The Ambulatory Surgery Unit Registered Nurse (ASU RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The incumbent will administer medications and procedures per established policies and guidelines, and influence care outcomes by collaborating with members of the interdisciplinary team.

The ASU RNcoordinates care prior to and on the day of procedure/surgery. The incumbent provides direct patient care to patients in a pre-operative clinic preparing patients for upcoming care, and on the day of care - to patients undergoing invasive and surgical procedures during the pre-operative/pre-procedural phase and during the post-operative/post-procedural phase of recovery.

This position is located in the Mental Health Care Line, VA Hudson Valley Health Care System. The incumbent is the Program Manager - Substance Abuse (SA) Dom, Montrose Campus. In the role of Program Manager - SA Dom the incumbent will provide high quality Psychological clinical treatment, assessment, education, research services and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the client.

Office of Integrity and Compliance Program Management: The Health System Specialist functioning as Compliance Specialist/Analyst is responsible for completing programmatic auditing of facility-wide revenue process and community care oversight. Participates in systematic reviews of services. Assists in developing, implementing, or coordinating the implementation of corrective actions resulting from such reviews. The incumbent serves as a Supervisory Health Systems Specialist (Credentialing and Privileging Manager). They oversee all functions related to healthcare providers' professional credentials supporting all divisions of the VA Medical Center (VAMC). The Incumbent is responsible for working collaboratively with local and VISN leadership to identify and/or implement best practices and advising VAMC leadership of all program requirements pertaining to the C&P program. They ensure quality assurance and provide oversight for the VAMC C&P program and ensures that the Medical Staff Bylaws/Rules and Regulations are reviewed and updated annually (or as needed) to reflect regulations or accreditation standards.

• Providing direct technical and administrative supervision to employees.
• Responsible for ensuring that employees are efficiently and effectively accomplishing quality work that is compliant with regulatory requirements and procedures, TJC standards, laws, and VHA C&P policies and requirements, as well as assuring implementation and accomplishment of the goals and objectives of the C&P Program.
• Identifying and presenting to leadership the resources necessary for accomplishing the mission and function of the facility's C&P program.
• Providing guidance to local leadership and C&P employees on all matters pertaining to the facility's C&P program.
• Attending committee meetings and presents providers' credentials, clinical practice histories and red flags.
• Serving as the primary consultant for the facility's C&P program; analyzes program audit results; interprets facility quarterly report card data; researches provider credentialing requirements; and identifies potential privileging issues.
• Coordinates annual committee review of clinical privileges and scope of practice forms, provides SME guidance to credentialing, privileging, adverse privileging actions, NPDB reporting, and SLB reporting.
• Serving as the Point of Contact (POC) and SME (Subject Matter Expert) for credentialing updates,
• Human Resources, providers and hiring services.
• Monitoring and auditing the VAMC C&P program utilizing VetPro reports, mandatory auditing and reporting tools.
• Ensuring national level reporting requirements are accurately completed including, but not limited to, quarterly security reports, quarterly accuracy certification of the Our Provider's website, and accuracy of the VA/DoD shared provider registry.
• Monitoring C&P program resources to ensure programs are appropriately staffed.
• Serving as a liaison and partner with Human Resources Service programs for common program goals and issues including, but not limited to, provider onboarding, provider exit reviews and assessments, privileging actions, and guidance related to clinical performance concerns.
• Responsible for ensuring quality assurance in the facility's C&P program; applying organizational and system-wide strategies for continuous quality improvement as they relate to C&P processes and procedures.
• Serves as the administrator for the National Practitioner Data Bank, Federation of Medical Boards, and Our Providers Database.

The individual works in the mental health clinic during regular business hours but may be required to work an irregular tour of duty, weekends, depending on the needs of the service and participate in social work coverage to the medical center off tour.

