The social worker in this veteran-centric approach often functions as a care coordinator and case manager for primary care veterans in all settings to ensure that health care meets the needs, as defined by the veteran. The primary care social worker is responsible to answer social work and advance directive consults, to triage and refer veteran needs to other social work practice areas as appropriate, and to utilize population health management to provide proactive outreach in order to improve quality of care. The primary care Social Worker will also work with veterans utilizing specialty care such as, but not limited to, urgent care, neurology and pain clinics as these veterans and families can often benefit from social work support in addressing social determinants of health. The role of a social worker case manager in the PACT is to assess and treat psychosocial and environmental factors that impact the patient's ability to achieve maximum health and wellness. Social work case managers assess the patient's psychological and emotional adjustment to illness within the context of medical diagnosis, prognosis, and treatment options.

The duties and responsibilities include but are not limited to:

• Maintain effective therapeutic relationships with veterans in the primary care clinic and/or their families.
• Complete thorough psychosocial assessments to determine the psychosocial functions and needs of veterans and/or their families.
• Actively participate as a member of the interdisciplinary treatment team.
• Facilitate action for community placements through collaboration with veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner.
• Serves as a liaison between veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.
• Provides consultation and education to veterans and their families regarding community resources, VA benefits and specialty programs and advance directives.
• Provides consultation to other treatment team and staff members regarding psychosocial needs of veterans and/or families and the impact the issues may have on the veterans health care planning and compliance with treatment.
• Make referrals to community and other governmental programs and agencies.
• Provide case management services to the veterans and their families throughout the continuum of care.

Major Duties

• Performs a variety of tasks involved with installing, modifying, troubleshooting and repairing new and existing systems, including, but not limited to: condensers, chillers, air handlers, compressed air, humidifying, dehumidifying, filtering, refrigerators, freezers and computer based Energy Management Control Systems (EMCS).
• Maintains, repairs and installs new and existing plumbing systems, to include, but not limited to: high/ low pressure piping, sanitary sewer, steam heat, steam generation, circulating, vacuum, and condensate pumps, expansion tanks, radiators, lavatories, commodes, and water heaters.
• Works from building plans, blueprints and sketches to plan and layout routing of piping, vents, wiring, refrigeration lines, ductwork and valves.

• Reads and interprets blueprints and wiring diagrams relative to the assigned equipment and utility systems. He/she must be able to read and understand technical manuals and instructions on air conditioning and refrigeration equipment, as well as pumps, valves, gas and oil fired boilers and furnaces.
• Estimates cost, both labor and material, and prepare lists of materials for the completion on any job to be done
• Uses hand and power tools, to include, but not limited to: multi-meters, diagnostic equipment, air velocity meters, flue gas analyzer, grinders, pipe threading machine, pipe cutter, Oxy-Acetylene torch, refrigeration gauges, and motorized drain cleaner.
• Troubleshoots, calibrates, balances and operates central heating, ventilation, air conditioning, and steam systems and their associated equipment and controls within the facility.
• At a minimum, possesses a Universal Technician certification in accordance with 40 CFR part 82, subpart F, as well as a Gas Fitters License.
• Creates leak-free joints in piping and tubing systems by using the appropriate method, to include, but not limited to: solder, silver solder, Teflon sealant, and compression fittings.
• Possesses the knowledge of electrical wiring and related components of pumps, air handlers, chillers, water heaters, exhaust fans, electronic valves, etc. and be able to troubleshoot, repair and/or install such items.

