The ED RN is responsible and accountable for all elements of the nursing process when providing care. They assess the patient by collecting pertinent data or information, including the triage of patients based on the five-level triage system to prioritize patients that require more immediate care. They use the assessment data to determine diagnoses, problems, or other concerns. They will identify expected outcomes to develop a collaborative and individualized plan for each patient. The ED RN will implement that plan, including care coordination, health teaching, and health promotion. The Dialysis Registered Nurse (RN) is a professional nurse with specialized knowledge and demonstrates advanced skills when providing and directing care for patients with end-stage renal disease (ESRD), acute kidney injury (AKI), and late-stage chronic kidney disease (CKD). Collaborates with other disciplines/departments to achieve desired patient outcomes. The primary purpose of the Peritoneal/Hemodialysis Nurse RN position is to demonstrate leadership in delivering and improving holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned. The Peritoneal/Hemodialysis RN demonstrates clinical competence through providing direct-care and case management to people with chronic kidney disease, provision of peritoneal/hemodialysis treatments, continuous renal replacement therapy (CRRT), patient and family education, assessment, and management of peritoneal and/or hemodialysis catheter access, and provision of the holistic needs of the patient under his/her charge. Responsibilities include but are not limited to renal anatomy and physiology, technical and didactic knowledge of dialysis therapy (i.e. water testing, treatment modalities, dialysis machine operations, disinfection and routine maintenance of dialysis equipment), coordination of care with the inter professional team (physicians, nurses, pharmacists, intensive care unit (ICU) nursing staff, renal dialysis technicians, renal social worker, and other members of the healthcare delivery team), management of the peritoneal dialysis prescribed treatments (adequacy testing, patient/caregiver education, completion of screening, conducting home visits as necessary, collaboration with the surgical teams as necessary for catheter management, and evaluation and management of peritonitis or other emergent conditions), data collection and reporting, assisting with management of peritoneal supplies for both the facility and the patients, assisting with medical staff education, and providing training to other medical center staff on peritoneal/Hemodialysis. Depending on the case load, the Peritoneal/Hemodialysis RN will assisting/performing the duties necessary to manage hemodialysis patients on or off the dialysis unit to include ICU, emergency department, and any other area approved for dialysis within the facility. The Peritoneal/Hemodialysis RN shares responsibility to ensure dialysis services are available 24/7 for emergencies by participating equally with peers in rotating weekend, and on-call duty assignments. In this first year, the novice is "taught about situations in terms of objective attributes and other measurable parameters of a patient's condition, features of the task world that c an be recognized without situational experience. The Surgical Specialty clinics require specialized nursing care and evaluation depending on services provided. The RN is responsible for the surgical and non-surgical patient care while utilizing all aspects of the nursing process and works in collaboration with the care coordinators, providers, residents, surgical technicians, and nurse manager. The RN systematically assess the health status of individuals and documents related health data in accordance with institutional policy providing pre, intra, and post procedure care. Additionally, the RN provides education to patients, families, significant others, and/or caregivers. The RN may be required to provide post-op care, wound care, or any other nursing interventions as permitted by governing licensing authorities and/or the Veterans Health Administration.

The Peer Specialist functions as an interdisciplinary team member, assisting physicians and other professional/non-professional personnel in a rehabilitation treatment program. Peer specialists perform a variety of therapeutic and supportive tasks that include assisting Veterans in articulating their goals for recovery and personal wellness, learning, and practicing new skills, helping them monitor their progress, assisting them in their treatment, modeling effective coping techniques and self-help strategies based on the peer specialist's own recovery experience, and supporting them in advocating for themselves to obtain effective services. This position will serve as a community-based peer specialist within the local VA facility catchment area. The working environment may include providing peer support services in places such as Veterans' homes, prisons, jails, treatment courts, shelters, tent encampments, street outreach, etc.

• Participates in the service, training, consultation, and other professional activities of VA health care services and functions as a fully vested member of the multidisciplinary team.
