The Oncology/Hematology Registered Nurse (RN) is a clinical expert who uses advanced specialized knowledge and judgement for delivery of high quality and evidence-based care to oncology patients and their families. Uses a team approach to identify, analyze, and resolve patient care problems with measurable outcomes. Completes physiological assessments and completes documentation in the electronic health record (EHR) in accordance to established policies and procedures. Triages patient problems and assists in the evaluation of symptoms and initiate interventions in collaboration with the interdisciplinary team. Monitors, tracks/tends, and reports ongoing care needs and outcomes to the interdisciplinary team. They direct the provision of nursing education, orientation, competencies, and providing quality improvement and outcome utilization. They provide a safe working environment while administering, handling, and disposing of antineoplastic drugs and medical waste. They oversee the administration and management of various medical procedures and treatments in the oncology setting, including vascular access devices and infusion treatments. Conducts in-service education based on insights gained from oncology professional development activities and acquiring specialized skills in oncology-specific areas, including chemotherapy administration, oncology electronic ordering systems, and calculating drug dosages. The Oncology/Hematology RN manages monitoring requirements for specific drugs, handling immediate infusion reactions, and preventing and managing oncology-related symptoms and adverse events, such as chemo-related symptoms and immune-related adverse events.

Team Leader. MTs at this level have professional oversight responsibilities of a laboratory team. .The team leader facilitates team or unit processes by working in collaboration with team members to ensure that tasks, priorities and goals are coordinated with laboratory management. The incumbent also provides technical advice to staff and other health care members and oversees work assignments and work flow. The team leader is responsible for ensuring that staff follow all policies and procedures and is a subject matter authority when there are questions of a technical or professional nature. The incumbent may be responsible for reporting personnel absences and may be responsible for staff schedules and other assignments delegated by the departmental supervisors.

Duties/Tasks

• Modifies or adapts new methods or techniques to improve or expand services.

This Food Service Worker position is located within the Nutrition & Food Service at the West Palm Beach VA Medical Center.

Major duties and responsibilities include but are not limited to:

• Sets up and breaks down assigned station with the correct supplies and food items;
• Portions food items into standard serving sizes using the proper utensils and specified dishware
• Selects and places correct items on patient trays
• Decides what food items to serve for the most common diets
• Identifies obvious discrepancies between the prescribed diets and the food items designated by the menu
• Sets up dining room tables for service, place food and beverages on tables, and replenish items as necessary;
• Delivers meal trays to the patients' bedsides; reporting the patients' comments and complaints to the supervisor or dietitian;
• Provides assistance to cooks in the food preparation area, such as weigh, measure and assemble ingredients according to standardized recipes;

The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing specific care to primary care patients. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The nurse is responsible for delivering and improving holistic, and primary care through collaborative strategies with members of the patient care team. The (PACT) RN is also is also responsible for improving Veteran driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns within the immediate practice setting.

Preferred Experience: 2 years RN experience in the area of Urgent Care, Primary Care, Nursing Homes and/or Emergency Room.

The WPB VA Medical Center has a designated role in the Geriatrics and Extended Care Service that serves as the Attending Physician and Medical Director of the Community Living Center (CLC). The incumbent provides clinical and administrative oversight of the CLC. They will also be a teaching faculty member in our Geriatrics Fellowship program. They will have the following duties and responsibilities:

• Serves as a resource to senior management and other clinical and administrative leaders on issues impacting the delivery of care for Veteran's who use the Geriatrics Department services.
• Plans, develops, and maintains a comprehensive program to ensure compliance with Veterans Health Administration (VHA) Guidelines and Directives and other relevant standards.
• Directs and coordinates medical care and participates in the resident care management system.
• Assists with identification, evaluation and resolution of medical and clinical concerns and issues that:
  (a) Affect resident care, medical care, or quality of life; or
  (b) Are related to the provision of services by physicians and other licensed health care providers.
• Participates in educational activities related to the delivery of services to Community Living Center residents and promotes and supports appropriate continuing education programs for other staff members.
• Coordinates medical provider assignments and coverage in the department in collaboration with the Chief of Geriatrics and Extended Care.
• Assures that medical providers and other staff members provide resident care services within established clinical privileges, scope of practice, competencies and qualifications.

