Main Purpose: To provide skilled high quality care to children and young people with complex medical needs, in the home or community setting. To carry out delegated clinical tasks providing support for children with complexhealth needs and those dependent on technology, following individualised care plans developed by the nursing team, having become competent through an intensive and on-going training program and with support and guidance from registered nurses. Main responsibilities: 1.Work as a member of the Childrens Continuing Care Team (CCCT) providing care shifts to support families of children with complex care needs, carrying out delegated clinical tasks with supervision from CCCT Nurses.2. Provide day or night care shifts as per a rota determined by each childs allocated care package, in the childs home or community e.g. school, place of respite.3. This role is within the department of Medical Oncology; the appointed candidate will join an existing team of 6 senior medical staff including Consultants & SAS doctors. To prioritise duties to meet the changing and conflicting demands in Outpatient clinics, rescheduling tasks as necessary and providing a prompt response to requests for records required at short notice. To archive case notes according to local procedure, updating the Patient administration system (PAS). Processing complex and sensitive digital and paper referrals as required, ensuring that all required data items are completed when registering new referrals and patients on PAS. The role

As a Functional Assessor, your main task will be to assess individuals and how their disabilities or conditions affect their work life. Drawing on your clinical expertise, you'll listen carefully and take down each person's medical history before performing a physical examination. You'll then produce concise, accurate reports for the Department of Work & Pensions (DWP) so they can decide each person's eligibility for benefits.

The role involves meeting people both face-to-face and via telephone/video. But whether you connect online or in person, you'll be sensitive, compassionate and able to ask the right questions while upholding best practice standards.

Rest assured that you'll have the time you need to make a precise assessment and give each person your whole attention. And while you'll work unsupervised, you'll be part of a wider multi-disciplinary team - support is all part of the package.

About you

• An HCPC registered Occupational Therapist with a minimum of 12 months' post-registration experience within the past five years.
• Continuous Professional Development present on your CV.

• Self-aware and focused on professional development
