The post holder will have a proficient knowledge of NHS Continuing Healthcare Framework and will advise on most aspects of NHS Continuing Healthcare Framework and also have neurological experience.

The post holder will assist for the day-to-day operational management of the Community Neuro Service within the Newham CHC Team.

The post holder will maintain a neuro clinical caseload that involves the reviews and care coordination of clients who are eligible for NHS continuing healthcare who are in placements and in community.

The post holder will support the development of care pathways, protocols and audits.

The post holder will support the introduction of appropriate, evidence-based specialists neurological placements and education for the team.

To contribute to the planning and development of services for neuro clients and their carers.

Facilitating inter- agency working, which aims to develop good case management practice in order to improve the experience of people, living in Newham with highly complex long term neurological disabilities, as they journey though the community settings.





To be aware of and report any concerns relating to potential of deprivation of liberty to ensure that a best interest assessment is conducted if required.

To provide written reports of clinical opinion to health and social care colleagues as required

To carry a continuing Healthcare caseload of service users with complex long term physical and neurological disability living at home in or out of the London Borough of Newham and registered with a Newham GP.

As a Senior Programme Manager in the Hospital Policy Team within the Integrated Urgent and Emergency Care Team, the post holder will lead on the development of hospital policy, best practice guidance and products to support improvements in the delivery of care to patients in the UEC hospital pathways. This will include critical stakeholder engagement work with the National Clinical Directors and Regional Clinical Advisors, regional, ICB and Trust staff across the NHS.
