The Primary Care Provider (PCP) Float will provide primary care services in support of primary care operations in the San Antonio service area. As a float provider, this position will provide support to Patient Aligned Care Team (PACT) at an assigned San Antonio Clinic and may be detailed to other VA Clinics in San Antonio as staffing and workload dictate.

THE PCP Float may be asked to provide coverage for a vacant/absent PACT provider to supplement PACT services by delivering same-day care, routine scheduled care, or new patient intake for PACT panel(s).

The Primary Care Physician is responsible for providing Comprehensive Primary Care for Veterans and should, in the context of a longitudinal relationship, fulfill all primary care needs, including acute and chronic illness, gender-specific PC, mental health (Screen for conditions such as depression, substance use/abuse, military sexual trauma and PTSD, and evaluate and treat uncomplicated mental health disorders as well as substance use disorders and refer patients appropriately for further evaluation and treatment of sexual assault and military sexual trauma, mental health disorders and substance use disorders) and preventative care (age-appropriate cancer screening e.g., colon, breast and cervical cancer - as well as weight management counseling, Chlamydia screening (when indicated), smoking cessation and immunizations, etc.) along with coordination of care delivery (when specialty care is needed, the primary care provider will coordinate this care and communicate with the specialty provider regarding the evaluation and treatment plan to ensure continuity of care).

The PACT Model is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of health services through an interdisciplinary team that includes the PCP, RN Care Manager, LVN, clerical associate as well as other clinical services necessary to meet the health goals and needs of the Veteran patient including specialists. While the Primary Care provider's relationship with the Veteran patient will remain key, that bond will be augmented and strengthen by involving other health care staff as part of an integrated team. Together the team takes responsibility for the Veteran patient's ongoing care through a cooperative effort among all members.

Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status, or demographic status (e.g., women veterans, OIF/OEF veterans, employees, etc.). Typically these duties are designated at the time of joining but occasionally during the course of employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives.

Physicians may be assigned duty as a supervising or collaborating physician for one or more mid-level providers. Physicians are expected to provide clinical oversight and review the care provided by mid-level practitioners. Most providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave.

As a nursing assistant, you serve as a member of the nursing team, performing a variety of nursing assignments in the outpatient clinical setting. Major Duties and Responsibilities
Incumbent plans, designs and instructs a variety of group fitness classes for small and large groups. Participates in brainstorming sessions to develop programs, events and classes for a well-rounded physical fitness program. Keep up to date on the latest fitness trends and developments. The PACT team is comprised of a provider, RN, LPN, and administrative support person. Although this position partners with other PACT team members, PACT LPNs successfully and consistently demonstrate independent technical judgment as well as comprehensive and thorough working knowledge of complex practices and procedures. The 4B infusion unit is an 8 chair and 3 bed treatment/recovery area within the South Texas Veterans Health Care System, San Antonio, TX at Audie L. Murphy Hospital. 4B Outpatient is a fast- paced unit with a focus of pre/post care of the procedural patient and recovery of outpatient procedural patients.

The 4B Infusion Unit Registered Nurse will primarily work within the outpatient clinic and trained in observation unit care. In this capacity, the RN will administer a variety of intravenous agents to include blood products. The RN will be responsible for routine maintenance of short and long-term intravenous catheters. The Candidates must have the drive and be committed to provide the veterans with efficient, high quality, and safe care in alignment with STVHCS ICARE values. • Assumes care for veteran patients and their families by providing direct and indirect nursing care, within the scope of RN practice, utilizing the components of nursing process to optimize patient health care outcomes.
• Primary duties for this position will include infusion therapy, cardiac Cath lab recovery, and infusion coordinator coverage.
• Documents patient education, interventions, and other encounters, in appropriately titled medical note(s) through the appropriate computerized program.

The RN Implant Coordinator demonstrates clinical competence by appropriately evaluating the different types of implants used throughout the medical center to determine the appropriate use, storage, and management of implants. The incumbent assumes responsibility/accountability for evaluation of all delegated tasks and is self-directed in the identification of learning needs and development of individual performance plans. Represents the Chief of Staff (COS) in all aspects of administrative leadership,
including, but not limited to:
Serves, in conjunction with the Chief of Staff, Director, Deputy Director,
Associate Director for Patient Care Services, and the Associate Director, as
member of the Medical Center Executive Team.

Serves as a member of key health care system managerial bodies including
serving as a member of the organized medical staff, attending its meetings, and

Duties include:
The position will provide anesthesia coverage for all specialties of surgery, beside specializing in cardiac
surgeries in a care team approach. Must have experience with anesthetic management for cardiac,
Thoracic, and major vascular procedures and providing anesthesia for insertion of ventricular assist
devices and ECMO.
This position will also be experienced in performing neuraxial (epidural and spinal) anesthesia. Additional
responsibilities include the provision of anesthetic care including transesophageal echocardiography for
Non-Cardiac Anesthesia Procedures.
As Director of Cardiac Anesthesia, the incumbent will be responsible for writing and updating standard
Operating Procedures for the Cardiac anesthesia section. Also, responsible for coordinating with the
residency site director and education committee the development of resident education curriculum.
Educational responsibilities: Coordinating with the residency site director and education committee the
development of resident education curriculum and actively participate in the evaluation of residents
rotating through the VA anesthesiology department.
1. Supervisory Controls
The Director of Cardiac Anesthesia reports directly to the Chief of Anesthesiology or designee at
MEDVAMC.
2. Knowledge, Skills, and Abilities (KSAs)
a. Comprehensive medical management of patients to be rendered unconscious or insensitive to pain
and stress during surgical, dental, and certain medical procedures including preoperative,
intraoperative, and postoperative examination, consultation, management, monitoring, evaluation and
treatment.
b. Knowledge and expertise in the following anesthesia core requirements: assessing, consulting, and
preparing patients for anesthesia; rendering patients insensible to pain during surgical, therapeutic, and
diagnostic procedures; monitoring and restoring homeostasis in perioperative and critically ill patients;
diagnosing and treating painful syndromes; managing cardiac and pulmonary resuscitation.
c. Ability to communicate effectively orally and in writing with a diverse group of professional staff
and management officials.
4. Clinical Responsibilities (80% of time)
a. Administration of general anesthesia (inhalation and intravenous) including invasive monitoring.
b. Administration of regional anesthesia including spinal, epidural, and peripheral nerve blocks.
c. Placement of venous catheters
d. Placement of arterial catheters and interpretation of arterial pressures and waveforms.
e. Placement of pulmonary artery catheters and interpretation of pressures/wave forms
f. Airway management including cricothyroidotomy
g. Management of ventilator dependent patients and one lung ventilation
h. Expertise in the following anesthesia core requirements: assessing, consulting, and preparing patients
for anesthesia; rendering patients insensible to pain during surgical, therapeutic, and diagnostic
procedures; monitoring and restoring homeostasis in perioperative and critically ill patients; diagnosing

i. Supervision of CRNA staff, physician residents, SRNAs and medical students in the comprehensive