Duties will include, but not limited to:

• The incumbent is responsible for completing the following duties/responsibilities: Completes structured diagnostic and clinical assessments for participants. This requires knowledge of medical and psychiatric terminology, diagnostic categories, and chart review.
• Reviews assessments completed by others, and obtains missing information from providers, patients, and medical records.
• The incumbent must have a thorough working knowledge Mental Health Suite treatment planning software.
• The incumbent must have a thorough working knowledge of VISTA/CPRS. Must keep abreast of the various changes made within these programs and is responsible for maintaining the integrity of information in these clinical records.
• Provides clinical and case-management services through face to face meetings, and via telephone and video technology.
• Conducts telephone calls and writes letters for the purpose of outreach for new patients, patients who cancel appts, patients who are 'no-shows' and other others.
• Provides psycho-education, support, counseling and/or psychotherapeutic intervention for emotional, psychiatric and psychosocial problems. This includes the development of an appropriate treatment plan in collaboration with other providers.
• Provides social work consultation to Primary Care providers, other Mental Health clinicians and team members in the development of individual treatment plans. This includes coordinating treatment and communicating with other services to facilitate continuity of care for Veterans.
• The incumbent communicates evaluations & recommendations to treatment team members.
• The incumbent develops community resources & coordinates these resources in the interest of the Veteran & his/her family.
• The incumbent specializes in staff development & patient education in the area of mental health treatment.
• The incumbent might participate in on-going research and program evaluations. The incumbent may, as appropriate, supervises the practice of social work students, or less experienced social worker.
• The incumbent will participate in committee assignments, as deemed necessary by the service manager.
• The incumbent will be responsible for maintaining accurate record of relevant trainings and participate in continuing education in accordance with VA policies and state licensure. The Social Worker has the ability to evaluate his/her own practice through professional peer review, case conferences and other organizational means. The Social Worker will complete all mandatory VA trainings in a timely manner.
• The incumbent will fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with NASW Code of Ethics, VHA Code of Ethics, and local Ethics policies, which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information, and protecting Veterans' personal identifying information.
• The incumbent documents in the patient's medical record in accordance with Social Work and medical center policies, and the requirements of The Joint Commission. This may include a Social Work screening and/or development of a biopsychosocial assessment and treatment plan within the time frame required by Social Work service standards. Progress notes are timely and reflect progress toward treatment goals. Outcomes are evaluated and documented. The incumbent will ensure appropriate hand off when transferring of patient care.
• Incumbent will demonstrate knowledge and utilization of Evidence Based Treatment, Measurement Based Treatment and contemporary treatment concepts/modalities widely recognized as valid in Social Work, Psychology and Psychiatry professions.
• Accepts other assignments as delegated by the Outpatient Psychiatry Department Program Manager and the Outpatient Social Work Supervisor.

• Preparing the clinical environment, which may include preparing medical and telecommunications equipment to be used for clinical examination and treatment.
• Serving as a telepresenter within scope of practice during clinical video telehealth (CVT) and VA Video Connect (VVC) encounters.
• Serving as an imager and clinical data manager for store and forward telehealth (SFT) clinical encounters.
• Taking measures to ensure patient care is delivered safely by following appropriate procedures for inventory management, cleaning of clinical equipment and the care environment while following all patient safety guidelines, protocols, and procedures.
• Ensuring that patient privacy is protected by complying with all information security and patient privacy regulations.
• Receiving and scheduling requests for simple and complex intra-facility, and inter- facility telehealth appointments using an electronic resource-based scheduling system.
• Monitoring and maintaining schedules of rooms, patients, technologies, clinical providers and presenters as needed to ensure efficient telehealth operations.
• Configuring, installing, troubleshooting, cleaning, and performing minor routine maintenance of assigned telehealth technology.
• Ensuring assigned telehealth technologies are managed appropriately to include inventory, secure, ready for use, and accounted for by preparing and tracking maintenance work orders, information systems, inventory records, and communicating with the National Telehealth Technology Help Desk to facilitate troubleshooting, repair, and coordination of replacement equipment as necessary.
• Maintaining regular contact with other staff members and the Facility Telehealth Coordinator to work out process issues, equipment needs, problems, data collection and any other logistical issues.
• Completing other duties as assigned.
