Team Leader. This Food Service Worker position is located within the Nutrition & Food Service at the West Palm Beach VA Medical Center.

Major duties and responsibilities include but are not limited to:

• Sets up and breaks down assigned station with the correct supplies and food items;
• Portions food items into standard serving sizes using the proper utensils and specified dishware

The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing specific care to primary care patients. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The WPB VA Medical Center has a designated role in the Geriatrics and Extended Care Service that serves as the Attending Physician and Medical Director of the Community Living Center (CLC). The incumbent provides clinical and administrative oversight of the CLC. They will also be a teaching faculty member in our Geriatrics Fellowship program. They will have the following duties and responsibilities:

• Serves as a resource to senior management and other clinical and administrative leaders on issues impacting the delivery of care for Veteran's who use the Geriatrics Department services.

The Pharmacy Controlled Substance Technician prepares and fills inpatient and outpatient orders and requests for controlled substances. The technician is responsible for transferring controlled substances from the inpatient pharmacy vault to outpatient pharmacy working stock, and to all inpatient and outpatient clinic CS cabinets or Pyxis machines, on a regular basis so as to assure that an adequate working stock is maintained in each area. This position requires thorough knowledge and understanding of federal, VA, and facility regulations regarding controlled substances and pharmacy processes in general and involves performing complex procedures over a range of functions.

• Follows procedures regarding Community Care referrals.
• Serves as the initial point of contact for units, clinics, patients, call centers, Care in the Community regarding various aspects of patient appointments and referrals.
• Stocks basic office supplies for the clinic.
• Enters Veterans seeking emergent/urgent care, scheduling appointments accurately.
• Pre-Registers Veterans during check-in and utilizing call lists and telephones Veterans before their scheduled visits. Incumbent updates all patient demographics.
• Communicates and interacts with patients in a manner which is appropriate to cognitive, emotional, and chronological maturation needs of the adult and/or geriatric patient.
• Assures that all appropriate VA forms are used correctly and regulations followed.

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Privileges include admission, work up, diagnosis, and provision of non-surgical treatment including consultation for patients admitted or in need of care to treat general medical problems, and provide consultative services to patients in the Long-Term Care setting.

The Hematologist/Oncologist is able to evaluate, diagnose, and provide treatment in an outpatient setting or in the form of inpatient consultative services to patients of all ages presenting with illnesses and disorders of the blood and blood-forming tissues and/or malignant tumors. This includes performing and interpreting peripheral blood smears, bone marrow aspirations and biopsy, administration of chemotherapeutic agents and biological response to modifiers through all therapeutic routes, including intrathecal chemotherapy, the management and care of indwelling venous access catheters, plasmapheresis, and therapeutic phlebotomy.

Administrative duties include:

• Active involvement in teaching residents and students, periodically directing Noon Conferences for their didactic training;
• Participation in the educational programs for Acute Care personnel as well as the medical staff;
• Participation in monitoring the quality, safety, and appropriateness of patient care.

The Orthotist, Prosthetist or Orthotist/Prosthetist, provide appropriate orthotic and/or prosthetic devices and treatment.

• An examination of the patient and assessment of their individual needs and desires.
• Compliance with the treatment plan.
• Design, component selection, measurement/casting of the patient, fabrication and fitting of the device to the patient.
• Proper documentation of the patient record.
• Continuing care and periodic examination of the patient and the orthotic/prosthetic device to assess fit, function and changing needs of the patient.
• Communicates with patient and/or caregiver about the recommended treatment plan, to involve them in orthotic or prosthetic care; include disclosure of potential risks/benefits.
• Verify patient care by documenting history, ongoing care and follow-up, using established record-keeping techniques.
• Refer patient, if appropriate, to higher level Orthotist and/or Prosthetist for intervention beyond orthotic/prosthetic scope of practice or privileging.
• Consults with higher-level Orthotist and/or Prosthetist to clarify or with physician/referral source to modify, if necessary, the original prescription and/or treatment plan.
• Identify material, design and components to support anticipated outcome.
• Inform patient, family and/or caregiver of the orthotic/prosthetic procedure, possible risks and time involved in the procedure.
• Select appropriate material/techniques to implement treatment plan.
• Provide patient with preparatory care for orthotic/prosthetic treatment.
• Prepare patient for procedure required to initiate treatment plan.
• Consult technical component/material resources as required.

• Educate/counsel patient and/or caregiver about the use and maintenance of the orthosis/prosthesis.

The incumbent is responsible for direct coordination of the credentialing and privileging process of the members of the Medical Service (Physicians, RNP's & PA(C)'s) as well as indirect overseeing of the credentialing and privileging process/scope of practice of the Emergency Physicians, Primary Care Physicians and Primary Care RNP's. The incumbent is involved with Resident credentialing and privileging as well which includes coordination and management of the daily operations of the Medicine Residency House staff Programs, which are fully integrated with the Residency programs of the Brigham and Women's Hospital, the Beth Israel Deaconess Medical Center and Boston University Medical Center.

Major Duties:

• The direct duties relate to new appointments, re-appointments, licensure renewals and clearance, Federation of State Medical Boards clearance, and credential verification. S/he maintains a database for all time sensitive certifications and licenses. S/he requests, receives, and reviews quality assurance ongoing and focused peer review process documentation from all of the physicians/RNP's/PA(C)'s mentioned above from the Medical Service as well as assuring the receipt of the documents from other services. This includes not only annual quality assurance/peer review but coordination of the more frequent focused peer reviews for those who are new appointees or those who require performance practice plans. S/he drafts memos for signature by the Chief and/or Associate Chief of Medicine. The incumbent is responsible for following up on continuing medical education credits and works with each new/re-appointed staff member to make sure that the appointment process is understood and completed in a timely and thorough manner.
• In addition to coordinating all administrative aspects of this program, the incumbent functions as the technical advisor to Medicine as the liaison with the Medical Staff Office on all credentialing and privileging matters. S/he ensures all recommendations from the Professional Standards Board are acted upon in a timely manner and provides technical guidance to the Medical staff on the intricacies of the Credentialing and Privileging process.
• The incumbent functions as the Medical liaison with the Nursing and ensures all requirements are met for the initial appointment of Medicine's Advance Practice Nurses and other technical positions. S/he may provide direction to nursing staff on qualifications for promotions and special advancements. S/he prepares and submits documentation for these promotions and advancement as called for.
• The incumbent works closely with the Human Resources Service on new appointments and changes in appointment status. This is especially true with the coordination of newly appointed WOC (without compensation) providers and Fee Basis providers. In this role, the incumbent must have a comprehensive and working knowledge of personnel procedures and practices.
• S/he completes any additional requests and work as directed by supervisor.
