Monitors vital circuits and incoming emergency type calls such as those to security, the police, the fire department, etc., to assure call completion and proper disposition. Responsible for issuing emergency keys and for the security of the Dial Central Office (DCO) after normal duty hours. Serves as telephone trouble desk operator after normal duty hours, reporting all emergency communication outages to the maintenance section. (7) Drug Therapy Monitoring - Drug therapy monitoring should be conducted by Clinical Pharmacists in all areas of pharmacy practice as an ongoing activity.
(8) Pharmacist Intervention - Relevant findings from drug therapy monitoring activities must be communicated to other health care professionals who are responsible for the patient's care in order to increase the effectiveness and minimize potential risks of drug therapy. In consultation with prescribers, the interpretation of such data will generally stimulate Clinical Pharmacist recommendations for revision of drug therapy plans. The incumbent is expected to provide documentation of the outcomes of interventions to his/her supervisor.
(9) Pharmaceutical Care - The Clinical Pharmacist shall practice in a collaborative manner with other health care providers in the provision of pharmaceutical care to patient populations as specified in the competency assessment checklist for the position. Pharmaceutical care is defined as the responsible provision of drug therapy for the purpose of achieving definite outcomes that improve a patient's quality of life.

* Ambulatory Surgery RN supports the VAIHCS mission in caring for complex veteran populations requiring acute and chronic medical care in the outpatient primary care setting.

* The Ambulatory Surgery RN has responsibility, authority, and accountability for managing the nursing care of Veteran within her unit.

* She/he interfaces with a variety of VA and community providers directly, over the phone, through correspondence, and electronically via the computer, fax modality, and teleconferencing.

* S/he participates in a variety of quality improvement functions as a vital team member of the Illiana Health Care System. Implements procedures as defined by policies and procedures of the Illiana Health Care System.

* Institutes appropriate action in emergency situations when there are critical changes in the patient's condition.

* This registered nurse has responsibility for assistance in Ambulatory Surgery Unit (ASU),Pre-operative Education, Intravenous Therapy Unit, General Surgery Clinic and other areas, as directed.

* Ambulatory Surgery Registered Nurse (RN), provides pre-operative/procedural care, including (but not limited to) the nursing assessment of patients, consent, and H&P validations, the starting of intravenous fluids including venipuncture, assuring all lab work and other pre-operative/procedural testing is completed and communicated to team members, confirms surgical sites are prepared and marked, and the administration of medications as ordered.

* Provides clear and concise preoperative teaching and reviews discharge instructions to patients and families with written and verbal communication.

*The Ambulatory RN monitors post-operative VA-PAS Phase II patient's level of consciousness, vital signs, and pain level after the recovery of anesthesia/sedation.

* Closely observes signs of side effect of post residual anesthesia medications.

* The Ambulatory RN maintains knowledge of current American Society of Peri Anesthesia Nurses (ASPAN) standards, VHA Directive, and service level standard operating procedures.

*The Ambulatory RN is responsible for ensuring comprehensive documentation of all phases of care and is timely, complete, and accurate in the electronic health record.

Using aseptic technique, compounds both complicated and uncomplicated parenteral admixtures in the form of large volume, small volume, infusion syringe or another modality as required.

Prepares sterile ophthalmic preparations and other complex sterile pharmaceuticals such as total parenteral nutrition and cardioplegia solutions.

Correctly interprets orders, selects, and measures appropriate ingredients, mixes ingredients in an aseptic and effective manner, inspects the final product for accuracy, suitability, and elegance, labels the product, maintains records of the manufacturing process, and supplies the product for delivery.

Understands specific handling considerations for storage, distribution, and disposal of medications.

Operates and maintains complicated pharmacy equipment including but not limited to Omnicell TM /Pyxis TM, laminar flow hoods, total parental nutrition (TPN) compounder, repeater pumps, etc.)

Ensures that the IV cleanrooms are cleaned daily, and appropriate logs are completed for daily, weekly, and monthly cleaning.

Stocking and maintaining the Pharmacy Service preparation area(s), reviewing anticipated inventory needs and notifying the procurement technician in a timely manner of products required.

Participates in glove fingertip and media fill testing, maintain competences passing didactic practical skill assessment initially and annually thereafter.

Assists in airborne particulate and microbial testing and surface testing to ensure cleaning schedules are met.

Responsibility for keeping abreast of changes in the Pharmacy Service (i.e., interdepartmental memoranda, relevant station memoranda, and policies and procedures.

Assists in training and orientation of new staff to ensure consistency in the training process.

Other Duties:

Performs daily refrigerator and or freezer temperature checks and/or monthly inspections for outdates according to local policy and standard operating procedures.

Assists in other patient care areas as deemed necessary to maintain the consistency of overall pharmacy services.
