The Trainee Mental Health and Wellbeing Practitioner will: attend all elements of the training programme of study and progress successfully through assessments of competence and attainment work in an adult community mental health service when not completing the programme of study develop and practice psychologically-informed interventions under supervision, working directly with adults in the community support collaborative care planning, alongside other members of the multi-disciplinary team work closely alongside a team of mental health professionals to deliver a set of brief wellbeing-focused psychologically informed interventions - not therapy - for example: Behavioural Activation and Graded Exposure using the GOALS programme Problem-solving Improving sleep Recognising and managing emotions Guided self-help for bulimia and binge-eating Confidence building Support with medicines management The Trainee Mental Health and Wellbeing Practitioner will be required to: Demonstrate the ability to work at degree level Have experience of work interacting with the public, working as part of a team, analysing complex information and communicating effectively Demonstrate the ability to establish and maintain empathic, supportive relationships with people in significant distress, their families and carers This is a training role within adult community mental health services. The post-holder will be enrolled in a graduate certificate or postgraduate certificate training delivering the national curriculum for Mental Health and Wellbeing Practitioners. Once successfully enrolled, the post-holder will attend all elements of the programme of study and progress successfully through assessments of competence and attainment. Until enrolment, and in the remaining part of the week during training, they will work as a trainee in the service, under close supervision and regular course assessment of practice competence. They will work within the limits of their experience and assessed competence for their stage of training: To work alongside service users, families, carers and multi-disciplinary team members to co-ordinate care, supporting collaborative decision-making about care and treatment. To deliver a set of wellbeing-focused psychologically-informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems. To work within the limits of level of training and experience as a developing practitioner, taking on clinical practice appropriate to assessed levels of competence and stage of training. The post-holder will work as a trainee under close case management supervision from a suitably qualified professional to provide high quality collaborative care planning as a member of the multi-disciplinary team, with a level of autonomy matched to level of demonstrated competence and stage of training. The post-holder will work under close clinical skills supervision from an HCPC registered clinical / counselling psychologist, BABCP accredited cognitive behavioural therapist or Senior MHWP to provide specified psychologically-informed wellbeing-focused interventions, with a level of autonomy matched to level of demonstrated competence and stage of training.

Key responsibilities of the role include to: Provide specialist psychological assessments of service users of the locality service, when care is managed under the CPA (community and in patients). This is to be based upon the appropriate use, interpretation and integration of complex psychological data from a variety of quantitative and qualitative sources including psychological and neuropsychological tests, self-report measures, rating scales, direct and indirect structured observations and assessments with service users and others involved in their care. This can include the interpretation and explanation of the results of cognitive and neuropsychological tests in the context of the service users circumstances and current difficulties and the communication of these findings to those involved in the network of care and in care planning. Be responsible for the formulation and implementation of a broad range of specialist psychological treatment and/or management of service users mental health problems, based upon an appropriate conceptual framework of those problems derived from a broad theoretical knowledge of psychology, and employing methods based upon evidence of proven efficacy, across the full range of care settings. Be responsible for implementing a range of specialist psychological interventions for individual service users, carers and groups, employed singly and in combination, adjusting and refining psychological formulations as practice and experience demand, and drawing upon different explanatory models to maintain a number of provisional hypotheses. Evaluate and make decisions about treatment options taking into account both theoretical and therapeutic models and highly complex factors concerning historical, development and cultural processes and systems which have shaped the individual, family or group. Be responsible for providing and receiving highly complex, sensitive, distressing and emotional information in relation to mental and physical health issues, where there are often difficulties in terms of acceptance or understanding. Communicate across language and cultural barriers, including working for sustained periods of time by communicating with service users, carers or groups through professional interpreters or advocates Spend sustained amounts of time with individuals who may be aggressive and hostile to themselves or others, who