The post holder will assist in the delivery of occupational therapy in the SWL PIC and across a range of health and social care locations. To provide leadership for junior staff. The post holder will provide vital support to the multidisciplinary team with integrated seamless services within community settings working in partnership with local authorities, other health care providers, GP practices and the acute Trust. The post holder will work with the multidisciplinary team, they will also liaise with multiple stakeholders in order to ensure that the day to day co-ordination and delivery of service is in a timely and professional manner. The post holder will play a key role in supporting the team with a range of therapy interventions, which enable the patient to receive the optimum care health outcomes. There will be a need to understand and undertake administration and co-ordination of tasks within the MDT as and when required. The post holder will work both independently and with others demonstrating the ability to use initiative, utilise IT systems and ensure effective and timely care in a professional manner. To select and apply Occupational Therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure. To plan and implement individual and/or group interventions, in collaboration with the client, using graded activity to achieve therapeutic goals. To monitor, evaluate and modify treatment for clients with multiple needs in order to measure progress and ensure effectiveness of intervention. To demonstrate and apply a broad level of understanding of the effect of disability and recommend adaptations to the clients environment. To demonstrate the ability to work flexibly to effectively meet the needs of patients. Maximise opportunities for health promotion/education approach within intervention. To recommend and provide assistive equipment ensuring that patients/service users and their carers are instructed in the safe use of such items. Being an effective member of the multi-disciplinary team Supervision, support, education and appraise of students, juniors and assistant staff. Maintain timely and accurate patient records in line with Trust Policy as an Autonomous practitioner. Ensure patients receive care at the right time, right place, by staff with the right skills Support the senior team in the development of any new ways if working that will ultimately improve patient care as and when required. Support the development and delivery of the community transformation Project including the implementation of robust working practices processes, audit and lessons learned. Abide by/act at all times in line with the HCPC rules of professional conduct and core standards of professional practice. Work in collaboration with all members of the MDT and external agencies. This will include attendance at MDT meetings, ward rounds, case conferences and discharge meetings. Undertaking specialist assessment and treatment of patients with a range of conditions including Falls, Frailty, and Neurological conditions. Communicate complex and sensitive information to patients and staff in an understandable form. Ability to manage the communication of unwelcome news in difficult situations. This will be supported by the senior managers in the Trust. Assist the community operations manager and SWL PIC manager to provide leadership and support to staff. Complete risk assessments for patients and staff, suggesting appropriate mitigation. To make changes in the delivery of these services in liaison with the Lead as appropriate which will be referenced to current research and evidence based practice. Assist the lead with transformational changes to improve service provision. To assist in the development of the service in association with other relevant professionals. collect service data and statistics as required. Take responsibility for own continued professional development and continued learning Take responsibility for the delivery of a quality and professional service at all times. To contribute to the education and training of Therapy staff, and other professionals across the Trust, through provision of in-service training related to the Occupational therapy assessment, treatment and management of a range of conditions. To contribute to the development of clinical protocols and guidelines based on best evidence in order to improve the standard of care provided to patients. Ensure adherence to Health & Safety standards. Contribute to audit of the service and improvements in patient care. Utilising knowledge of National and other Guidelines. Adhere to Trust policies and procedures and department standards and the wide Safe care remit of community business unit. Manage own emotional needs within the delivery of the service and access the appropriate support mechanisms within the Trust. The post holder will be expected to kneel, sit on heels, move inert adult limbs, support patients whilst walking, transfer patients, use hoists and push wheelchairs on a daily basis To provide advice to other professionals such as GPs, Consultants, Nursing staff, Social Workers and other Health Care Professionals to assist diagnosis, treatment and discharge management of a wide range of individuals in your caseload. The post will be based in Gateshead, primarily in the SWL PIC. The post holder will be expected to travel to patients living within the borough of Gateshead and locally, if the patient is registered with a Gateshead GP. The post holder will be expected to work flexibly over 7 days General Professional Duties: To support training and development. To maintain personal professional competency and appropriate development. To carry out the duties and responsibilities of the post in accordance with Trust policies. Required to comply with all relevant national and local statutory and mandatory requirements including Health & Safety and Infection Control The post will involve travel locally and the post holder will be required to work flexibly to support the community business unit.
