MAIN DUTIES AND RESPONSIBILITIES 1. CLINICAL 1.1. Under supervision and with support delivers low intensity treatments to patients with mild to moderate presentations of anxiety and depression 1.2. Proactive in identifying learning opportunities as a trainee PWP within service, utilises available support and resources. 1.3. Assess and support people with a common mental health problems in the self-management of their recovery. 1.4. Undertakes patient centered assessments which identify areas where the person wishes to see change and or recovery and makes an accurate assessment of risk to self and others. 1.5. Make decisions on suitability of new referrals, adhering to the departments referral protocols, and refer clients on to the relevant service or back to the referral agent as necessary or step up the persons treatment to high intensity in conjunction with clinical supervision. 1.6. Provide a range of information and support for evidence based high volume low intensity psychological treatments. This may include guided self help, computerised CBT, information about pharmacological treatments. This work may be face to face, telephone or via video call. 1.7. Educate and involve family members and others in treatment as necessary and appropriate. 1.8. Adhere to an agreed activity contract relating to the number of patient contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient. 1.9. Attend multi-disciplinary meetings relating to referrals or patients in treatment, where appropriate. 1.10. Complete all requirements relating to data collection within the service. 1.11. Keep coherent records of all clinical activity and patient contact in line with service protocols 1.12. Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach. 1.13. Assess and integrate issues surrounding work and employment into the overall therapy process 1.14. Develop risk assessment skills to support patients access the appropriate and safe level of support. 2. PROFESSIONAL 2.1. Ensure the maintenance of standards of practice according to the employer and any regulating and keep up to date on new recommendations/guidelines set by the department of health (e.g NHS plan, National Service Framework, National Institute for Clinical Excellence). 2.2. Ensure that patient confidentiality is protected at all times. 2.3. Be aware of and keep up to date with advances in the modalities of treatment for common mental health problems. 2.4. Ensure clear objectives are identified, discussed and reviewed with supervisors and/or line managers on a regular basis as part of continuing professional development. 2.5. Attend clinical / managerial supervision on a regular basis as agreed with Manager. 2.6. Participate in individual performance review and respond to agreed objectives. 2.7. Keep up to date all records in relation to Continuing Professional Development (CPD) and ensure personal development plan maintains up to date specialist knowledge of latest theoretical and service delivery models/developments. 2.8. Attend relevant conferences / workshops in line with identified professional objectives. 2.9. To attend 100% of the PWP training programme provided by Exeter University and to complete the components of the course as set out by the University. 3. GENERAL 3.1. To contribute to the development of best practice within the service. 3.2. To maintain up-to-date knowledge of legislation, national and local policies and procedures in relation to Mental Health and Primary Care Services. 3.3. Clinical - Prioritise the day to day management of own caseload and that of the team, delegating activities as appropriate. Ensure the patient is aware of the role and philosophy of OT and components of the OT process. To be responsible for the comprehensive assessment, treatment and evaluation of patients including those with complex presentation using treatment techniques to promote normal movement, motor relearning, cognitive therapy, compensatory techniques and daily living skills. To work as part of a MDT to manage acutely unwell patients in the community, including taking observations and liaising with relevant medical professionals/team members. To use a selection of both standardised and non standardised assessments to assist and aid diagnosis of cognitive, sensory, perceptual, behavioural, physical, psychosocial and social functioning e.g. to make clinical interpretations resulting from assessments of your own caseload and that of your team. To assess, prescribe and authorise aids and adaptive equipment from the core catalogue and make recommendations for minor and major adaptations to increase / enable or compensate for independent function, teaching compensatory techniques as required. To assess seating and positioning needs and treat as appropriate including assessment for and prescription of standard wheelchairs and accessories when accredited referring on to the wheelchair services as required for assessment of complex and specialist mobility and seating needs. To assess for potential splinting and positioning to prevent, treat and manage deformity and improve function for the upper limb. Plan and review treatment programmes in close liaison with all members of the multi disciplinary team, patients carers and family in accordance with consent given. To record and report regularly on patients progress and treatment to the multi disciplinary team or in accordance with the policies and procedures laid down in individual clinical areas including the original referrer. To plan, organise and lead both formal and informal treatments and health education groups for both patients and carers. Education of patients and carers regarding the impact of cognitive, perceptual and physical deficits on patient safety and independence in the home, leisure and workplace. To impart sensitive information and advice to patients and carers which could have life changing consequences. To provide an opportunity for patients and carers to express their feelings whilst encouraging acceptance and adjustment to their new circumstances. Demonstrating to and informing other community staff re ongoing patient needs in the community, i.e. home care staff, district nurses. To facilitate return of patients currently in residential or nursing care who wish to return home after a period of stay in a care setting. Liaise with and ensure good communication with statutory and voluntary bodies to establish a comprehensive package to ensure continuation of care in the community. Refer to follow up services to ensure treatment programmes are in place to progress functional independence based on identified needs at the end of episode of care. To provide support and supervision to Band 5 OT's, assistants and students as required. To identify occupational therapy clinical areas of strengths and weaknesses within the team and monitor both workload and caseload of Band 5 / non-professional staff and give advice on treatment interventions. To be responsible for the discharge of patients from your caseload. To provide written communication with outside agencies and charitable bodies to support the need for services and equipment on discharge from the service. Actively contribute to service development. Professional/Clinical Under the direction of the Clinical Director of Pharmacy and supported by the Advanced Clinical Pharmacist you will: Deliver optimum clinical pharmacy services to patients within our care Attend MDTs and consultant ward rounds. The post holder will assist registered staff within the Complex Needs team at harbour Drug and alcohol services, working under supervision, in providing support in the community for people experincing mental health and co-existing substance misues issues. The post holder will work alongside the other services and staff working with people that live complex lives in the community, where they will manage a caseload under supervision from a registered member of staff. The aim is to promote recovery from addiction and improve the health and wellbeing outcomes for individuals, linking them in with Community activities and wider supports which include improving access to activities, meaningful employment, and training. The role will require the post holder to work across the complex lives system, developing in consultation with service users flexible and realistic support packages and person-centred plans within agreed guidelines and service models. This role will undertake oral health appointments with patients of the service, including adults/children and those with additional needs or who are medically compromised and to undertake fluoride varnish application under the prescription of a clinician.
