Job Description Nurse Reports to Clinical Team Leader Job Summary The post involves working as a member of a multi-disciplinary team providing specialist palliative care and support for children and young people from 0-35 years. The post holder will be expected to work without direct supervision, providing a high standard of holistic care to children and young people with a variety of complex health needs. Effective communication skills are required to support children, young people and their families during their stay in the hospices. The hospices provide care over a 24-hour period and the post holder is expected to work flexibly across all shifts. Key Responsibilities To provide a high standard of palliative care for children, young adults with complex health care needs and members of their families. To develop and maintain the clinical skills required to deliver complex care using practice based on best evidence. To work as a member of the multi-disciplinary Care Team providing care, advice and support in line with professional accountability. To provide supervision and support to other members of the Care Team, valuing the skills and qualities of all Team members, sharing expertise and learning new skills freely and willingly. To be aware of the safety and safeguarding needs of children, young adults, families and other staff and to comply with: The Children Act 2004 DOH No Secrets Guidance on developing and implementing multi-agency policies and procedures to protect vulnerable adults from abuse (Mar 2000) Health and Safety at Work Act 1974 NMC code and guidelines Key Tasks Undertake clinical nursing assessment of the child /young person and develop comprehensive care plans in partnership with family members ensuring practice based on best evidence. Clinical assessment will include evaluation of symptoms and review of symptom management plans in collaboration with the wider multi-disciplinary team. Deliver a high standard of clinical nursing care which is based on best evidence. Communicate effectively and sensitively with children, young people and their families. Be responsible for the safe administration of medication following the Medicines Management policy/SOP and NMC guidance. Lead and co-ordinate a small team during a shift under the supervision of the Site Coordinator organising the care and activities for the shift delegating to non -registered staff appropriately. Maintain accurate records of all types, whether written or electronic and maintain confidentiality at all times. Maintain an up to date knowledge and awareness of issues around the safeguarding of children and young adults, ensuring that appropriate steps are facilitated through effective communication across multidisciplinary agencies. Be responsible for developing and maintaining own skills identified in the Clinical Competency Framework with support from the Practice Education team. Maintain agreed governance processes through incident reporting and risk assessment. Participate in audits as required. Liaise with outside agencies involved in the care and support of the children and young people and their families. Attend meetings as required, including staff meetings and support sessions. Participate in the orientation of new staff and students; act as mentor if requested, on completion of the Mentorship course. Be responsible for personal professional development, maintaining awareness of current developments in palliative care for children, young people and families. All staff are reminded of their responsibility to adhere to Trust and Departmental Infection Protection Policies, including PPE use, cleaning and decontamination of the environment and equipment in order to protect their own health and that of service users, other employees and visitors. The post holder will provide leadership and management of the care team to ensure effective delivery of care for children and young people with life limiting and life threatening conditions across both Naomi House and Jacksplace. Key Responsibilities: To lead the provision of a very high standard of care and support for all children, young people and their families attending Naomi House and Jacksplace. To provide clinical leadership to staff at Naomi House and Jacksplace over a shift period. To liaise fully with the Clinical Team Leaders/Care Managers and other Site Co-ordinators. To support the Clinical Team Leader in the line management, development and support of a team of nurses and carers. Managerial Responsibilities To lead and coordinate the day-to-day provision of care to the children, young people and their families in both houses on any clinical shift. To be responsible for ensuring an holistic assessment is completed for all children and young people to enable up to date care plans incorporating all necessary risk assessments. To allocate staff appropriately on each shift to ensure effective use of resources. To ensure excellent and timely communication with the Clinical Team Leader in house or on call and to escalate any concerns appropriately. To line manage staff as delegated by the Clinical Team Leader, participating in appraisals and day to day staff management. To provide support and supervision to staff and students in the care team, across both hospice settings. To work in partnership with the hospice multidisciplinary team as well as any other agencies involved in the care, to ensure the delivery of seamless services. To maintain an up to date knowledge of organisational policies and ensure that these are adhered to across all areas of work with children, young people and families. To assist in promoting the economic, efficient and equitable use of resources. To ensure that Health and Safety requirements are met and that risks to service users and staff are assessed and action taken to minimise these. Clinical Responsibilities To deliver a high standard of individualised holistic care and support to children, young people and their families, promoting empowerment, advocacy and partnership. To ensure that clinical documentation is up to date and adheres to record keeping standards. To manage staffing and allocate resources across both hospices according to assessed acuity on each shift. To support the Practice Education team to embed evidence based nursing care. To monitor standards of care, participating in audit and required actions. To maintain agreed governance processes through incident reporting and risk assessment. To communicate effectively with all members of the hospice multidisciplinary team, as well as any other agencies involved in the care and support of referred children and young people. To recognise the need for psychological support and work appropriately alongside the Family Support Team. To maintain an up to date knowledge and awareness of issues around the safeguarding of children and young adults, ensuring effective communication across multidisciplinary agencies and adherence to agreed safeguarding policies. We are looking for an enthusiastic Rehabilitation Assistant to join the Sutton Health & Care Maximising Independence Service. The Maximising Independence Service comprises of community Frailty, Falls Prevention and uni disciplinary physiotherapy pathways. The services include community centre based Frailty MDT Clinic, Falls Prevention Groups and home visits.

You will be part of the MDT team that comprises of a Nurse, Physiotherapists, Occupational therapist, Assistant Practitioners, Rehabilitation assistants and Administrative staff. You would also be expected to support with administrative duties within the team where needed.

You, as part of the team, will work closely with the other community services within Sutton Health and Care that help to prevent admission from the community, accelerate discharge from emergency department and acute medical wards and provide supported discharge through bed and home-based intermediate care. You will be supported by the Clinical Manager and the clinical staff.

The successful candidates will be a ‘generalist with a special interest model’ of placing ward based acute and sub-acute ward based general internal medicine as a core function while maintaining a specialty interest. The candidate will be welcomed into a well organised department providing a wide range of services in ward based General Internal Medicine and specialty and sub-specialty level services in Diabetes and Endocrinology.
