To work as a member of a corporate workstream for an identified District of High Schools and Primary schools. To manage the defined workload, using a public health approach to provide and maintain a high quality service that promotes the physical and mental health of the school aged population and their families.
To lead to the delivery of the Healthy Child Programme 5–19 and co-ordinate the delivery of public health interventions for the post holders assigned schools under the direction of the Team Leader. This will include occasionally supporting immunisations as we have an immunisation team, health screening/surveillance health assessment and a requirement to support the health needs of any vulnerable children/young people within the caseload. Work in collaboration with partnership agencies to address the needs of vulnerable children including those on the child protection register.
To work in partnership with members of the wider Right Start and School Nursing Team for the designated area.
Our integrated model brings together key professionals from Health, Education, Clinical Psychology and the Special Education Needs and Disability team (SEND). Being based in our children’s centres you will be delivering key services in the heart of the community.
The Public Health Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies
Lead on the delivery of the required health assessments and reviews
Working in partnership with other key stakeholders to safeguard and protect children and young people.
Working collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family
Provide training where necessary to young people, parents, teaching personnel and other professionals.
Provide signposting to other services and agencies, referring and liaising appropriately.
Take and act upon referrals from other agencies and services, as appropriate.
Provide a confidential support and advice service; that is easily accessible, for young people within a high school or other venue on a regular basis.
Utilise opportunities to promote healthy lifestyles, positive mental health and the public health agenda.
Ensuring children not in employment, education or training, or children educated at home receive the universal offer.
Ensure performance data is collated in a timely manner
Cover for named team leader as required.
