Reporting to the Programme Lead, the Community Case Manager is key in managing care services for clients and enhancing the capabilities of community carers.
Roles & Responsibilities:
Client Care
- Conduct home visits to assess clients' needs and create personalized care plans.
- Perform psycho-social assessments to recommend suitable service linkages.
- Administer nursing procedures (e.g., monitoring vital signs) in home settings, ensuring safe and efficient use of resources.
- Educate patients and caregivers to improve health outcomes and self-care.
- Collaborate with external healthcare partners to coordinate seamless care.
- Maintain accurate visit logs and client documentation.
Team Leadership & Development
- Lead and support a team of micro-jobbers, overseeing skill development and work scheduling/assignments.
- Provide guidance on job-related tasks and address challenges faced by micro-jobbers.
- Attend trainings and case conferences to enhance client care and support for caregivers.
Strategic & Administrative Support
- Collaborate with the Programme Lead to plan, execute and enhance program operations and strategic direction.
- Engage in projects as assigned by supervisors.
- Handle administrative tasks, including data collection and report preparation.
- Ensuring compliance with healthcare regulations and organizational policies.
Candidate Profile:
- Registered Nurse with the Singapore Nursing Board.
- Diploma or degree in Nursing.
- Minimum 3 years of experience, with at least 1 year in community nursing preferred.
- Additional qualifications in social work or case management are advantageous.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Passion for elderly care in community settings.
- Proficient in Microsoft Office.
