The Hillingdon Hospital wishes to appoint a Senior Clinical Fellowto join the General Surgery department for a 6 month fixed-term contract (03/04/24) - 01/10/24).

The job covers a wide variety of general surgery and colorectal surgery. It is expected that the candidate will be closely involved in the colorectal service providing support in both elective colorectal surgery and out-patient commitments.

This is an excellent senior post with lots of scope to improve both elective general and colorectal surgery as well as emergency surgery.

The role will focus primarily on colorectal and general surgery and the post holder will be required to partake in the on-call rota. The post is essential to meet the ongoing colorectal and General Surgical expansion.

The department has 11 substantive consultants and five locum consultants with middle grade and juniors to support them. This post does not have educational approval and is therefore a service post.

Additionally the candidate will participate in elective general surgery operating lists including elective colon resections, laparoscopic cholecystectomies and both laparosocopic and open hernia repairs.

The Hillingdon Hospital, which is situated near Uxbridge in the south of the London Borough of Hillingdon, has over 600 beds. In the north sector, Mount Vernon Hospital is also managed by Hillingdon Trust.

The hospital offers a wide range of specialities, treating 44,000 in-patients a year. Amongst these services is a large A&E department, which deals with approximately 100,000 attendances per year, an Acute Cardiac Unit and Intensive Care Unit with an adjoining four-bedded High Dependency Unit.

Five years ago the A&E department underwent a redevelopment programme. Facilities exist for assessment, treating and discharge of minor injuries. In addition to the refurbishment of the resuscitation area, there exists a five-bedded Observation ward. A Medical and Surgical Assessment Unit is on Fleming ward to which all emergency admissions are directed. There is already consultant physician presence and responsibility in place for medical emergencies.

Hillingdon Hospital has seven operating theatres including a dedicated Orthopaedic theatre. The hospital has an excellent Day Care Unit adjacent to the operating theatres comprising of 23 beds. At Mount Vernon Hospital site there is a new treatment centre providing modern Orthopaedic and General Surgical wards, theatres with laparoscopic facilities and outpatient consulting rooms.

The Division of Support Service offers excellent diagnostic facilities in Endoscopy, Pathology, Radiology and Medical Physics at both sites.

The Waiting List Co-ordinator will manage the waiting list team. It carries specific responsibilities for ensuring the systems and procedures used by the team are operating effectively and within the context of the Trusts policies and priorities. The post holder will be accountable for ensuring that patients at risk of breaching are managed under the supervision of the appropriate Directorate Manager for the relevant specialty. Job Purpose The post holder is expected to understand and apply the Patient Access Policy to their everyday activity, ensuring that patients are managed fairly and contributing to the ability of the Trust to maintain waiting time targets. The Waiting List Office administrative service will contribute towards a systematic approach to delivering the NHS Plan commitments for access to elective and day case surgery, specifically the drive to guarantee maximum waiting times and introduce Patient Choice through booking systems. The sensitivity around clinical and management priorities will require the post holder to work with and have the confidence of Consultants and Directorate Managers. Communication with patients will be proactive, inviting discussion and consultation around their preferences and availability. The Liaison Psychiatry Service provides a 24 hour, 7 day a week in and out of hours comprehensive service to patients, clinicians and carers at the local District General Hospital (DGH). This includes specialist mental health assessment and intervention to people of all ages from the age of 18 who present or, are admitted to the DGH. We provide a specialist consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases, and providing both formal and informal education to qualified and unqualified staff, and offer a broad range of specialist psychosocial treatments for individual inpatients. Staff within the service are also representing the Devon Partnership Trust in the general hospital setting and will play a vital role in communicating and maintaining a good working relationship between the two organisations. Through consultation, assessment and interventions, the Liaison Psychiatry Service aims to - Arrive at a formulation which will identify the correct care pathway and ensure this is implemented Improve the quality of mental health care for all patients Improve quality of life for both patients and carers Identify and manage risk Reduce the length of stay in the general hospital Prevent hospital admissions Reduce the number of incidents and complaints Measure improvements of health outcomes Determine service needs A key aim of the service is to contribute to the acute care pathway, ensuring that those persons who present to the DGH receive a timely specialist mental health assessment by a specialist mental health worker. A significant proportion of these presentations will be those who present with acts of self harm and suicidal behaviours. In Torbay, our service incorporates the following areas: Acute Care Pathway (ACP) Ensuring a timely response to people who attend the Emergency Department and facilitating discharge from the Acute Medical Unit and other wards. Inpatient Assessing and reviewing patients who are kept on caseload for a period of monitoring whilst they remain in-patients in Torbay and South Devon DGH. This will include dementia, delirium, depression, eating disorders, psychosis, medication management & support for patient and ward in challenging situations, on-going risk assessment and monitoring. Integrated Psychological Medicine Service (IPMS) Ensuring pathways are in place to allow access to the right treatment at the right time for those who present with both complex physical and psychological needs. There are high rates of mental health problems among people with long-term medical conditions and people with severe mental illness have a reduced life expectancy which can be due to physical health issues.
