The Role

We are seeking a Senior Key Worker in Mid Essex to carry out the statutory assessment of Young Carers, to implement support for Young Carers (aged 0-18) and young adult carers (16 - 24) and their families, through a tiered key worker approach to level of need, and to provide individualised support in relation to their caring role, raising achievement and aspirations and to allow each Young Carer to have the same life chances as their peers. As a community-based worker you will be meeting families face to face, completing assessments, and providing individual and groupwork support, as well as line managing a team to support Young Carers and their families.

Accountabilities

• To work with young carers and other agencies to develop and coordinate the delivery of a whole family support plan, agreeing the support to be provided, the co-ordination of this support and time limited objectives for the family, reviewing progress and restating goals where appropriate.
• To take up referrals and work with Essex young carers in order to identify, assess, understand and meet their needs, and to allocate Senior Key Worker or Key Worker support within the remit of the support required and to manage the workload of Key Worker Support resource within each quadrant.
• To line manage, supervise, and monitor the workload of Young Carer Key Workers within the relevant quadrant.
• To facilitate through youth work approaches the learning and development for Essex young carers to enhance their future development or meet identified needs, such as provision of specific training opportunities, focussed peer group support and/or targeted support workshops.
• To promote young carers self-awareness, confidence, and participation, encouraging their involvement in the design of their young carers service and supporting them in evaluating the impact of the service upon their own situation.
• To ensure that Essex County Council policies, procedures, and practice for safeguarding and promoting the welfare of children and young people are implemented effectively.
• To work flexibly and deliver services as directed.

The Experience You Will Bring

• A relevant professional qualification which is focussed on working with young people (e.g. LDSS, Social work, Youth and Community, Teacher, Careers) or equivalent or demonstrably equivalent experience
• A broad understanding of the issues affecting young carers and young people, opportunities for learning and the services provided by the partner organisations
• Experience of working with young people on a one to one basis and in groups
• Excellent communication skills.
• Experience in effectively working collaboratively with other agencies at a local level
• Experience of successfully involving young people and families in decision-making

• Be flexible in their approach and have excellent time management skills and ability to work evening, weekend, and residential work to respond to young people's needs

In addition to working directly with patients identified by GPs and other primary care staff; will include support with the management of long term conditions where anxiety and depression can often be misunderstood as an exacerbation of symptoms; and support to colleagues through shared learning and consultancy regarding effective management of challenging cases.

The role requires you to work with service users and carers, close to their home, to deliver highly accessible and responsive interventions via flexible referral routes. The key principles of the team are to offer interventions within primary care to promote a truly integrated, holistic approach to care.

· Provide professional clinical leadership at multi-disciplinary meetings i.e.referral/review

/business, professional or safeguarding meetings.

· Assess and review care and support and supervise lower banded staff in this process as required.

· Be responsible for the administration of medications (nurse applicants), assessment of side effects, review concordance and clinical effectiveness of medication regime in line with Trust policy including:

- Providing education around the medication prescribed, side effects, actions and aids tocompliance.

- Providing education and support to develop service user’s understanding of Physical Healthcare and training packages forstaff

- Providing practical support, guidance and education for service users, carers and other professionals asrequired.

· Provide specialist assessment and intervention commensurate with skills and training with colleagues whererequired.

· Lead and participate in formulation, delivery and review of individualised evidence based care/care packages according to NICE guidance including non-medicationstrategies.

· Lead in the continuing development of a high quality service and contribute to local emergency plan, team risk assessment etc. that is underpinned by evidence based practice, care pathways/packages and NICE and DHGuidance.

· Create and maintain working relationships with statutory and non-statutory agencies involved in the provision of Physical Healthcare and Mental HealthCare.

· Participate with the Team Manager in the recruitment of employees in accordance with Trust Recruitment Policy and NHS Safer Recruitment Guidelines. Ensure that appropriate corporate and departmental Induction is undertaken immediately following appointment and that staff records that are retained are accurate andup-to-date.

· Develop a Primary Care Mental Health Operational Clinical Policy with EPUT and primary care colleagues andstakeholders.

· Work closely with and assist the Team Manager with development and implementation of operational/clinical policies contributing to the formulation and review of appropriate policies as required.

· Deliver training where appropriate for otherprofessionals.

· Complete and return timely and accurate statistics to the InformationDepartment.

· Ensure compliance in meeting agreedOutcomes/KPI’s.

· Identify aspects of the service which need to be improved and implement appropriate changes in conjunction with the MDT, EPUT management and primary careteams.

· In conjunction with the Team Manager, work within the available budget of the service and attend regular Performance Management meetings asrequired.

· Complete highly complex nursing needsassessments.

· Ensure that the service, through the implementation of performance management, meets all key performance indicators and targets identified by the Trust and adhere to local and governmentinitiatives.

· Be able to use computer systems including Word and E-mail for effective communication ofinformation.

· Provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality oflife.

This is a 12-month fixed term contract or secondment opportunity.

Children and Families is recognised as a service in which social work can develop and prosper, with a culture of collaboration, integrity and compassion; promoting development and wellbeing of children, young people and their families; protecting them from neglect and abuse and working together with partners delivering a range of early help, family support and effective social work interventions, which build resilience, remove barriers and enable children and young people to look forward to a brighter future.

Family Centres

Essex Family Centres provide support to fieldwork teams in delivering assessment and intervention across a range of social work issues such as sexual abuse, domestic abuse and trauma. Where required, parenting assessments including assessment of parents with additional needs, can be completed as directed by the court or under Public Law Outline. Family centres also deliver a variety of interventions to bring about positive change within families, help adults provide positive parenting, and increase the resilience of children so that they can stay with their families where it's safe to do so.

The social workers within the North Essex Family Centre Service are responsible for managing a caseload of assessment and specific targeted intervention and group work, to achieve positive and improved outcomes for children.

The role

This is a fantastic opportunity to join the North Essex Family Centre team. The opportunity will provide the right candidate with the opportunity to develop knowledge, skills and practice across a wide range of social work issues and to further develop skills in assessment. The senior practitioners within the Essex Family Centre Service are responsible for managing a caseload of specific targeted intervention work, to achieve positive and improved outcomes for children. The post holder is responsible for bringing to bear the highest possible standards of practice ability and skills to work in line with policies and procedures.

Senior practitioners within the team generally have a specialist area of practice interest which they share within the team and across the wider service, applicants therefore are required to be enthusiastic about developing their social work knowledge across a wide range of areas, for example working with trauma, domestic abuse and sexual abuse to name a few.

Senior practitioners will have some supervisory responsibilities for the development of other staff and students within the team and to deputise for the manager in their absence.

The Experience You Will Bring

• Diploma/MA in Social Work, CQSW, CSS or equivalent and current Social Work England registration
• Must be able to work in a relationship based way and be a supportive team member.
• Must be able to write to a high standard and demonstrate analytical skills
• Experience of giving evidence in court preferable
• Desirable to have a practice educator qualification / experience of supporting and mentoring others including ASYE's through their first year in practice
• Evidence of continuing professional development
• IT/Word Processing Skills
• Evidence of recent practice at an experienced social worker level, supported by comprehensive working knowledge and experience within the field of practice in Children Services, preferably with experience of Child Protection work including section 47 investigations and assessments.
• Considerable, recent experience of communicating with children of all ages and working in partnership with parents and/or carers.
• Experience in planning activities and programmes of intervention for children, young people and parents.
• Detailed knowledge of current legislation pertinent to Child Protection (e.g. CA1989, CJA1991), Guidance and Regulations
• Knowledge and understanding of child development, relating theory and practice.
