As an Outpatient Nurse Navigator at the Referral Access Center, you will guide and support patients receiving outpatient services.

Role: Outpatient Nurse Navigator (RN)
In this role, you will educate patients, coordinate care, assess patient needs, provide emotional support, navigate through the healh care system, and ensure effective communication among health care team members. Your goal is to enhance the patient experience, improve outcomes, and streamline the outpatient care process.
Position Highlights:
What You'll Do:
- Rounds on/reviews assigned patients regularly and evaluates patient progress with a plan of care. Communicates the plan of care to the patient and family. Solicits concerns, questions, and issues for resolution.
- Provides emotional support, counseling, clinical education, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
- Serves as a liaison between patient and clinical staff, administration, physicians, managed care companies, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.
- Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Intervenes as appropriate to advocate for patient and family
Your Qualifications:
- Possess a current State of Maine RN license or the ability to obtain one promptly
- At least 5 years of acute care nursing experience is preferred
- Strong communication and critical thinking skills are essential for this role.
- Ability to use strong critical thinking skills to perform duties in a proactive and organized way
- Ability to prioritize workload, handle multiple tasks, and to work independently with limited supervision
