We are looking to recruit a Band 6 Community Respiratory Healthcare Professionalbased at The Coley Clinic in Reading.
We are looking for someone who will lead on raising awareness in chronic obstructive pulmonary disease (COPD) management and we will give you the opportunity to develop upon existing knowledge and skills alongside developing an understanding of service needs and organisational strategies from a management perspective.
Youwill be lookingafter the Berkshire West Community Based Pulmonary Rehabilitation Programme, part of the Community Respiratory Service, based within the Cardiac and Respiratory Specialist Services.
You will have experience in respiratorycare andpulmonary rehabilitation, experience with old age medical conditions and supervision of junior colleagues and students. We are looking for someone who will lead on raising awareness in chronic obstructive pulmonary disease management.
As part of the Community RespiratoryTeamyou will use clinical reasoning skills, clinical research,auditand data collection to provide support and advice in a community setting.

• Work as part of an integrated team leading the care for those with chronic respiratory disease.
• Work as part of a 2 Hour Rapid Response Service with the aim of reducing the need for acute service intervention – reducing admissions, supporting early discharges, supportingcolleaguesand empowering patients to manage their condition with theultimate aimof improving quality of care.
• Tobe responsible forrunningthe Berkshire West Community Based Pulmonary Rehabilitation Programme
• Work within an integrated service to improve the lives of patients with respiratory conditions and heart failure.
• Work as part of the Home Oxygen Assessment and Review Service (HOS-AR) toassess patientsin Berkshire West who have an oxygen prescription, ensuring that this isappropriate forthe patient’s needs and requirements.
