Job overview a) To identify and carry out the specialist assessment of individuals who may be at risk of falls and fracture. b) To plan and provide programmes of care according to a persons individual and complex needs and without direct supervision in a variety of settings, including clinics, exercise groups, health promotion, and education c) To manage a caseload of patients utilising specialist clinical assessment, diagnostic and treatment skills in the management of patients at risk of falls d) To provide training and education on falls prevention to a broad range of staff working across the local health and social care sector, including Residential and Home Care Services, and voluntary organisations e) To supervise assistant staff/support workers and undergraduate students and provide training to staff and carers f) To develop strong links with the Primary Care Trust, Sussex Ambulance and the Voluntary and Independent Sector and local community to deliver an effective service g) Adhere to professional standards and guidelines of practice, education and conduct h) To support the Falls and Fracture Prevention Service lead in the development and delivery of the Falls Prevention Service and develop the role of the Falls Specialist Practitioner i) The service will initially be provided Monday to Friday but as the service develops the post holder will be expected to work as part of a 7 day service 2. Communication and working relationships a) Describe To communicate complex/sensitive information to service users where there might be barriers to understanding b) To lead brave conversations around sensitive aspects of care planning c) To work collaboratively with relevant SCFT team leads, service managers professional lead and clinicians as well as leads within other organisations. d) To work collaboratively with other professionals to ensure delivery of effective clinical leadership across sites and services.
