This role offers an excellent opportunity for a senior nurse to take a leading role in advancing the care of patients transitioning from paediatric to adult services. As a newly created position, there is significant potential to shape and develop the provision of care for this patient group.

The post holder will collaborate closely with the Paediatric Team, Paediatric Consultants, Adult Medicine Team, MDT members, and both clinical and administrative colleagues across the Paediatric and Adult services.

This is a full-time position, working 37.5 hours per week (Monday to Friday). Cross-site working may be required depending on service needs.

West Hertfordshire NHS Teaching Hospitals Trust is developing a model of care to prepare and support young people in accessing an exemplary co-ordinated transition pathway between both Children and Adult services.
The Children and Young Peoples Transition Nurse Specialist will be an integral part of this developing service.

The role will include:
• Ensuring that the transition of care from children's to adult services is appropriately managed and delivered effectively within West Herts
• Facilitation of accurate, open communication and coordination with a range of people organisations, divisions, departments, and multi-professional stake holders.
• Be a key member of the Children and Young Peoples team as well as supporting effective communication and stakeholder management.
• Work collaboratively with the ‘Carer for carers service’.
• Ensure that the views of young people are intrinsic in the models of care developed.
