Job Summary: To provide a high-quality, advanced, expert nursing and cost-effective service to patients referred to the Centre for Active Lifestyle Management (CALM) Service. This comprises a multi-professional team (Psychology, Physiotherapy and Nursing) who work with patients with chronic pain to help them improve their quality of life. The Division aims to provide an environment conducive to the on-going development of all staff and advancements in the quality of the service by ensuring provision of a high standard of training, teaching and clinical care. The Clinical Nurse Specialist will be an advanced and experienced practitioner within the field of chronic pain and they will be expected to provide clinical support, working independently or assisting with multi-professional interventions in which full training has been undertaken based on clinical practice standards. The role of Clinical Nurse Specialist requires a high degree of personal and professional autonomy in making clinical judgements. Taking sole and joint responsibility as appropriate for patients during their time in the care of the CALM service to ensure a successful patient-based outcome, which will require assessment, planning and implementation of treatment. The Clinical Nurse Specialist will be required to give advice and guidance to other health professionals regarding chronic pain interventions and clinical practice. They will be responsible for the nursing leadership within the CALM service, giving support and guidance within the multidisciplinary team and contributing to the high-quality care given to patients in accordance with clinical governance. The postholder will ensure the delivery and expansion of an expert and advanced nursing service through effective interdisciplinary care, utilising clinical and managerial expertise. This will include a review of the effectiveness of treatments offered. Key Result Areas & Performance: All the following are to be carried out while working in close collaboration with members of the multi-disciplinary team led by the Consultant Clinical Psychologist. The post-holder will need to demonstrate clinical expertise within the field of chronic pain and work autonomously, managing own allocated clinics independently as well as joint working with the multi-professional team and seeking guidance/support when necessary. Contributing to the design and delivery of the physical health content of the CALM programme, as well as contributing to the handbook for the programme. Taking an active role within the multidisciplinary team in discussing patients, advising and explaining medical pathologies and treatment methods, and others matters relating to complex chronic pain patients in order to ensure appropriate referral, management and treatment. Sharing responsibility with other members of the CALM team for writing letters regarding clinical decisions to patients and referrers. Offering a supportive service to patients and their carers, disseminating specialist knowledge enabling individuals to try and adapt to changes in their lives caused by chronic pain. Providing, promoting and maintaining a support and advice service to patients and their carers, to include provision of information giving from diagnosis through to all aspects of chronic pain, in liaison with relevant healthcare professionals. Providing clinical supervision and consultation as appropriate. Contributing to service evaluation, research and policy development. Developing the service in a structured interdisciplinary way, initiating on-going change and progression in the role of a Clinical Nurse Specialist. Developing interdisciplinary clinical management protocols and working within them. You will be working with both adults and young people, within what is a very interesting and rewarding area of care that is continuing to develop further.

We are looking for a skilled and confident individual who has experience of complex assessments (including assessment of risk), decision making and who is adept at building relationships with service users and professional partners.

The successful applicant will work across the service in order to help identify individuals who present with vulnerabilities that may include mental health issues, neurodevelopmental disorders, substance misuse and who may present with a degree of risk.

As a Liaison and Diversion Practitioner, some of the responsibilities you can expect will include:
• Proactive screening of cases in police custody or at court.
• On-the-day triaging and assessment - including full mental state examinations, risk assessment and gathering of background history for individuals presenting with a wide variety of established or suspected needs. These can include anxiety, challenging behaviour, serious mental illness, personality disorder, learning disorders, social communication and language difficulties, substance misuse and associated social issues within police custody.
• Assisting detainees who present with urgent and non-urgent clinical issues that may include onward referrals for a Mental Health Act assessment.
• Providing advice, information and support to custody/court personnel.
• Provision of written assessment summaries to police, court and other relevant agencies.
• Liaising with CAMHS, YOT practitioners or other relevant agencies for young people.
• Recording data for regular auditing and evaluation.

Welcome to CNWL Community Eating Disorders Service for Children and Young People. Eating Disorders is a specialism within children’s mental health, therefore finding candidates with rich experience is really important. It would be fantastic to have staff ‘ready-to-go', but we appreciate the need to grow and develop people in this field and improve young people’s access to care.

In this role you will provide senior nurse leadership and line management and supervision to allocated members of the CYP nursing team. You will also contribute to the wider nursing initiatives within the All-Age Eating Disorders Service and the wider CAMHS Eating Disorders Service line, as appropriate.

What do we do?

The recommended evidence-based intervention indicated for this patient group is Family-Based Treatment (FBT) for eating disorders which all clinicians in the team are trained to deliver. We offer systemic therapy, for those who an individual therapy approach is more appropriate. In addition, we also offer a parent group to support those caring for a young person with an eating disorder. We link with CNWL CAMHS for joint work (for co-morbidities) and other agencies as necessary.

Who are we?

The team consists of nursing, psychology, assistant psychologists, systemic psychotherapists, a dietician, paediatrician and psychiatry. We are expanding into the use of systemic practitioners, Occupational Therapy, and developing nurses to the Advanced Clinical Practitioner level. The leads of the service are a Clinical Team Lead, Consultant Psychiatrist, and is linked to the ED service which is overseen by Head of ED and a Clinical Director for ED. More recent developments are better links with the Adult Service, development of an Intensive Community Team and new projects with our neighbours at Chelsea and Westminster Hospital.



Where are we?

Our main base is in Chelsea (next to Chelsea and Westminster Hospital). Thanks to the heavy NHS influence many local cafes, restaurants and bars give discounts ranging from 10-20% to all NHS staff.

Our second site is newly opened and based at Roxborough Road which has the tube station of Harrow-on-the-Hill very close by, and clinicians typically work there once or twice per week with the option for this to be frequent if preferable to the staff member.

JOB PURPOSE
• The post holder will be responsible for the assessment and treatment of children, young people and their families affected by eating disorders, as part of a multi-disciplinary team.
• To be responsible for the continued development of the treatment pathway from the point of referral through to discharge.
• Provide liaison to GPs, Paediatricians, CAMHS Teams and other stakeholders, ensuring ease of access and support for children and young people with an eating disorder.
• To work closely with the Adult Eating Disorder Service to ensure a seamless transition into Adult Services from Child and Adolescent Services when required.
• To provide effective nurse leadership, expertise, advice and support to the team and ensure the provision of high-quality care to children and families.
• To act as a role model and highly competent practitioner
• To supervise other nurses within the service and contribute to the wider nursing initiatives within the All Age Service and the wider CAMHS/Eating Disorders Service line, as appropriate.
• To act as a designated resource to children and families, and other professionals caring for them.
• To facilitate the continued development of the Community Eating Disorder Service, as part of a multidisciplinary team.
• To promote an innovative and progressive attitude to the continual improvement of patient care through research and evidence-based practice.
• To provide clinical interventions relevant to the CAMHS patient group such as: FBT, supportive clinical management, DBT skills, CBT-E skills and other evidenced based models of care
• To work with families as appropriate
• To provide consultation and advice to referrers; children & young people and their families.
• To work autonomously within professional guidelines and exercise responsibility for the systematic governance of systemic practice within the service.
• To take an active role, as and when necessary, in the development of specialist provision of specific projects and services in collaboration with other members of the MDT and in response to service need.
• To take responsibility for contributing to research, audit, policy and service development in line with clinical governance.
