Major duties and responsibilities will include:

The technician independently operates and monitors neurodiagnostic equipment in the Integrated Sleep Disorders Center Richmond VAMC, VISN 6. The technician performs a full range of standard, but complex examinations including overnight sleep studies, MSLT (multiple sleep latency tests), and PAP (positive airway pressure). They are responsible for properly and accurately calibrating all instruments prior to PSG tracings. Following laboratory protocol, they independently determine the need for CPAP therapy. Technicians at this level will provide briefings and orientation about sleep studies and lab policy to residents, fellows, and other personnel who may rotate thru department. They may serve as mentor to lower level technicians.

Duties at this level may involve more complex procedures (under closer supervision) to prepare the technician for promotion to higher levels.

The technician will operate computerized polysomnographs for recording continuous all night physiological data including EEG, EOG, EKG, EMG, and respiratory parameters. They will use devices such as oximetry, respiratory effort devices, nasal pressure devices, thermistors, thermocouples, continuous positive airway pressure (CPAP) machines, and laboratory computers. They will select and fit appropriate CPAP patient interface devices and provide education to patients during the daytime CPAP Group Clinics. The technician will conduct routine maintenance and adjustment checks on the equipment during the daytime CPAP Clinics (CPAP Maintenance Clinics, Walk-ins, Telehealth Clinics). Additionally, he/she will be able to address CPAP compliance via machine downloads and remote monitoring with machine modems. The technician will recognize and report any equipment failure to the supervisor.

The technician will know medical terminology, anatomy, physiology as it relates to sleep disorders. They have advanced knowledge of The International classification of Sleep disorders and the ability to recognize uncommon and or rare sleep disorders. The technician will understand the relationship of events, sleep stages, and possible medical conditions which influence or result from events occurring during sleep. They will use this knowledge to review medical records and read and understand routine and complex sleep study orders. They will know normal and abnormal results and will document abnormalities during procedure.

The Polysomnography (PSG) technician will utilize standard or alternate position for patients with common physical disabilities for the procedure being performed. They will utilize two methods of patient identification. They will explain the testing procedure to the patient. They will utilize the 10/20 method of electrode application during procedures. The technician will follow established laboratory protocol in performing standard overnight sleep studies, PAP, and MSLT's. The technician will independently titrate and document PAP level to achieve therapeutic goals. They will know the indications, complications, and contraindications to CPAP therapy. They will know how to use Bi-Level PAP therapy. They will obtain written permission to perform audio and video during the procedures that require such. They will recognize biologic and non-biologic (artifacts) EEG waveforms. They will know corrective actions to eliminate artifacts. They will recognize all levels of sleep. The technician has knowledge of event characteristics (ie. respiratory, cardiac, sleep stages, seizures, etc.) and will take appropriate action. They will make changes in recording techniques and parameters to optimize display of the clinical question (e.g. seizure monitoring, parasomnias). They will score and analyze sleep stages on the recordings. The technician recognizes obvious clinical manifestations of various types of epileptic and non epileptic seizures as they occur. The technician annotates observed behavior or anomalies on recordings. They use equipment specific processing software to execute several automation functions such as storing, retrieving, and transferring, electronic document and files. The technician will summarize and report Polysomnographic data in narrative form. They will recognize life threatening or medically emergent patient situations and take appropriate action to provide emergent care.

1. Perform a complete echocardiogram test by obtaining and recording real time two dimensional ultrasound pictures, m-mode, pulsed and continuous wave and color doppler. Incumbent will be responsible for specific measurements and calculations that will be included in the echocardiogram test.

DUTIES:
Home Sleep Testing Program:
a. Review the patient's history and verify the medical order
b. Verify patient identification, complete the required paper work prior to issuing the equipment.
c. Patient education and instruction appropriate for home sleep testing
d. Must adhere to cleaning and disinfection procedures for portable monitoring devices
e. Scoring portable monitoring recordings according to AASM scoring parameters; identify artifact, inadequate signals and study failures and generate a report for sleep physician to review
f. The technologist will be responsible for screening consults for home sleep studies or in-lab polysomnogram, scheduling the procedures deemed necessary and maintaining the portable sleep technical needs and clinic schedules.

