We have a new opportunity for a full-time Senior Social Worker in our Young Adults Team, working 37 hours a week. Our Young Adults Team consists of Social Workers and Adult Support Co-ordinators who collaborate closely in a highly supportive environment. The team is led by 4 Senior Social Workers and the Team Manager. Team members bring a wealth of experience and knowledge, supporting the professional development of newer members. What will you be doing? Part of the role involves working alongside Childrens services with young people under 18 years to prepare them and their families for adulthood. You will support young adults through change, encourage progression, and acknowledge individual, family, and community strengths through a transforming lives approach. You will work alongside Adult Learning Disability services, particularly health colleagues, and closely with Education services. Legal and Court work is a core part of the senior role, particularly concerning MCA Dols. Senior Social Workers hold a small caseload and line manage 2-3 team members. Senior Social Workers also deputise for the Team Manager when needed. Knowledge and experience of working with people with a learning disability is desirable but not essential. Advanced working knowledge of the Care Act 2014 and Mental Capacity Act is important as well as direct experience in managing safeguarding and complex casework in previous roles. Line management and Court experience will be a significant advantage for the role, but working alongside 3 other experienced Senior Social Workers means there is scope for professional development in this area. The post holder will provide direct personal nursing care, primarily in the patients home and on occasions in other settings such as Community Hospitals, Residential and Nursing homes and Eden Valley Hospice. The post holder will provide practical nursing care, emotional, social and psychological support to individuals with palliative care needs, their families and carers. To lead and inspire the team to deliver a high quality and effective recovery service for service users and carers. To provide clinical leadership to ensure where appropriate the service will work effectively alongside other services, to manage risk and prevent hospital admission for service users with the most complex and acute needs. To undertake and support the specialist ongoing assessment of physiological and/or psychological functioning for service users with the most complex and health and wellbeing needs. This may include: a. The use of standardised assessment tools, i.e. the Cluster Allocation Support Tool (CAST), KGV. b. The Recovery Star. c. History, strengths and aspirations. d. Mental state. e. Impact of culture and diversity. f. Functional needs. g. The needs of family and carer. h. Evaluation of risk. i. Physical health. j. Complicating factors. k. The interventions and treatments required to enable positive change. l. Social care needs. m. Safeguarding and public protection. n. Capacity under the Mental Capacity Act.
