DUTIES

• Focuses on researching, diagnosing and treating skin disorders and diseases.
• Complete patient examinations and may perform minor skin surgeries. Job duties also entail prescribing topical medication for skin problems.

  • 1. Type of skin cancer or noncancerous or precancerous growth

• Examinations-Evaluates patients by examining thoroughly. Appropriate clinical, laboratory and other pertinent data is obtained to determine care the patient is provided.
• Diagnosis--Interprets and evaluates clinical data for the purpose of determining diagnosis and clinical status.
• Therapeutic Ability--Prescribes appropriate and timely treatment; applies indicated therapeutic procedures; recognizes and assesses changes in behavior, symptoms, signs, and other pertinent facts about the patient and modifies plan of treatment when indicated. Psychiatrists - Major duties and responsibilities include:

Mental Health Connect: Core responsibilities include providing include providing triage and evaluation services to Veterans new to Mental Health. Duties will include consulting with primary care physicians, providing brief and comprehensive mental health assessments as well as time-limited follow up to Veterans not established in specialty mental health. Incumbent must be skilled in general outpatient psychiatry, including diagnostic evaluations, pharmacotherapy and psychotherapy. Competent in the management of individuals with a broad range of clinical problems, including post-traumatic stress disorder, mood, anxiety and psychotic disorders, substance use disorders and psychiatric disorders in the elderly Veteran population.

General Psychiatrist: Core responsibilities include providing comprehensive psychiatric medical care (assessment, consultation, treatment)
to patients on the Mental Health Diamond Team based in Dallas or patients served at the Fort Worth Mental Health Clinic. Physician should be competent in the management of individuals with a broad range of clinical problems, including but not limited to post-traumatic stress disorder, mood, anxiety and psychotic disorders, substance use disorders and psychiatric disorders in the elderly Veteran population.

Addiction Psychiatrist: Core responsibilities include providing comprehensive psychiatric medical care (assessment, consultation, treatment) to patients in the Dallas Mental Health Gold Team, an outpatient clinic focusing on providing care to Veterans with addictions and substance use disorders, or Diamond Team, a general mental health team.

Primary Care - Mental Health Integration Psychiatrist: Core responsibilities include providing integrated mental health care in primary care clinic settings through collaboration with primary care physicians and primary care nurses. Physician should be competent in the management of individuals with a broad range of clinical problems, including but not limited to post-traumatic stress disorder, mood, anxiety and psychotic disorders, substance use disorders and psychiatric disorders in the elderly Veteran population.

The staff audiologist conducts a full range of audiological evaluations to determine the type and degree of hearing loss, determines patients' need for amplification, predicts and measures the effectiveness of amplification, and determines need for and type of rehabilitation and counseling of veterans with hearing loss and their families. The staff audiologist conducts and interprets behavioral, electroacoustic, or electrophysiological methods used to assess hearing, tinnitus, balance and neural system function.

Duties:

• Conduct, plan, and coordinate all clinical and administrative aspects of a full range of services associated with contemporary audiology practice.
• Perform clinical duties and responsibilities to include direct patient care involving a full range of audiology and treatment services. This will include selection and fitting of hearing aids and other amplification systems and devices, selectin and fitting of hearing protection and custom ear protection devices, aural (auditory) rehabilitation, auditory brainstem response (ABR), otoacoustic emissions, balance evaluation, and counselling of hearing impaired veterans and family members/caretakers.

The Primary Care Provider (PCP) Float will provide primary care services in support of primary care operations in the San Antonio service area. As a float provider, this position will provide support to Patient Aligned Care Team (PACT) at an assigned San Antonio Clinic and may be detailed to other VA Clinics in San Antonio as staffing and workload dictate.

THE PCP Float may be asked to provide coverage for a vacant/absent PACT provider to supplement PACT services by delivering same-day care, routine scheduled care, or new patient intake for PACT panel(s).

The Primary Care Physician is responsible for providing Comprehensive Primary Care for Veterans and should, in the context of a longitudinal relationship, fulfill all primary care needs, including acute and chronic illness, gender-specific PC, mental health (Screen for conditions such as depression, substance use/abuse, military sexual trauma and PTSD, and evaluate and treat uncomplicated mental health disorders as well as substance use disorders and refer patients appropriately for further evaluation and treatment of sexual assault and military sexual trauma, mental health disorders and substance use disorders) and preventative care (age-appropriate cancer screening e.g., colon, breast and cervical cancer - as well as weight management counseling, Chlamydia screening (when indicated), smoking cessation and immunizations, etc.) along with coordination of care delivery (when specialty care is needed, the primary care provider will coordinate this care and communicate with the specialty provider regarding the evaluation and treatment plan to ensure continuity of care).

