The Close to Me Service seeks to reduce travel time for Veterans, improve VA care continuity, and provide Veteran-centric care. Studies show that transportation and travel can be the biggest financial barrier to Veterans accessing care. Furthermore, conflicts with work, other commitments, caregiver time constraints, or cost of transportation can be reasons that prevent Veterans from having the ability to travel. These barriers may hinder Veterans' abilities to receive consistent treatments and adhere to their care regimens. Close to Me (CTM) employs an innovative care model to address Veteran need for more accessible cancer care services.

Duties of the CTM Registered Nurse (RN) include, but are not limited to:

• Travels to and from Community Based Outpatient Clinics (CBOCs)affiliated with coordinating site VA Ann Arbor Healthcare System (VAAAHS) located in Howell, Michigan and Toledo, Ohio
• Coordinates patient care with local CBOC staffing outside of VAAAHS such as calling or messaging local CBOC clinic RN, Medical Support Assistants (MSAs), licensed practical nurses (LPNs), patients, providers
• Verifies labs for patients outside of VAAAHS (within VA system) and coordinating with Nurse Practitioner (NP) or Medical Doctor (MD) as needed - making sure labs within parameters

• Is an integral member of the service leadership team, working closely with the Associate Chief of Staff (ACOS) for Mental Health, Deputy ACOS for Mental Health, and Associate Chief Nursing Service (ACNS) as well as Mental Health Service supervisors to ensure successful administrative operations.
• Serves as the business manager of the Mental Health Service, working closely with administrative chiefs throughout the medical center and the COS Health System Specialist to ensure effective, efficient processes that meet regulatory requirements.

• Functions as a facility domain manager for the TMS educational package. This system houses each employee's official education and training record, as well as serving as the mechanism for providing computer-based training for employees. As a resident TMS authority, the Program Specialist will assist to identify issues and problems associated with these programs and systems, and using initiative, will implement alternative solutions based on expert knowledge. He/She is responsible to ensure proper implementation of updates and changes in the package. Fields calls from all levels of TMS users requiring assistance and provides solution to problem issues.
• Responsible for ensuring mandatory training programs are available for staff at all levels of the organization. Duties include maintaining and updating comprehensive information on mandatory training considered for all employees or targeted to specific employees by nature of their profession or job duties. Creates computer-based training modules in the system by working with subject-matter experts. • Identifies the psychosocial complications that can be caused due to a sudden onset of an acute or chronic illness. At risk factors are addressed and a preliminary disposition plan is documented.
• Perform insightful assessment of serious and complicated cases involving psychiatric illness, catastrophic medical conditions, dementia, and other high-risk diagnoses.
• Assesses the patient and family's understanding of their diagnosis and proposed treatments in the clinic settings.
• Screens and assesses the Veteran's psychosocial needs and barriers to facilitate his or her access to appropriate VA and community resources. Through collaborative relationships with the Veteran, his or her family/caregiver, significant other, legal guardian, and other VA programs and VA staff, the social worker coordinates the gathering of information vital to the process of setting appropriate treatment goals and time frames for achieving those goals. The goal of the assessment is to highlight the Veteran's strengths, limitations, coping/management skills specific to adjustment to the regimen and internal/external supports and service needs.
• Effectively use social work knowledge base, professional skill, objectivity, and insight into the formulation of the assessment. The initial assessment will be completed as specified by the policy for the assigned work area, updated and on-going progress notes will document continuation of plans, meetings, medical or social concerns, and/or changes related to patient care.

• Independently develops psychosocial treatment plans in accordance with resulting interdisciplinary team meetings including goals for psychosocial clinical treatment based on the initial assessment of the Veteran.
• Uses advanced practice skills to advise and recommend appropriate actions related to the comprehensive treatment plan.
• Determines realistic goals with the Veteran, including such factors as the appropriate type of intervention and/or level of care, the setting conducive to providing such intervention or care, the need for caregiver support to allow for the least restrictive level of care, transportation options/barriers, and the steps to obtaining resources (VA or community) that make the achievement of goals possible.

• To facilitate the continuity of a Veteran's recovery, the social worker will arrange referrals to needed VA and/or community resources (e.g., home health care, assisted living facilities, extended care facilities/nursing homes, services available through area agencies on aging, community support groups, hospice care, community-based extended care).
• Will collaborate with other service providers in re-assessing the Veteran's needs during the eligibility determination process and coordinating access to information needed for the referral.
• Provides education to community care partners regarding general VA benefits and services during care coordination for Veterans.
• Serves as a liaison between Veterans and/or their families and VA and community resources to ensure thorough delivery of services and follows Veteran/Caregiver through continuum of health care.