• Confers with the Maintenance Scheduler and/or the M&R Operations Supervisor regarding his findings relative to maintenance and repairs to utility systems and equipment and helps plan and coordinate the repair of any issues.
• Knows codes and regulations pertaining to the practice of HVAC, refrigeration, plumbing and steam. Plumbing includes but is not limited to domestic water, heating/cooling piping, natural gas/propane, compressed air, sanitary drain piping.
• Plans his/her work in compliance with the prescribed standards of quality and quantity in accordance with established policy, codes, and regulations.
• Performs maintenance of all air conditioning and refrigeration equipment, including changing oil, replacing gaskets, replacing defective lines and/or valves including expansion valves, back pressure valves, water control valves, pressure regulators, compressors, receivers, condensers, evaporators, and automatic controls.
• Performs maintenance, repair, and installation on, but not limited to the following dietetic equipment: reach-in and walk-in refrigerators/freezers and all related components, ice making machines, hot-cold food carts, and lowerators.
• Maintains, repairs and modifies compressed air systems (excluding Medical Air) throughout the facility to ensure proper operation, pressure and filtration.
• Performs maintenance and repair, extensions and alterations to all piping, fixtures, appliances and appurtenances in connection with any of the following: sanitary drainage or storm drainage facilities; the venting systems; the water supply system; gas distribution within or adjacent to buildings, structures or conveyance and throughout the grounds of the medical center.
• Installs and maintains all types of equipment such as lavatories, commodes, showers, and dietetic equipment, hot water heaters, boilers, radiators, pressure regulating valves, sprinkler systems, sterilizers, steam traps, back-flow preventers, and all thermostatic equipment.
• Makes preventive maintenance inspections of all steam, water, and gas lines, and on fixtures and appliances, relative to those portions of utility systems.
• Performs the following related tasks in connection with said duties: pipe covering, welding, brazing, sheet metal work and minor electrical work such as controls on pumps and HV AC equipment, chrono-flow and motorized valves, vacuum pumps and pneumatic systems and piping or plumbing related to x-ray and dental equipment.
• Follows all Lock-Out Tag-Out procedures as required by OSHA and VA regulations.

The Transport Planning Team is looking to recruit a Principal Transport Planner to deliver our residents' parking programme and support other transport projects such as our car club and mobility hubs. As a Principal Transport Planner, you will be responsible for the provision of expert advice on strategic transport issues, particularly those regarding parking provision. This role has a focus on the development of transport strategy, including delivery of our Portsmouth Transport Strategy and Parking Strategy. As well as developing emerging transport policy and strategy, your work will help to shape a number of high-profile pieces of work. What you'll need to succeed

You will have a relevant degree level education OR demonstrable equivalent professional experience. Membership of a professional body is desirable but not essential. You will be able to communicate messages (which may be complicated or sensitive) clearly, concisely and at a level appropriate to the audience, who may not be specialists in transport planning.

You will have excellent communication skills with the ability to represent the service with direction at highly challenging internal/ external meetings, with integrity and confidence. What is the role?

• Carrying out inspections on blocks of flats, communal areas, play sites, parking sites and garages ensuring they are maintained in a safe, clean, and tidy condition, identify and report repairs that are required; routinely carrying out minor repairs to housing communal areas, such as changing light bulbs which will require the use of ladders.
• Communicating with residents in person, on the phone and in writing.
• Using word templates to issue letters to residents, following correct procedures regarding letter and report writing.
• Reporting and logging incidents of anti-social behaviour.
• Responding to calls and listening to residents to gain an understanding of the situation / circumstances.
• Reacting to problems appropriately and when necessary challenging resident behaviour.
• Speaking to tenants and leaseholders regarding the correct disposal of black bags and bulk items with a view to re-educating those who are not disposing of these items in the correct way. Educating residents about recycling.
• This is not an office-based role, and you will need to be able to walk up several flights of stairs, move refuse bins and access bins to investigate waste.
• This is a varied and challenging role that involves dealing with a wide section of the local community and other external agencies such as the police as well as putting you in close contact with numerous departments within Portsmouth City Council, so your ability to work in partnership with others will be paramount.

Who is the person?

You need to:
1. Have experience of working with the public and be comfortable visiting customers in their home.
2. Have the ability to communicate effectively at all levels from residents to Councillors, social services, and staff across Portsmouth City Council.
3. Have a proactive approach when dealing with and identifying issues in the area, engaging with customers, in a calm and tactful manner, and challenging behaviours when necessary.
4. Be able to work independently and as part of a team, using your own initiative and judgement in order to find appropriate and effective solutions.
