Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing individuals on the caseload of the team, and will use clinical reasoning and critical thinking to effectively meet the needs of CHC funded individuals. The post holder will develop knowledge in relation to continuing healthcare, underpinned by theory and clinical expertise, and will work in partnership with other disciplines and agencies, including GPs, social services, community nursing and therapists, care agencies, care homes and mental health services in order to manage the care of people with highly complex needs, with a particular focus on Learning Disabilities and Autism.

As Lead for CHC Operations in BNSSG ICB you will be responsible for: day-to-day operational performance of the service. leading services to drive transformational change towards excellence, providing reports and audits to give system assurance of quality and governance in service delivery.. supporting the current clinical leadership team in effective recruitment, then training and developing of staff knowledge, skills and experience within the service to achieve low attrition rates and ensure staff feel valued and able to excel. leading on development work and projects across the Funded Care Services within the ICB, and participating in similar across the health and social care network, forging joint working and a collaborative approach, with the patient central to all decision-making. representing the BNSSG ICB Funded Care Team in local, regional and national platforms, such as communities of practice and NHS England forums. developing your own knowledge and skills to the point of being a subject expert in matters pertaining to Funded Care service delivery. These include, but are not restricted to: the National Framework for Continuing Healthcare, the Mental Capacity Act, NHS England and BNSSH ICB Policies, Personal Health Budgets, Safeguarding, Inclusivity and Person Centred Care, Quality Standards and IT innovation in healthcare.

The Health and Wellbeing Coach will work in the Bristol, North Somerset and South Gloucestershirecommunity providing health and lifestyle coaching and care co-ordination within their areas. This will include: Independently running health & wellbeing workshops/exercise classes in a group, in a virtual setting or face to face to NHS service users experiencing mental wellbeing difficulties, who may also experience long term health conditions. Independently conducting health & lifestyle initial appointments and 1-1 sessions via telephone or video call. The Healthy Living Healthy Minds programme aims to help improve service users wellbeing by focusing on improving lifestyle factors, such as physical activity levels and healthier eating. Thus, in-depth coaching sessions are important to address the causes and barriers to improving lifestyle. Contributing to the Healthy Living Healthy Minds programme development and assisting in producing resources. For candidates who do not hold the training for Level 3 GP Referral (also known as Level 3 Exercise Referral), this will be provided and will be expected to be completed within the first 6 months of employment (funded by Vita Health Group). This is a fixed term contract for 6 months. Skills and Experience: Essential: Minimum level 3 accredited qualification in personal training or exercise instruction (such as REPs or CIMSPA) Experience of delivering face-to-face or virtual exercise classes Experience of working individually and within a team Ability to encourage behavioural changes, and design individualised health & wellbeing plans Sound health and wellbeing knowledge including exercise and nutrition Experience of working with people experiencing mental wellbeing difficulties Willingness to work Wednesday evenings Basic knowledge of the Talking Therapies mental health service Good IT skills, including Microsoft Office Adept in time management and prioritisation Friendly and approachable with excellent written and verbal communication skills An awareness of and commitment to supporting and facilitating diversity and inclusion Willingness to travel, which may include overnight stays on occasion, as and when required Desirable: Level 3 GP Referral (Also known as Level 3 Exercise Referral) Certificate / diploma in behavioural change, personal coaching, or motivational interviewing Recognised qualifications in exercise class instruction Why Vita Health Group? The Role: We have several opportunities to recruit enthusiastic and committed Trainee Psychological Wellbeing Practitioners (Trainee PWPs) to join our expanding NHS Talking Therapies service operating across Bristol, North Somerset and South Gloucestershire. In this role, you will provide Low Intensity interventions whilst undertaking the Post-Graduate Certificate delivered by the University of Exeter commencing in March 2025.This training post will equip you to provide Step 2 Guided Self-Help Low Intensity Interventions to clients with a range of common mental health problems, such as anxiety and depression, for which CBT interventions are demonstrated to be clinically effective. You will be joining a motivated workforce and will be supported in the completion of both your academic study and clinical delivery by skilled and experienced supervisors. This role will be offered on a permanentcontract. The role typically comprises 3 days per week undertaking clinical practice with the service and 2 days per week attending university. University days will be hybrid training of a combination of remote and face-to-face teaching in a Bristol based location. The role is likely to include remote working but attendance at one of the BNSSG offices will be required at least twice a week. Skills and Experience: Essential: Undergraduate degree or evidence of previous successful study at Regulated Qualifications Framework Level 5 or Level 6; Evidence of one years working experience in settings with high exposure to people with mental health difficulties. Evidence of interpersonal skills to engage and develop working alliances with colleagues and patient. Evidence of an openness to learning new knowledge and skills. Ability to work under pressure. Excellent administrative and time management skills. Flexible and able to provide one evening shift (12-8pm) as required by service needs.
