10. Update individual patient records on the Trust Electronic Patient Record (EPR) to clarify what action has been taken.
11. Respond promptly to any referrals that are received via the Safeguarding Adults Team where children may be involved. Take any necessary action to share relevant information with Oxford Health and Social Care professionals and update EPR.
12. Prioritise and process DoLS applications and forward to the relevant agency promptly.
13. In the absence of senior staff, take responsibility for referring cases of concern to other relevant professionals. This may include referrals to outside agencies.
14. Maintain and record information and statistics with regard to strict confidentiality. Respond to requests for data when required within the Trust Information Governance procedure.
15. Monitor attendances for any instances of regular attendance or themes.
16. Assist in ensuring that the Child Protection, Children We Care For and children known to the Youth Justice and Exploitation Service lists are maintained and updated on the shared drive and that an alert ‘Flag’ is added or removed appropriately and in a timely manner.
17. Assist in responding to Initial Child Protection Conference invites and record on EPR and the database. Check invitation responses are kept up to date.
18. Assist in uploading Domestic Abuse Reports involving pregnant women onto the relevant patient record and advising the appropriate Community Midwifery Team.
19. Carry out photocopying, scanning and other admin duties where required.
20. Help manage and respond to shared email accounts.
21. Receive incoming telephone calls and respond to enquiries in a timely and appropriate manner.
22. Attend and contribute to daily team meetings. Take notes/minutes of meetings as required by the department.
23. Obtain medical records as required and track them appropriately.
24. Work independently and as part of the Safeguarding Team. To prioritise and organise own workload, being proactive in offering support to the rest of the Safeguarding Team.
25. Cover duties of admin staff and provide cover during times of staff shortage or absence to ensure provision of an effective service.
26. Adhere to and monitor your own statutory and mandatory training as set by the Trust and assume your own responsibility with regard to health and safety.

Staff Management:
• To be responsible for the line management of the Clinical Genetics Secretariat and admin team, providing first level EPR support, ensuring that adequate cover is provided in all areas of activities.

To deal with telephone contact with members of the public appropriately and sensitively in situations which may be stressful for both the patient and the staff member involved.
• Development and implementation of new or revised administration policy and procedures.
• Taking and relaying as appropriate, telephone enquiries and messages, using initiative and prioritising skills when disseminating information.
• To assist the Clinical Unit Manager and Patient Access Manager in the writing of reports and documents.
• To work closely with the Clinical Unit Manager and Outpatient Access Manager by undertaking and assisting project management directly related to the Department. This role will require analytical skills as well as being a capable communicator with excellent organisational ability.

• To act as role model.
• To carry out surveys/audits as requested.

• To receive and make telephone calls and take appropriate action/discuss with appropriate staff maintaining a friendly, professional and courteous manner at all times. These conversations can often be of a distressing nature.
• Maintaining physical and electronic filing systems as well as other general ad hoc office management procedures.
• Must exercise judgement to analyse and resolve problems.
• The post holder is required to work unsupervised and is expected to plan and organise his/her own time.
• Provision of information and advice to patients and relatives about admissions and appointments exercising judgement when dealing with patient enquiries; analyse and resolve patient problems.
• Be responsible for communicating cancellations to all relevant parties with the reasons for cancellation including patients and relatives.
• Provide a courteous and responsive assistance to Clinicians and other Trust personnel and to those visiting the Trust from outside agencies.
• Will be asked to show flexibility should the need arise in covering for other personnel when on leave/sickness etc.

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager To support in the publication of the IPC annual statement in conjunction with the practice manager To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice To develop, implement and embed health promotion and wellbeing programmes To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF As required, to diagnose and manage acute and chronic conditions Provide wound care (ulcer/Doppler, etc.) to patients To review patient medications to enhance compliance To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages To co-ordinate the provision of travel medicine services To request pathology services and process pathology results as required To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required To maintain accurate clinical records in conjunction with extant legislation To ensure SNOMED CT codes are used effectively by all members of the nursing team To ensure the appropriate maintenance and use of chronic disease registers To ensure compliance with the practice chaperone policy To assist GPs with minor surgery when required To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To recognise, assess and refer patients presenting with mental health needs To implement vaccination programmes for adults and children To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) To support the clinical team with all safeguarding matters, in accordance with local and national policies To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately To deliver opportunistic health promotion where appropriate In addition to the primary key responsibilities, the Practice Nurse may be requested to: Develop and review audit protocols and process for the practice, effectively utilising the audit cycle Develop, review and implement nursing protocols in conjunction with the partners Support all members of the nursing team, providing guidance when necessary, conducting appraisals and training needs analyses as required Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the practice in order to improve patient care Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA) Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patients Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation

The role includes undertaking Healthy Child Programme transitional assessments and planning, delivering and evaluating public health initiatives through school and community-based health promotion activities. The role will require you to adhere to Safeguarding policies and actively engage under the supervision from a SCPHN in safeguarding processes and meetings. You will work in partnership with other agencies and develop, participate and maintain interagency networks. Provide NHS general medical services to patients registered on the practice list, delivering core primary care services as defined by the local ICS and NHS England requirements. Deliver comprehensive clinical care to patients, including consultation sessions, telephone triage, handling correspondence, reviewing results, conducting medication reviews, clinical administration, safeguarding, adhering to infection control policies, completing medical reports, and attending surgery meetings. Prescribe safe and effective medicines management which conforms to: NICE guidance and DH directives Good Prescribing Practice as defined by British National Formulary (BNF) guidance from Royal Pharmaceutical Society of Great Britain on safe and secure handling of medicine Operate safe management and regular review for all patients receiving repeat prescriptions Controlled Drugs legislation Shared protocols agreed between the OCCG and other NHS providers Locally specified Patient Group Directions Willing to cover duty doctor sessions on rotation basis. Requirement to take on patient list and be responsible for covering your own lab results and Docman during your scheduled admin allocation each week. Maintain the highest personal and professional standards as outlined by the GMC, prioritising individual patient care. Collaborate with the Practice Manager and Clinical Lead to support the development, execution, and review of the surgerys action plan, focusing on list growth, patient satisfaction, QOF, enhanced services, KPI targets, resource management and cost control. Be supportive of any complaints policy and procedure to ensure proper investigation and appropriate follow-up action in the event of a complaining being made about any aspect of the service provided or any personal provider of the service Work closely with other GPs and the clinical team to provide patient care and actively participate in team practice meetings. Record and report significant events and clinical governance issues to the Clinical Lead or Practice Manager. Perform additional duties as reasonably requested.
