Hepatobiliary Medicine at the Royal Free Hospital



The Royal Free Hospital has, for a long time been synonymous with research and clinical excellence in the field of hepatology, and with the current near exponential increase in the incidence of liver disease, we aim to remain at the frontier of patient care, research and education.

The Royal Free provides secondary and tertiary Hepatology services. There is excellent access to ITU, a 24 hour TIPS rota and 24 hour consultant led GI bleeding rota. The liver transplant service at the Royal Free has been established for 30 years. The number of transplants performed has increased recently and currently stands at approximately 140 liver transplants per year. We have a live donor program. We are a tertiary centre for patients with hepatobiliary and pancreatic malignancy and the full range of surgical, locoregional and oncological treatments is provided for these patients. Dedicated clinics are established for viral hepatitis, liver metabolic disease, primary hepatocellular malignancy, autoimmune liver disease, PBC, PSC, NAFLD, Vascular liver disease as well as clinics for general hepatology and liver transplantation. A number of very active clinical research programs run with themes such as clinical and experimental portal hypertension, acute and acute on chronic liver failure, liver support and liver transplantation, non-invasive fibrosis assessment, clinical trials of new anti-viral agents, metabolic studies of hepatic encephalopathy, pruritis pathways and examining the effect of remote ischaemia pre-conditioning on liver graft function.

A training contract and areas of particular interest would be identified prior to commencement of the post but the following areas could be provided at UHBW:
• Management of complex viral liver disease
• Decompensated chronic liver disease and alcoholic hepatitis
• Portal hypertension
• Liver cancer
• Long term follow-up of the transplant patient and transplant assessment
• Acute liver failure

• Other training experience available: ERCP/EUS, research meetings, audit/governance

The trainee would have supervised clinic experience, through between 2 and 3 specialist Hepatology clinics weekly (including independently-run ‘Hot-clinic’ for urgent referrals). Direct consultation with the supervising consultant would be expected at the end of each clinic; a half hour discussion session will be included. The trainee would participate in ward cover of Hepatology patients in conjunction with a consultant Hepatologist (there is always hepatology cover) and share responsibility for daily senior hepatology in-reach to AMU/ED/ITU and additional referrals.

It is expected that the trainee will present regular case summary and literature review (i.e. case-based) on a regular basis at the weekly Friday lunchtime Hepatology meeting. A log of these presentations will be kept and form part of the assessment of completeness of training.

This is anticipated to be a fixed-term appointment for 12 months, with the possibility of extending the post if the appointment is successful. There is one post, and the successful candidate can choose either a gynaecology-focused or obstetric-focused job.

The post-holder must be a senior clinical fellow with specialist interests, clinical skills and knowledge in either high-risk obstetrics, maternal medicine or acute gynaecology and gynaecology ultrasound scan.

We have an exciting opportunity for an enthusiastic, motivated and ambitious candidate who is looking to build a career in aseptic manufacturing, to join our busy chemotherapy manufacturing team at Barts Health NHS Trust. The post holder must be keen to learn and develop with excellent communication and interpersonal skills and will undertake a wide variety of roles within the unit including, but not limited to aseptic preparation, stock management, training and in-process checking. The post holder will be based at St Bartholomew’s Hospital supporting our well established, dedicated chemotherapy manufacturing unit.

You will participate in training initiatives to meet both individual and departmental goals. Once fully trained and we will look to enrol you on the Science Manufacturing Technician Level 3 course. Applicants should hold an NVQ Level 2 in Pharmacy Services (or equivalent). Previous experience of working in aseptic production is desirable but not essential as full training will be provided. An opportunity has arisen for two senior clinicians to join the new NELFT Dialectical Behavioural Therapy for Adolescents (DBT-A) service. This is part of an exciting expansion of CAMHS specialist services within NCEL that align closely to national, regional and local health and care goals and objectives. The role is a 1-year fixed term opportunity (or possible secondment) with the aim of securing further extension into a permanent role with the successful implementation of the service. The post holders will work closely with the Principal DBT Therapist, leadership team, clinicians and managers across Tier 4 units, CAMHS and crisis teams, the NCEL Provider collaborative and other stakeholders.

The post holders will be responsible for assessing, managing, planning, and delivering care to their allocated patients, maintaining professional and legal accountability. They will support their peers, work effectively both independently and as part of a team, and supervise pre-registration students. A commitment to collaborative working is essential, with a focus on actively supporting and liaising with other health and social professionals and agencies.

The post holder will be responsible for ensuring adherence to the DBT model within the service. The NCEL DBT service offers specialised DBT assessments and interventions to young people and their families referred by CAMHS services. The service is expected to coordinate with CAMHS teams to ensure clear transitions for young people entering or leaving the DBT service and, when appropriate, to support transitions to Adult Services. The post holders will deliver a highly specialised clinical psychology service to these young people and their families who require DBT, manage their own caseload, and provide specialist supervision, consultation, and training to team members. Probationary Period

High Cost Area Supplement - Outer London

Additionally, this role includes a High Cost Area Supplement of 15% of the basic salary, with a minimum of £4,313 and a maximum of £5,436 per annum, pro-rata for part-time staff.
1. Supporting the DBT Lead’s efforts in monitoring clinical outcomes, conducting audits, engaging in research, and driving quality improvement initiatives.
2. Ensure the systematic provision of a highly specialised clinical service to service users of the NCEL DBT Service, with a focus on delivering Dialectical Behaviour Therapy (DBT) across all sectors of care. This will be done in alignment with the principles of CYP-IAPT.
3. Provide highly specialised assessments and therapy, while also offering advice and consultation regarding the care of service users to non-psychologist colleagues, professionals from other agencies, and other non-professional caregivers.
4. The post-holder will offer specialist work as Clinical/Counselling Psychologist of the NCEL DBT-A Service delivered from a central base in Barking and Dagenham with potential outreach to Community CAMHS clinics in various localities as required.