Salem VA Medical Center and Affiliated CBOC CPAP/BIPAP Clinics:
a. Applicant will be responsible for setting up CPAP and Bi-level equipment, fitting patients with the correct mask type and size, and educating them on their therapy so that they are able to be successful and compliant with therapy
b. Instruction for patient training for nebulizers and oxygen
c. Help patients with sleep equipment including oxygen and provide education accordingly
d. Process document orders including machine exchanges, update and confirm future orders.
e. Review patient file for complete and thorough documentation acquisition
f. Supports orientation and training of other personnel at the direction of the Supervisor

Carpentry
The incumbent uses skilled techniques to construct, install, repair, and modify wood and wood substitute structures and items requiring close tolerance fit and structural soundness. Typical work assignments include constructing and repairing a wide variety of structural items such as building frames, rafters, concrete forms, walls, staircases, door and window frames, interior and exterior trim, and miscellaneous items such as workbenches, counter tops, bookcases, equipment cabinets, computer consoles, and items requiring limited decorative trim and molding. The incumbent lays out and installs various modular furniture assemblies. The incumbent sets up and operates a wide variety of wood working machines and tools. Plastering
The work also involves mixing, application, and finishing of plaster surfaces in the construction and repair of interior walls and ceilings. The incumbent performs new - plastering on all types of backgrounds such as metal, wood or gypsum lath, wallboard, and masonry surfaces, as well as repairs and patches plaster and stucco surfaces.

Masonry
Work also involves the maintenance, repair, alteration, and construction of exterior and interior surfaces and structures built of a variety of brick, block, and stone. The incumbent works from oral instructions, blueprints, work orders, and rough sketches. The incumbent plans and lays out the work to be done, selects the proper tools, determines materials to be used, and accomplishes the work in accordance with appropriate methods and accepted trade practices. The incumbent lays common and face brick, firebrick, cinder and cement block, glass block, terra cotta, and various artificial and natural stone in constructing and repairing such items as walls, walks, manholes, boilers, and catch basins. Painting
Paints surfaces by patching, sanding, dusting, and applying paint by brush, spray, or roller to interior or exterior surfaces. Determines the methods, techniques, devices, and materials that are best suited to the surface finish required to insure surfaces have a smooth textured and decorative appearance, for example glossy finish, flat finish, grained effect, or a finish that must meet specified thickness. Mixes and applies pre-coating agents such as pickling and alkali compounds to remove scale, rust, dirt and old paint from surfaces so that coating materials will adhere properly leveling dents and scars. Applies coating materials including paint, varnishes, shellacs, and special painting materials with any of the accepted trade devices including brushes, rollers, rollers with extension handles, airless spray guns including pole guns and graining devices designed to give special effects. Uses coating techniques that insure surfaces are fully coated, protected and free from drips and runs.

Performs other directly related duties as assigned.

The Pharmacy Technician duties consist of, but are not limited to the following:

Medication Distribution System

• Complies with all VHA, Medical Center, and pharmacy regulations, directives, policies, and procedures.
• Is knowledgeable on pharmacy production reports and how to interpret them for comparison to individual production.
• Reads and interprets prescriptions from the veteran electronic health record systems (EHRS) or in written format.
• Reviews prescription orders for technical accuracy.
• Prepares medications and ensures the orderable item matches the item being dispensed regarding drug, strength, quantity, or appropriate supply item. Subject to a final check by a pharmacist.

• Able to use the current facility phone system.
• Maintains appropriate phone etiquette and professionalism.
• Able to work independently in phone center locations.
• Accurately collects and clarifies patient information.

• Fills medication "to be mailed" in accordance with description provided in the medication distribution section.
• Ensures prescription label matches mailing label and maintains accuracy in other software or paper systems.
• Able to transport mail containers to pick up destinations, restock supplies, lift mail bags or containers.
• Assists with other medication distribution activities as directed by pharmacist, supervisor or as delineated as part of the shift responsibility.

• Assists the pharmacist with problem solving patient care or medication issues.
• Assists in quality improvement projects as an individual or on assigned teams to improve pharmacy service.
• Assists to complete required pharmacy reporting obligations as directed.
• Maintains pharmacy security. This includes the physical security of the pharmacy and the security of electronic records.

• These functions will be supported assisting in or the absence of the sterile compounding technician (i.e., after hours, weekends, and leave).
• Follow USP 797/800 Guidelines for the compounding of all sterile preparations, including ophthalmic, cardioplegic, TPN, hazardous, large & small volume IV solutions, PCAs, etc.

• Performs review of medications prepared by another pharmacy employee to be dispensed to hospital areas.
• Inventories and replenishes emergency trays. 3. Operates and maintains automated dispensing unit dose systems.
• Inspects packaged items for accuracy and suitability before submitting for final check, prior to distribution to hospital areas.

• Correctly inventories, assesses, and determines the need for floor stock and external medication.
• Follows established policies and procedures, schedules, inventory levels, and usage rate reports to determine appropriate stock levels for specific areas of use.
• Accurately fills prescription items for wards using Automatic Replenishment reports.
• Performs and documents an inspection of assigned hospital areas for medication storage.
• Removes all expired and deteriorated drugs from the automated dispensing stations, stocks shelves, and the unit dose automated dispensing systems.
