The post holder will provide direct personal nursing care, primarily in the patients home and on occasions in other settings such as Community Hospitals, Residential and Nursing homes and Eden Valley Hospice. The post holder will provide practical nursing care, emotional, social and psychological support to individuals with palliative care needs, their families and carers. This role involves regular and extended involvement with the stress of caring for dying patients and their families.

Clinical/Operational To assist the Sonographers and Superintendent in order to facilitate the smooth running of the scanning departments on a daily basis. Organising the daily throughput of the department in conjunction with the sonographers by ensuring that all request cards for each day are correct, noting any special instructions regarding patient preparation. Liaising with wards to arrange the transportation of patients to the department, and likewise liaising with the porters. To assist in patient care, getting patients changed, giving oral preparation, assisting patients in and out of the scan room and on and off the scanner table. To set up the trolleys and assist the radiologists with biopsies, drainages and any other interventional procedures. To answer the telephone and deal promptly with enquiries from patients, doctors and other hospital staff. To ensure the availability of adequate supplies by monitoring stock levels and reordering when necessary. To be able to use the departmental radiology information computer to register and track patients, to check results and appointments. To have a flexible approach and assist generally with any other duties as requested by the Superintendent radiographer and clinical manager. As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. As directed by the Patient Pathway Coordinators ensure a full administration service is provided within the various services of Ophthalmology. This may include providing basic cover for the Patient Pathway Coordinators during periods of sickness, annual leave or workload increases. To collect health records as required from the Medical Records Libraries or elsewhere in the hospital and to assist with the preparation and pulling of clinic notes. To respond to queries relating to health records, either by telephone or in person and to ensure all health records are tracked using EPR thereby maintaining an accurate location of all records.
