Chronic disease management contribute to the monitoring of patients with, for example, respiratory disease, diabetes, coronary heart disease, renal failure, hypertension. Encourage patients to take part in health screening. The role may require, when appropriately trained, to perform cervical screening tests. Care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities. Providing and monitoring care Carry out procedures safely and effectively within the PCN practices and also in the patients home (including care homes), clinical tasks to include: Clinical observations such as BP / Pulse / Temperature / Oxygen saturations / point of care blood glucose / BMI Venepuncture Spirometry ECG Diabetic foot checks Sample processing and management Provide care and support with hygiene and maintenance of skin integrity including minor and complex wound care Administration of medicines via oral, topical and inhalation routes and administer injections using subcutaneous and intra-muscular routes. This will include immunisations influenza / shingles/ RSV/ pneumonia, B12 Assist GP / ANP with minor surgery / invasive procedures for example coil fitting Contribute to on-going assessment and recognise when it is necessary to refer patients onto others for reassessment. Working in teams Demonstrate the ability to recognise changing priorities seeking advice and guidance from the registered nurses or other registered care professionals as appropriate. Support, supervise and act as a role model to undergraduate student nurses (years one and two), trainee nurse associates, health care support workers and those new to care roles, review the quality of the care they provide, promoting reflection and providing constructive feedback. Participate in team meetings, clinical supervision and where required, contribute to setting the agenda and recording minutes. Contribute to team reflection activities, to promote improvements in practice and services. Improving safety and quality of care Protect health through understanding and applying the principles of infection prevention and control, including communicable disease surveillance and antimicrobial stewardship and resistance. Maintaining a safe work and care environment in particular; Cleaning and restocking consulting rooms, stock control of consumables. Adhering to infection control policies and procedures to include participation in regular audits, hand hygiene training, vaccine/cold chain storage awareness, spillage control procedures. Contributing to integrated care Ensure clear, concise, accurate and legible records and all communication is maintained in relation to care delivered adhering to local and national guidance. Develop relationships across the MDT to support integration of the role across health and social care including primary care, secondary care, and mental health. Communicate effectively across a wide range of channels and with a wide range of individuals, public, health and social care professionals, maintaining the focus of communication on delivering and improving health and care services. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals. All staff have the responsibility to support all learners and to keep their knowledge of supporting learners in practice up to date, according to appropriate governing bodies regulations. Role of a Shunter driver:

• Ensure full trailers are taken on and empty trailers take off the infeed docks in a timely manner as communicated by Operations.
• Ensure empty trailers are on the relevant outbound docks and secured for loading.
• Communicate with the Warehouse team to ensure drivers and sub-contractors are given the correct trailer for the movement they are scheduled to.
• Ensure all trailers are moved around the site in a safe and secure manner following all site health and safety procedures, reporting any incidents or near misses,
• Ensure customer material is not damaged during transportation to / from docks.
• Ensure all trailers are in good condition before use and liaise with Operations on site to report any defects.
• Complete scheduled movements of trailers around the site, on / off docks and in the trailer park.
• Sort and prioritize workload to ensure time commitments are met.
• Ensure correct completion of any reports / schedules documentation.
• Comply with DHL's uniform requirements and ensure all relevant ID passes are worn at all times.
• Accurately record working and driving hours to comply with road transport regulations.
• Achieve targets, productivity, service and compliance.
• Ensure health and safety rules of the site are strictly adhered to, paying particular attention to the processes regarding unloading and loading.
• Complete daily vehicle safety checks, safe loading / securing a load of trailers, report maintenance defects, adhere to company driving standards, fuel efficiency and telematics training.

Retail Merchandising Team Driver

Are you looking for retail work? Do you like driving and working all over the country? Do you have a keen eye for detail? Are you looking for a role that is varied and flexible?

As a Retail Merchandising Driver for Momentum Instore, you will be working in major retailers around the country. We will provide you with transport and accommodation, with a team of merchandisers to replenish shelves.

Flexibility is key! The hours and days/nights of work vary with each project, although usually you will be expected to work 11 hour shifts. We aim to keep the work near to home, but you may be needed to travel and stay away from home.

A full UK driving licence is essential as you need to drive yourself and your team colleagues to store. We will provide you with a pool or hire car. You will need to be over 23 years old and been driving for at least two years. This is for insurance purposes.

Knowledge of retailing and experience of planograms and merchandising is useful, but we offer a 13 week training plan to applicants with a great work attitude.

We will fast-track anyone who have previous experience in systems / dressing or team leading.

The nitty-gritty

As a Retail Merchandiser, you will need to:


Merchandise and replenish shelves using planograms within a set timescale.
Profile shelving, install equipment and implement display to a high standard.
Clean and maintain equipment and general housekeeping.
Lifting and manual handling up to 25kgs may be necessary.


What you can expect from us:


Starting basic rate at £11.44 per hour
Weekly pay
Work is intermittent and the projects you are asked to support could be over days, nights or weekends.
If you are asked to stay away from home, shared accommodation will be provided.
Driving to Success Scheme: Drivers who take two, or more passengers in their car can earn an extra 50p an hour per shift

Location: Field Based, regionalised, however travelling and staying away from home is sometimes required

Hours: Shifts are usually 11 hours with a 1 hour unpaid break and a 30 minute paid break
