Understands and follows the administrative input required to support the patients standard clinical pathways from the time the patient is referred through to discharge. Coordinates all administrative processes for each patients pathway to ensure it is managed smoothly and within agreed time frames. Ensure all patient demographics and contact details are kept up to date on Cerner When accessing patient records ensure computerised tracking systems are used appropriately. Phone patients to provide instructions, reminders and information where necessary. Attend and participate in meetings relevant to the role as required. Key activities of the post holder will be to: Act as a specialist practitioner and role model and manage the day to day pharmaceutical care of cystic fibrosis patients. Participate regularly in multidisciplinary ward rounds and outpatient review meetings delivering and promoting high quality, cost effective, patient focussed care. Participate in clinical audit and research, risk management and clinical guideline development. Liaise with homecare delivery companies, pharmacy medicines finance team, shared care hospitals and patients and their carers to ensure timely, safe and cost effective clinically appropriate medication supply.
