HC-One Scotland is developing a Specialist Dementia Care Community (SDCC) at Darnley Court care home. Darnley Court has provided care for people living with dementia for many years but following considerable investment in environmental layout and design, workforce and L&D in Carmichael community, we have exciting new opportunities available for those passionate about supporting people who are emotionally distressed by their dementia and require a specialist service to ensure their needs can be met appropriately. As an SDCC, Carmichael will become a therapeutic care setting that demonstrates that quality of care and quality of life are of equal significance. It will provide a bespoke living environment embracing space and calming de-escalation rooms, along with enhanced staffing and specialist training for Colleagues in positive approaches to behaviour, psychosocial therapies, de-escalation interventions and safe restrictive practices. The therapeutic ambition will be that people will be understood as having needs to be met, rather than being defined by their condition. The Dementia Nurse Practitioner will support, deliver, and coordinate person-centred dementia care and therapeutic interventions in a purpose-designed residential setting to improve clinical outcomes for those living with complex dementia presentations, working within a team of nursing, social care, psychology and wellbeing practitioners. An active NMC Pin & Minimum of 2 years practice experience in a dementia care community is essential for this role.

The assistant practitioner will work alongside colleagues to prepare and deliver group based interventions in a variety of settings, county wide to groups of people, newly diagnosed with dementia and their families. You will be confidant speaking to members of the public and be able to modify your communication style as required by the audience and situation. You will have an understanding of dementia and be able to provide care and support as required. You will act professionally at all times and demonstrate a positive image of the service. You will maintain accurate records and know your limitations and when to escalate risk and safeguarding concerns to the registered team members. You will support the team to engage with families on the waiting list for the service. You will be involved in all aspects of the service and team working as required. You will be supported by registered staff and full training will be given including monthly clinical supervision. The post holder will support various aspects of this busy service, with a commitment to an out of hours rota. The daytime role will be affiliated to one of our elective services. The postholder will attend clinics, theatre sessions and ward rounds as per allocation in a weekly firm timetable. There are ample opportunities for work based assessments from the supervising consultants. You will also be allocated a Clinical Supervisor. In addition, there is a weekly local General Surgery teaching programme which you would be expected to attend and contribute to. Although this is not a training post it should be possible for an enthusiastic doctor to map their experiences to many of the core curriculum programme competences as all aspects of the syllabus are relevant to this attachment. We encourage our doctors to involve themselves in audit and research. You would be expected to make yourself familiar with the processes of the audit cycle and to assess the role of audit in clinical governance and in improving patient care. Supervision for projects will be available, and support may also be available from hospital audit departments.

This post is fixed term for 12 months covering maternity leave, starting at the beginning of March 2025. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation Assessing the health care needs of patients with undifferentiated and undiagnosed problems Screening patients for disease risk factors and early signs of illness In consultation with patients and in line with current practice disease management protocols, developing care plans for health Providing counselling and health education Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate Recording clear and contemporaneous consultation notes to agreed standards Collecting data for audit purposes Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
