Clinical Safety Navigators are responsible for the oversight and management of safety and risk to our patients within our Emergency Operations Centres. A key part of the role is to maintain oversight of the Trusts awaiting incidents, with the capability of managing EOC clinical resources to provide clinical safety assurance and risk mitigation to our patients. In order to be a successful Staff in our team you will: Be registered with the NMC. Have experience and training of managing unconscious patients and working closely with care staff and multi-disciplinary team, anaesthetists and allied professionals. Possess prior experience working with adults with complex mental health difficulties Be passionate about working with adults with complex mental health difficulties It would also be advantageous if you are: Have experience delivering a high standard of care in clinical environments and off-site units delivering treatments requiring the use of anaesthesia. You must be a GPhC (GB) registered pharmacist and have gained a post-graduate clinical pharmacy diploma or equivalent experience. Excellent verbal and written English communication and presentation skills are also essential. This is for an experienced pharmacist with a proven record of being a flexible team player able to cope with unpredictable work patterns. The role is a permanent part-time post.

The post holder will work as part of a team to provide a safe, cost effective, efficient care environment for patients, relatives and carers. They will be expected to function clinically within the Operating Department and other associated areas as required.

The role will range from handling GP and patient queries for clinics, navigating e-Referral Service and tertiary referrals , making appointments using the various trust systems, answering telephone calls from patients or GPs wishing to make or change appointments or making general enquiries. Working as part of a team, including clinical staff, the post holder is expected to deliver a responsible and high quality appointment booking service to patients and other service users.An up-to date working knowledge of clinics will be needed to enable the post holder to make, amend and cancel appointments and handle queries from internal and external sources, as this is a multi-skilled and multi-functioned role. The main duties will be answering telephone calls from patients or carers wishing to make or change appointments or making general enquiries, as well as dealing with email requests.

Clinical Carry out a full range of delegated duties, including assessment, planning, implementation and evaluation of the patients condition and care with support from the registered nurse. Contribute to the daily preparation of clinical areas. During the admissions and discharge phase of the patients perioperative pathway provide skilled knowledgeable assistance. Ensuring accurate documentation of observations and effectively communicate pre and post-operative information. Responsible for the monitoring, documentation and handover of patients care. Manage and administer medications in line with NMC Nursing Associate guidelines and Trust policies. Education and Training Successfully complete key learning outcomes from the preceptorship and or local induction and competency programme during the agreed time parameters. Initiate own development within agreed parameters with the Team leader. Be involved in any aspects of training that needs to be cascaded to the whole team such as moving and handling/infection control. Support the induction of new staff to the department. Act as supervisor for unregistered and new to role staff once appropriate training has been given. Within the scope of practice as a nurse associate be prepared to extend knowledge and skills by rotating to other areas in consultation with the department lead. To support and use of technology to enhance patient care within the department. Be involved in the correct use of equipment and assist in the training of others as required.

The successful candidate will be part of a small team intent on driving the implementation of national best practice guidance for lower limb care. We hold a unique position in the south east cost where lower limb traumas are referred within a day of injury which with a prompt assessment and implementation of a robust care plan can reduce the risk of the development of leg ulcers. They will be responsible for assessing, diagnosing, implementing, planning and reviewing care needs for the lower limb patient. It is desirable that they are experienced in performing complex Doppler assessments, interpreting ABPI and implementing treatment according to findings. Patients are assessed initially from a photo on referral after which a plan is formulated, putting the patient at the centre of the assessment process. You will need to use clinical expertise and work autonomously, although a weekly MDT discusses more complex patients when needed. You will be expected to attend patients in all environments they may be in, and offering specialist advice to nursing homes, district nurses, practice nurses and acute nurses in their care of their patients. Therefore you must be a confident car driver with UK licence experience, a car is provided during working hours. A candidate must have excellent interpersonal skills,using communication of all forms, and skilled in the use of information technology. Undertake wound therapies, such as negative pressure wound therapy, larval therapy, ABPI assessment and application of compression therapies as required. In cases of larger lower limb wound where good conservative management will still lead to a lengthy healing time, pre-assess patient for skin grafting and provide the initial postoperative care, handing over to community or acute teams when appropriate. The job role also involves visiting patients at home for the QVH, if there is significant reasons the patient can not attend to have a dressing change, so a working knowledge of a variety of plastics procedures is of benefit, but not essential as a period of supernumerary time can be spent with the dressings clinic or wards.

The post holder will be required to work as part of a skilled theatre team to support the provision of an effective and safe clinical service. Work as part of a team under supervision to provide a safe, effective environment of care for patients, relatives and their carers. Under direction of a registered practitioner, to support a comprehensive level of care is given, maintaining high standards and continuity of care to patients. Support the registered practitioners in the delivery of direct and indirect patient care. Under the direction of a registered practitioner, undertake a range of delegated clinical peri-operative duties including record keeping, patient observations and specimen handling. Assist the theatre practitioners in the checking of sterilised equipment, ensuring related paperwork is in order and completed correctly. Communicate with peri-operative team, patients and their carers in a professional and compassionate manner. To escalate any concerns about a patient to the relevant practitioner immediately. Assist patients when necessary whilst ensuring the dignity of the patient is maintained at all times, e.g. undressing, chaperoning, emotional and physical support overcoming barriers to communication as necessary. Acting as a positive role model for peers, supporting other HCAs as required. Maintain professionalism at all times and respect colleagues. Responsible for supporting the organisation, planning, delivery of peer support across health and justice services, in keeping with Trust operational and strategic plans and policies, ensuring the delivery of effective, efficient and high-quality services for service users. To challenge, support and work in partnership with health and justice teams (using personal lived experience) in delivering the strategic direction of the peer recovery working approach. Identify and monitor peer/lived experience integration issues, bring this to the forefront for open dialogue and discussions. Championing peer support across the Trust, inter-organisationally and nationally. Provide training, presentation and representation of peer support as appropriate. To take responsibility to supporting peer/lived experience workers within Health and Justice Services. Complete key peer/lived experience support project work as agreed including initiating, researching, liaison with others, developing and delivery. To monitor and report on peer/lived experience support development and delivery.
