Main Purpose: To provide skilled high quality care to children and young people with complex medical needs, in the home or community setting. To carry out delegated clinical tasks providing support for children with complexhealth needs and those dependent on technology, following individualised care plans developed by the nursing team, having become competent through an intensive and on-going training program and with support and guidance from registered nurses. Main responsibilities: 1.Work as a member of the Childrens Continuing Care Team (CCCT) providing care shifts to support families of children with complex care needs, carrying out delegated clinical tasks with supervision from CCCT Nurses.2. Provide day or night care shifts as per a rota determined by each childs allocated care package, in the childs home or community e.g. school, place of respite.3. Provide a high standard of care at all times, meeting the individual needs of the child or young person and being attentive and alert throughout the shift to ensure the childs needs are fully met.4. Work closely with families and CCCT to promote the childs well-being.5. Accurately observe and record the childs condition, noting any changes and initiating appropriate action as required.6. Maintain up to date and accurate records in accordance with NHS standards for records and record keeping.7. Report any changes in the child/young persons condition or potential hazards in the environment to the family and the On Call/Lead CCCT Nurse.8. Report incidents and accidents to the Lead CCCT Nurse. Have an understanding of risk assessment management.9. Acknowledge the parameters of the role and refer any issues beyond the scope of the role to the On Call/Lead CCCT Nurse.10. Ensure a safe environment and safe use of equipment.11. Ensure fully conversant with emergency procedures.12. Participate in and complete an orientation and on-going training program comprising face to face and online education, ensuring that competencies in specific clinical procedures set out in protocols relevantto the child or young person are achieved and maintained.13. Maintain clinical knowledge and skills. To keep up to date with changes in practices and make full use of relevant opportunities for self-development.14. Attend mandatory training and safeguarding supervision sessions.15. Attend planned clinical supervision sessions and seek support when required. Frequent contact with families with complex health needs requires strong interpersonal skills and a high degree of professionalism, emotional resilience. The ability to ask for support is imperative when dealing with distressing and disturbing situations.16. Provide ad hoc support to the Childrens Outreach Nursing Service.

This role is within the department of Medical Oncology; the appointed candidate will join an existing team of 6 senior medical staff including Consultants & SAS doctors. On joining the department, you will become part of a well-established, friendly team and will help us continue to drive forward improvements in the service that we provide to our patients. As a senior member of the team, you will become a key part in the delivery of training and supervision to our junior colleagues. Clinical Duties:
• Multidisciplinary team working is key component of the working within the Trust and involves regular MDT discussions with colleagues
• Providing evidence-based care for patients in a multidisciplinary setting.
• Continuing responsibility for the care of patients in your charge, including all administrative duties associated with patient care (with secretarial support)
• Development of sub specialty interests that fit in with Division needs and the strategic direction of the Trust.
• Responsibilities for carrying out teaching, examination and accreditation duties are required, and for contributing to undergraduate and post-graduate and continuing medical education activity.
• Provision of cover for Consultant colleagues’ periods of leave in accordance with arrangements agreed within Trust policy.
• Participation in clinical audit and in continuing medical education.
• Conducting suitable duties in cases of emergencies and unforeseen circumstances.
• The post holder will not participate in an out of hours consultant rota, detailed within the job plan section