• As a subject matter expert on the services and resources available to Veterans through both VA and the community, the social worker will monitor patterns of resource availability, identifying unmet needs and the resultant impact on the Veteran's psychosocial functioning.
• Will communicate with appropriate stakeholders (e.g., other VA programs, community partners) to encourage the development of new and innovative resources.

• Advocate for Veterans to ensure that they have access to all needed care and resources within VA and in the community.
• When appropriate and feasible, the social worker will educate and encourage the Veteran and his or her family/caregiver to advocate on his/her own behalf, fostering independence and empowerment.

• Will assist in mitigating any medical or psychiatric crises arising in presenting Veterans, within the scope of social work practice, and participate in developing crisis management and safety plans to minimize the event of future crises.

• Nurse Manager (NM) is the designated first-line supervisory authority and is responsible and accountable for the 24/7 operation of their assigned units and provides coverage for other NMs as needed. The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They function as a role model, educator, practitioner, and supervisor providing leadership to the team. They enhance care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence-based cutting-edge programs/processes.
• The NM seeks knowledge from professional organizations to enhance leadership and career growth. They adhere to ethical principles in decision-making, fostering accountability at personal, organizational, and staff levels. They equip staff with tools for culturally competent care and promotes a culture of accountability for high-quality, cost-effective outcomes. They cultivate alliances across the care continuum, building trust through active listening, engagement, and emotional intelligence.
• The NM participates in the development of care delivery models and environmental design, ensuring evidence-based, accessible, affordable, and equitable patient care. Ensures compliance with accreditation and regulatory standards. Aligns nursing/clinical objectives with organizational strategic outcomes, participating in strategy planning and execution. They promote an inclusive workplace valuing diversity to achieve the organizations vision and mission.
• The NM maintains knowledge and ensures compliance with all regulatory and accreditation standards, Directives, policies, and standard operating procedures (SOP).

The position leads a team comprised of Peer Specialists and may lead Without Compensation (WOC) peer support employees, within a common supervisory structure, to provide services, integrate processes, and meet goals of peer support service programming at the facility. As the leader of a team of peer support staff, the position facilitates, coordinates, coaches, and problem solve with the members of the peer support staff team. The position also acts as the recognized liaison to and from the peer support staff team and the supervisor. The work of the position enables the peer support provider team as a whole to deliver improved quality, quantity, and timely services to Veterans.
Duties include, but not limited to the following:

• Ensures that the organization's strategic plan, mission, vision, and values are communicated to the peer support staff they lead and integrated into the team's strategies, goals, objectives, work plans, products, and services.
• Articulates and communicates to the peer support staff they lead the assignments, projects, problems to be solved, actionable events, milestones, and/or program issues under review as well as the deadlines and time frames for completion. The position communicates the supervisor's instructions to the team.
• Coaches the peer support staff they lead in the selection and application of appropriate problem-solving methods and techniques, provides advice on work methods, practices and procedures, and assists the team and/or individual members in identifying the parameters of viable solutions.
• Maintains awareness of the status and progress of peer support staff's work and makes day-to-day adjustments to ensure their work aligns with program goals and processes.
• Leads the team in identifying, distributing, and balancing workload and tasks in accordance with established workflow, skill level and/or occupational specialization; makes adjustments to accomplish the workload in accordance with established priorities to ensure timely accomplishment of assigned team tasks; and ensures that each team member has an integral role in developing the final team products and services.
• Identifies training needs and assists in the development of training for peer specialists and Veterans seeking to become peer specialists.
• Serves as a coach, facilitator and/or negotiator in coordinating team initiatives and in consensus building activities among team members.
• Maintains working knowledge of treatments and services available for the purpose of appropriate referrals.
• Represents the team in dealings with the supervisor for the purpose of obtaining resources (e.g., computer hardware and software, use of overtime or compensatory time), and securing needed information or decisions from the supervisor on major work problems and issues that arise.
• Participates in the service, training, consultation, and other professional activities of VA health care services and functions as a fully vested member of the multidisciplinary team.
• Uses personal recovery experiences to model coping techniques and recovery tools to coach Veterans on relevant skills needed for self-management of their health condition(s) and pursuit of their personal wellness.
• Observes behaviors that might indicate difficulty adapting or responding to treatment (e.g., missed assessment appointments, failure to attend or maintain abstinence, risk to self or others, disruptive behavior), completes appropriate documentation, and reports concerns to the treatment team in a timely manner.
• Uses ongoing individual and group meetings to teach Veterans how to identify and combat negative self-talk and overcome fears by providing a forum that allows Veteran to share their experiences.
• As necessary, handles crisis interventions for Veterans and addresses other emergent situations without the benefit of specific instructions.
• Informs Veterans about VA and community supports and resources, including how to effectively utilize them in the recovery and wellness process.
• Maintains a working knowledge of current trends and developments in holistic approaches in wellness and recovery by reading books, journals, and other relevant materials.
• Performs other related duties as assigned.