Duties include but not limited to:

• Assisting medical and nursing providers when performing medical & diagnostic examinations by assisting with duties such as monitoring vital signs, collecting biometrics, preparing and positioning the patient, completing the history and reminders, or by performing a variety of routine diagnostic tests.
• Taking and recording measurements and vital signs.
• Reporting deviations in vital signs and/or weight to the LPN/RN/MD.
• Measuring and recording height.
• Completing Health Promotion activities and asks reason for visit.
• Obtaining and labeling specimens to include clean catch urine specimens or sterile specimens.
• Drawing and labeling, and packaging specimens for the laboratory.
• Transporting specimens to the laboratory and calls the laboratory for reports.
• Reporting/recording results for the provider as appropriate.
• Performing routine diagnostic tests such as venipuncture, bladder scan, ultrasound, glucometer, pregnancy testing, urinary tract infection testing and electrocardiograms, as directed by the physician or professional nurse, for the purpose of diagnostic testing. He/she then calibrates and transmits data via electronic records.
• Assisting the physician in a variety of diagnostic examinations and must be able to receive instructions for difficult exams such as lumbar puncture, biopsies, colposcopies, LEEPs, intrauterine device placements, and pap smears.
• Positioning patient according to specific instructions from surgeon or primary care physician, drapes patient, preps and adjusts specialized equipment with multiple features, and passes instruments.
• Taking tissue cultures or specimen samples from difficult specialized procedures.
• Answering patient's questions during procedures.
• Chaperoning and comforting patients receiving gender specific examinations, including those that received Military Sexual Trauma. He/she possesses knowledge of surgical instruments in this clinical setting such as dilators, tenaculums, kevorkians, loops and balls, forceps and assists doctors with size selections. He/she possesses knowledge of proper handling and pre-cleaning of reusable medical equipment. Procedures are delicate and instrumentation is elaborate requiring settings and measurements that are very fine.
• Making precise adjustments to clarify readings and measurements and reports observations of the clinic patient regarding mobility, nutrition, pain/ comfort level, chief complaint, etc.
• Performing patient care procedures such as collection of specimens, applying dressings, adjusting simple supportive equipment. Performs simple procedures such as collection of blood samples or other fluid samples from ambulatory patients and application of sterile dressings to uncomplicated wounds. Uses proper sterile techniques for collection of samples.
• Verifying correct labeling and patient identification.
• Recording test time and intervals as appropriate. Assists with or performs standard health care procedures. Special procedures for women's health can include the annual breast, pap and cervical screening and interview.
• Teaching of patients and family members the necessity to continue good health maintenance, personal hygiene & self-care behaviors.
• Assisting with general health education and classes as assigned.
• Providing assistance of a technical, specialized, or support nature to health and medical personnel in the diagnosis or treatment of patient illnesses or medical conditions. Is able to teach clients how to read their meters, Blood Pressure's and other self monitored tests.
• Teaching patients and family members the necessity of personal hygiene and continuing proper self care behaviors.
• Promoting shared decision making behaviors.
• Providing general clerical/administrative support coordination work for the Clinic.
• Performing a variety of receptionist, record keeping, and miscellaneous duties in a general medical facility. Receives visitors, answers the telephone, and refers calls to appropriate staff or records messages. Schedules appointments and procedures.
• Orienting providers and new personnel to the clinical area including the proper procedure for requesting medical tests and the requirement for completion of exams.
• Assisting in familiarizing residents and new staff to operations, CPRS, various medical equipment in training to clinic setting.
• Participating in orientation of residents and new staff by explaining the roles of the healthcare team, assisting them with navigation of VA systems, and assigns them patients and rooms.

Major duties include, but are not limited to the following:

• Able to anticipate and implement preventive patient care measures. This PACT is responsible for direct patient care on the PACT teams and facilitating the appropriate plan of care.
• Expected to manage patient care in an astute manner, which promotes positive patient outcomes including the areas of
• triage, telephone triage, and scheduled and unscheduled patients.
• Possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs, providing care as considered necessary.

The LPN must have demonstrated in-depth knowledge and the clinical expertise to perform complex practical nursing care to patients with a variety of physical and/or behavioral problems, is responsible for upholding the philosophy and objectives of the Nursing Service and is accountable for adhering to established policies, procedures, standards of care and standards of practice, and for maintaining competencies consistent with roles and responsibilities.

• Demonstrates proficiency in initiating, performing, and completing assigned duties while providing care to diverse patient populations.

• Be familiar with which items are cleaned locally (in clinic) and which items are sent to sterile processing service (SPS) for reprocessing and/or sterilization. If items are to be transported for reprocessing, he/she must be aware of the initial decontamination steps that must be taken prior to transport of item and the appropriate method of transport.
• Be responsible for following the local Standard Operating Procedure (SOP) drafted in accordance with the manufacturer's written recommendations. SPS and Infection Prevention and Control should be consulted when clarification of written instructions are needed. No one should be reprocessing any reusable medical equipment based on vendor's verbal instructions alone; this is not an adequate practice.