The PACT Model is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of health services through an interdisciplinary team that includes the PCP, RN Care Manager, LVN, clerical associate as well as other clinical services necessary to meet the health goals and needs of the Veteran patient including specialists. While the Primary Care provider's relationship with the Veteran patient will remain key, that bond will be augmented and strengthen by involving other health care staff as part of an integrated team. Together the team takes responsibility for the Veteran patient's ongoing care through a cooperative effort among all members.

Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status, or demographic status (e.g., women veterans, OIF/OEF veterans, employees, etc.). Typically these duties are designated at the time of joining but occasionally during the course of employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives.

Physicians may be assigned duty as a supervising or collaborating physician for one or more mid-level providers. Physicians are expected to provide clinical oversight and review the care provided by mid-level practitioners. Most providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave.

The Physicians (MD/DO) in Primary Care (PACT) is accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care. The MD/DO works in collaboration with heath care professionals to provide longitudinal case management of the PCP's entire PACT panel with independent care delivery for acute/same day needs, or when otherwise requested by the PCP.

The Physician, Patient Aligned Care Team duties include, but are not limited to the following:

• Provide independent care delivery for chronic, acute/same day medical needs. The type of care is served to the Veteran population, with diseases consistent with those seen in the general population namely diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, etc. A part of your responsibility is to write orders for diagnostic testing, prescribe medications, consult with specialty service, and utilize computerized/electronic medical records. Major function of the position includes Health risk Assessment, goal setting for prevention, and chronic diseases management. A major focus is in preventive medicine with focus on the achievement of positive outcomes for patients, improve coordination of care, decrease clinic waits or delays, and appropriate use of resources.
• Provide direct patient care to include, but are not limited to: history and physicals, medication management, evaluation of diagnostic tests and treatment planning, clinical reminder completion as applicable.
• Function as a leader in the PACT approach in the delivery of care, which emphasizes a team-based approach to healthcare. Oversee PACTs compliance to Message Manager, MyHealtheVet, and encounter completion. Proficient in Tele-Health to home visits (VA Video Connect), and Comprehensive Women's Health care.
• Document all medical information in an electronic medical record.

The Associate Chief of Staff, Geriatrics & Extended Care will serve as Medical Director for Geriatrics Program at Central Texas VA Health Care System (CTVHCS) and will supervise other Physicians, Fellows, Nurse Practitioners and Physician Assistants in CTVHCS. The Physician will also work as an Attending Physician in Hospice and Palliative Care Program, taking care of Veterans who need hospice and palliative care to improve quality of life for patients who have a serious life limiting or terminal illness focusing on quality of life through relief of symptoms and attending to their emotion and spiritual needs and end of life care.

The Associate Chief of Staff, Geriatrics & Extended Care will be responsible for consults, admissions, discharges, and routine care of the Veterans in these programs. He/she will also help coordinate care with other members of the Interdisciplinary Team and with Specialists inside and outside the VA and provide education on disease process, medications, and care needs to Veterans, families, and staff. Will participate in screening Veterans for CLC admission, both hospice and other treating specialties within CLC.

• May be tasked with leading and managing installation or upgrade projects for medical devices and systems. This includes pre-implementation planning and coordination with end users, vendors, and other onsite support staff (OI&T, Informatics, SPS, etc.).
• Additional duties are performed as assigned by supervisor related to maintenance, remediation, and tracking of medical systems (action items, special projects, etc.) to meet mission critical requirements. May be asked to assist in other administrative duties as well, including participating in hiring panels.

• 3 - 5 years' experience in a hospital or medical device provider setting
• Medical Device Networking, including DICOM and PACS System, experience
• Certifications such as CBET, CRES, or N+ preferred
• Ability to work as a team player
• Ability to communicate with clinical staff

You will serve as a clinical diagnostic radiologist. You will provide interpretive and patient care services within the scope of a clinical diagnostic radiologist and will have responsibilities in other areas such as education. You must demonstrate the knowledge and skills necessary to provide all applicable diagnostic imaging services. May perform non- endovascular procedures if qualified and privileged to do so. Must possess sufficient computer skills necessary to work with advanced electronic medical record, PACS and voice recognition systems. The majority of the time will be devoted to clinical care with sufficient time for administrative duties as assigned by the Chief, Imaging Service.

Pay: Competitive salary, annual performance bonus, regular salary increases
