• Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate. Recommends formulary alternatives to non-VA, non-formulary medications when appropriate.
• Monitors for and reports medication errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings in accordance with facility policies and procedures.
• Has expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
• Participates in medication reconciliation by updating the medication profile to reflect an accurate, active list of VA and non-VA medications. Alerts provider if patient has changed his/her medication regimen to the extent that the medication management should be reviewed by the provider.
• Reviews and evaluates orders for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.
