Key Responsibilities: Trusted Assessor Model Implementation: Lead the introduction and integration of a trusted assessor model to streamline patient flow and improve care efficiency. Collaboration Across Teams: Work closely with the Urgent Community Response Team and ASC teams to support hospital discharge, admission avoidance pathways, Homefirst, and virtual ward pathways. Multi-Disciplinary Coordination: Engage with a large multi-disciplinary team (MDT) comprising nursing, allied health professionals, administration, and social care staff who deliver urgent, short-term support during crises. Change Management: Drive change initiatives and ensure effective implementation of new processes across health and social care organisations. Patient Care Continuity: Ensure service users receive appropriate care following initial short-term intervention, whether they return to independent living or require longer-term support. Educational Support: Develop and implement education and training programs for staff across health and social care settings to ensure understanding and effective utilisation of the trusted assessor model Candidate Profile: Experience: Proven experience in either health or social care settings. Leadership Skills: Strong leadership abilities with a clear focus on managing change effectively. Communication Skills: Excellent verbal and written communication skills. Project Management: Experience in project management or relevant project work is highly desirable. Team Collaboration: Ability to work collaboratively within a multi-disciplinary team environment. Educational Expertise: Experience in developing and delivering educational programs to support staff development and improve service delivery.
