This role will support the Clinical Pharmacy service within ESHT, and the successful candidate will have the opportunity to work closely and be supported by both the Lead Divisional Pharmacist for Medicine and the newly created Medicines Value Programme Lead Pharmacist, in further developing this role. The Medicines Value Programme is a new and ambitious project that is designed to provide enhanced medicines optimisation within the specialisms with a commitment from the Trust to develop exciting, advanced pharmacist practitioner roles. These roles are designed to support practitioners who wish to develop a portfolio of practice for credentialling against advanced and consultant level frameworks.We are looking for a forward thinking and motivated pharmacist who is looking to take a step into leading on pharmacy projects and developing a scope of practice within one or more of the specialist medicine clinical specialities at ESHT. These are Gastroenterology, Rheumatology, Neurology, and Dermatology.

The post holder will be involved in providing support to Specialist Medicine areas within the Medicine Division. You will work closely with clinicians, senior nurses, and managers within the Division, and you will also provide support and input into the Clinical Pharmacy service by helping to provide support for leadership supervision and developing colleagues and trainees. As such we are seeking a patient focused pharmacist aiming to build on established clinical skills who will work closely with clinicians in providing a high-quality service to patients within ESHT. Role Summary The aim of this post is to provide comprehensive, highly specialised assessment, care, advice and support to patients with suspected or confirmed OG cancer and their cares/family. They act as a key worker and liaise with other healthcare professionals to provide the best possible specialist care for their patients. This will be for the local patient group in addition to those referred from peripheral Trusts for treatment (we are a specialist centre for surgical and endoscopic interventions/ treatment of these cancers and a cancer centre. A Tertiary centre) This will be met through a high standard of knowledge and skills, both academically and through significant experience of the national cancer agenda and its delivery. Work closely with the Trust lead cancer nurse to ensure implementation of national cancer targets. Providing a response and action plan to the National cancer patient survey (NCPS) Provide expert knowledge and support to members of the nursing team in an environment which promotes critical thinking, reflection, development, growth and positivity. In turn a high quality service to patients and carers. Provide an excellent quality and supportive service to patients as part of the OG team, providing nurse led face to face diagnostic clinics and review clinics. This will include liaising with other health care professionals internally and primary health care team in order to provide the best possible care for patients and carers/family. Ensure continuity of care from pre/ diagnosis and throughout the whole patient journey/ cancer pathway; from diagnosis to end of life care. To make decisions autonomously with regards to the whole cancer pathway and act accordingly with the support medical colleagues when required. To act as an expert professional lead and ensure good quality staff support and delivery of high quality cancer care to patients, families and carers. The post holder will act as an OG cancer resource, collaborating with all members of the multi-professional team. The post holder will provide specific nursing knowledge and expertise on all aspects of OG cancer; and is expected to participate in education/ training to other staff members within and external to the trust and as part of the NCPS. The post holder will have a lead role in the development of the OG cancer services ensuring this relates to the needs of the patients and is in line with the NHS long term plan/ The Cancer Taskforce plan/ Improving outcomes Guidance NHSE/ NICE/ Cancer Survivorship Agenda.

Clinical: 1. To manage a clinical caseload 2. To maintain a cohesive team dynamic and be able to respond to and manage the demands of the various stakeholders. 3. To provide clinical advice and support for patients, carers and other healthcare professionals in the treatment and management of dermatological conditions and its complications. 4. To participate in the maintenance and development of local and national service initiatives to provide the highest standards of care in line with accepted best clinical practice and the aims and objectives of the Trust, the local Commissioners and the wider National Health Service. 5. To initiate and develop personalised care plans to ensure high quality care specific to dermatological conditions and facilitate the patients in self-management of their long-term condition, understanding of their potentially life-long diagnosis, long term management of their condition, medication, management of possible complications 6. To be an active member of the multidisciplinary team by using professional judgement and assuming responsibility and accountability for assessing, implementing and evaluating clinical care for patients to meet their needs in a safe caring environment that promotes dignity, privacy and respect. Ward-based Clinical Pharmacy Services Works as part of the ward-based clinical pharmacy team to provide near-patient services, for example medicines reconciliation, ordering of medication for in-patient use, processing and final accuracy checking of discharge prescriptions and discharge counselling. Provides a service to the Patient Orientated Pharmacy (POP) Project, including delivering training and providing cover. Provides advice, with own competence, to nursing staff in clinical areas in relation to supply and availability of medication in order to reduce the incidence of missed doses. Ensures that medicines within the clinical areas are stored correctly and securely, giving feedback to staff, if required, when there are deviations from best practice. General/Dispensary Carries out the supply of ward medicine stock items, temporary stocks for named patients and handles enquiries under the direction of the Dispensary Manager and Clinical Pharmacists, by following the defined SOP's. Dispenses discharge, inpatient and outpatient prescriptions under the supervision of a Dispensary Manager and clinical pharmacist by following the defined SOP's. Performs final accuracy checks of dispensed items, following appropriate training and demonstration of competence, in line with departmental SOPs, for those items that have been clinically checked. Maintains a safe working environment within the department and good internal and external communications with nursing staff and other hospital departments, referring any difficulties to be a pharmacist or senior member of staff. By liaison with clinical pharmacists, through the Principal Technician - Logistics, reviews and maintains ward medicines stock levels. Participates in the topping-up service for drug distribution in the hospital and other off-site locations when required, updating records as necessary. Problem solves and deals with enquiries from patients and users, passing enquiry's to more senior staff when necessary. Provides written and oral information on the use of medicines to patients attending the pharmacy. Maintains adequate medicine stock levels and undertakes stock checks in the dispensaries. Additional Duties Supports junior pharmacy staff in performing their duties. Undertakes a supervisory role in the in-patient dispensary on a sessional basis, ensuring that work flow is maintained and all KPIs for pharmacy operational services are met. Provides personal managerial support to the Principal and/or Chief Technicians. We receive patients from the Emergency Department, the Acute Medical Unit, Medical same day emergency care and specialist clinics from within our hospital.
