Hours: 37.5 hours a week (normally 9-5, but flexibility required)
Salary: £26,520 per annum
Holidays: 25 days per year plus 8 public holidays (pro rata)
Period of Notice: 1 week during probation; 4 weeks thereafter

Cyfle Cymru is a Peer Mentoring project which helps people develop confidence to make positive life changes, and provides support to access training, education, volunteering and employment. Our peer mentors draw on their own lived experience to support people affected by substance misuse and/or mental health conditions. Cyfle Cymru will support people aged 16 to 24 (Not in education, employment or training) and those 25+ that are economically inactive or long term unemployed.
Job Purpose:
• Sourcing eligible participants by networking with potential referrers/ agencies ensuring adequate eligibility evidence
• Raising awareness of the service through presentations, attending appropriate meetings and events
• Ensuring the registration outcomes are delivered on target
• Record all activities accurately including client registration forms and outcome monitoring forms required of the scheme.
• Refer participants to other services within the partnership, and other external agencies as appropriate.
• Provide excellent customer service, identify the needs of internal and external customers and strive to deliver a prompt, effective and personalised service.
• Promote training opportunities, group activities and volunteering events that are available to the client group
• Ensuring action plans are completed on registration
• Work alongside the Peer Mentors in delivering the projects goals and objectives
• To support the Project Manager in raising awareness of the service
Responsibilities:
• Take action to ensure deadlines and outcomes are achieved.
• Assess candidates for suitability of the Cyfle Cymru Peer Mentoring service.
• Work with the Peer Mentors and Employment liaison officer to discuss training and employment opportunities for participants to a gain a positive outcome. This will include goal setting, develop activities and on-going placements.
• Monitor and act on customer feedback, picking cues regarding customer satisfaction.
• Time management – Establish priorities and develop clear and logical plans to achieve goals
• Utilise any other tools and techniques such as brief interventions and Adult Directions in assisting participants achieve their goals and objectives for the Peer Mentoring Scheme.
• Record all activities accurately and in compliance with both organisational and Welsh Government regulations. This would include client registration forms, sessions records and any other outcome monitoring required of the Scheme.
• Competent to complete all tasks online via on a database system, and able to upload evidence
Client Work:
• To communicate skilfully and sensitively with highly complex and sensitive information, taking into account sensory and cultural barriers to communication.
• To provide additional advice to aid recovery of Substance misuse and/or Mental health.
• To provide one to one support to clients within the registration process.
• To be able to demonstrate enthusiasm for development and progressions of the client group by providing details action plans that can be supported by the Peer Mentor.
• To refer clients to other services within the partnership, and other external agencies as appropriate.
• The ability to prioritise and organise own workload
• To be accountable for own decisions and actions in consultation with supervision.
• To undertake clinical administrative tasks and reports relating to mentoring project when necessary.
• To provide feedback to referrers when appropriate
• Maintaining up to date knowledge of the client groups and activities available on the service
