Support Worker - Grade D
Primary purpose
of role
To provide support and a range of activities to adult service users with learning
Disabilities and complex needs. To work as part of a team, or on a one-to-one
basis, under direction, both within a day service or community setting, in line
with individual support plans and assessed needs.
The post holder will be working closely with, and supporting, vulnerable adults.
Key activities
Support service users with all aspects of personal care, eating, drinking and
PEG feeds. Ensuring a person is treated with dignity, sensitivity and respect
at all times. (40%)
Support and empower service users to develop daily independent living
skills, including simple snack/ meal preparation, shopping, managing service
user’s personal monies. Enable service users to make informed choices,
delivering a range of identified activities. (10%)
Use a variety of communication methods to aid communication,
independence and choice making. (20%)
Assist with the safe administration of medication and emergency rescue
medication, in accordance with the health/ medical needs of service users.
(5%)
Support customers to access social/community activities of their choice,
build, and sustain relationships with their peers, family, friends and wider
community. (15%)
Undertake daily report writing, including the completion of service user’s
daily diaries, Incident/Illness Report Forms, seizure record charts and
assessments. (5%)
Advocating for individuals who attend the service to ensure their voice is
heard. (5%)
Carry out other duties appropriate to the grade of the post
Essential
qualifications/
knowledge
Literacy skills to keep records and contribute to reviews and meetings.
Experience of working with people with learning disabilities and complex
needs.
Knowledge and experience of the need for dignity, respect and sensitivity
when supporting the service user group.
Undertake personal care tasks in accordance with Manual Handling
Regulations.
Evidence of a commitment to continuous personal development.
Experience and understanding of people with a learning disability.
Knowledge of the need for dignity, respect and sensitivity when working
with people with a learning disability.
Basic level of literacy and numeracy.
Basic IT Skills.
Essential to Driver Role only
Must hold a current, full UK driving license.
PCC Minibus Driver Certificate or willingness to attend the training course.
PCC Driver & Passenger Assistant Certificate or willingness to attend the
training course.
Desirable
qualifications/
knowledge
NVQ Level 2 in Care/Health and Social care Diploma
Learning Disability Qualifications (LDQ)
Knowledge and/or experience of working with people who challenge the
service.
Knowledge of relevant Plymouth City Council policies and procedures.
(E.g. Equal Opportunities, Health and Safety, Risk Assessments, Manual
Handling Regulations).
Knowledge of other relevant legislation covering
Handling Regulations).
Knowledge of other relevant legislation covering disclosure, safeguarding
health and safety, discrimination and human rights.
Current, full UK driving license
Essential
experience
Physical skills, co-ordination required for operating specific equipment such
as hoists, feeding and mobility aids while supporting service user’s personal
care and mobility needs.
Effective communication skills through Total Communication, with the use of
signs, symbols, photos, gesture and objects of reference.
Skills to implement individual and group activities/sessions, with clear aims
and objectives.
Ability to support service users with transport requirements.
Caring skills required as jobholder is dealing with the basic needs of
demanding service users.
Supporting the demands of service users, especially when working in the
community, requiring some interpretation of information or situations to
respond to service users’ needs.
Desirable
experience
Knowledge and understanding of Safeguarding Adults local and national
procedures and legislation.
Knowledge and understanding of the Skills for Care: Code of Conduct for
Healthcare Support Workers and Adult Social Care Workers in England.
