multiple complex steps; must interpret fasting glucose level and determine appropriate dosage of glucose/lactose by integrating patients age, weight and pregnancy status.
• Assists with monitoring of inventory and maintenance of adequate phlebotomy supplies and thorough cleaning/disinfecting of work areas. Selects appropriate Personal Protective Equipment (PPE) for handling of patients, samples, transport, and cleaning. Operates, maintains, and troubleshoots laboratory centrifuges for processing of blood samples to maintain specimen integrity.
• Maintains laboratory facilities, equipment and supplies in such a manner to provide for safety and efficiency of laboratory operations.
• Conducts and documents periodic light maintenance and Quality Control of laboratory equipment, including cleaning, restocking, checking controls, and verifying that automated maintenance procedures have functioned appropriately.
• Processes and prepares test samples for shipping, using universal blood and bodily fluid precautions and lab and regulatory procedures for processing bio-hazard material for transport out to referral laboratories.
• Accepts computer and written requests for all clinical departments, to include primary care, surgical, and medicine.
• Coordinates with all other clinical departments, to include medicine, surgery, dialysis, dermatology, to complete order entry and accessioning of test orders in the computer, correcting duplications or entry errors.
• Performs other duties as assigned