may have poor communication and/or self-care skills and special physical and/or mental health needs. To further embed and develop the current competency testing for fetal monitoring to assess and improve the knowledge and skills of all staff providing intrapartum and antenatal care across all maternity settings. the post holder will provide leadership, guidance and expertise within the wider multi-professional maternity team for fetal surveillance and intrapartum risk assessment. This will include putting in place strategies and frameworks that ensure the Maternity service meets the Trust and national standards for fetal monitoring, Feto-maternal Surveillance (MDU) and acute care (Triage). To produce and cascade out and embed into practice an intelligent intermittentauscultation training package and lead on the use of this in training midwives whoprovide intrapartum care on the Delivery Suites, the Midwifery Led Unit and in thewomans home. Ensure that all midwives and obstetricians providing antenatal and intrapartum careundertake the competency test in assessing fetal wellbeing. In conjunction with the midwifery education team provide enhanced teaching andadditional support to staff who need to repeat the competency test to in order toachieve and demonstrate the expected levels of knowledge. Lead weekly CTG workshops, maintaining an electronic database to evidence attendance registers and ensure compliance to the mandatory requirement for midwives and medical staff. Disseminate learning which emerges from workshops. Organise CTG master classes on the monthly mandatory training week ensuring theattendance of all eligible staff. Develop, in conjunction with the Maternity Matrons, Obstetric Lead for Delivery Suite and the Obstetric Lead for Risk, an audit form (on a daily basis) for case reviewsall emergency deliveries i.e. instrumental and caesarean sections in the previoustwenty-four hours to review the interpretation of CTGs and subsequentmanagement of care. Ensure staff are assessing fetal well-being in the context of the woman's health,pregnancy, gestation and stage of labour by observing how handover is communicated at change of shifts, midwife to doctor communication when referrals are made and listening to assessments made over the telephone (SBAR). Scope and purpose of the role To deliver a high standard of patient care as a salaried GP in nursing Homes, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base To manage a clinical caseload, dealing with patients needs in a nursing Home setting Provide advanced GP care and case management to patients with multiple complex long-term conditions Undertake medical and social care assessments to initiate interventions to improve quality of life in the nursing Home setting Reduce the fragmentation of care To provide informal training and support to the nursing home team (where applicable) and ensure the highest standards of care are provided for the patients Job Summary The post holder is an experienced GP, who acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required. Key Responsibilities Clinical role The post-holder will Make professionally autonomous decisions for which he/she is accountable Provide face to face visits via a clinical ward round during the working week. Provide a first point of contact for Nursing Homes for advice regarding medical/nursing conditions of patients, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. This includes a comprehensive review (ensuring coding of all significant health conditions and allergies) of newly admitted patients to the Nursing Home and patients readmitted to the Nursing Home following discharge from hospital. This process would include a post-discharge reconciliation of the medication. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports To prescribe safe, effective and appropriate medication as defined by current legislative framework. This would include the prescription of monthly repeat medication, acute medication and the additional/cessation of medication as indicated (e.g. following discharge from hospital or from the advice of other healthcare professions including but not limited to specialist consultants). Additionally, this includes liaison with both community pharmacists (for example to ensure the continuity of supply of medication) and other pharmacists (including the medicines management pharmacists from GSTT). Provide polypharmacy/drug reviews of medications. To prescribe medication safely and in line with local CCG/national guidance including evidence based and cost-effective prescribing. Provide safe, evidence-based, cost-effective, individualised patient care. This significantly includes documentation about resuscitation and advanced care planning (onto recognised registers such as Co-ordinate My Care) of all patients newly admitted to the Nursing Home as well as a retrospective review of already admitted patients where such resuscitation and advanced care planning discussions have not yet been performed. This is likely to be performed with the patient and/or their representatives (dependent on circumstances including where which involving the patient may not be appropriate) and also be done in Best Interest meetings with other relevant healthcare professionals where a patient lacks capacity about advanced care decision making (and where an advanced directive does not exist). Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Refer patients directly to other services/agencies as appropriate including social services and allied agencies/services.
