This role will be temporarily based at our current unit, Linden Lodge at Nottingham City Hospital, until we move and will continue to deliver early, goal-directed rehabilitation to patients transitioning from our regional Neurosurgery, Spinal and Major Trauma Centres.



In this role you will provide occupational therapy for a caseload of complex rehabilitation patients from neurology, neurosurgery, spinal, and major trauma specialities. You will work creatively and collaboratively with the MDT, patients and their families, as well as external agencies. You will work with the occupational therapy and wider multidisciplinary team to ensure effective service delivery, supporting staff and actively developing the service.

You will receive support and training opportunities for you to develop in this role to reach your full potential. This will include peer and senior support, regular supervision, in-house and external training, as well as varied opportunities to engage in wider learning experiences.

The successful candidate will have excellent core occupational therapy clinical skills, previous experience in neurological rehabilitation, as well as a wide range of clinical experience through other specialities. You will demonstrate a high level of communication and interpersonal skills, with the ability to work both autonomously and as part of a team.

There will be a requirement to contribute to planned weekend and bank holiday rotas.

HC-One Scotland is developing a Specialist Dementia Care Community (SDCC) at Darnley Court care home. Darnley Court has provided care for people living with dementia for many years but following considerable investment in environmental layout and design, workforce and L&D in Carmichael community, we have exciting new opportunities available for those passionate about supporting people who are emotionally distressed by their dementia and require a specialist service to ensure their needs can be met appropriately. As an SDCC, Carmichael will become a therapeutic care setting that demonstrates that quality of care and quality of life are of equal significance. It will provide a bespoke living environment embracing space and calming de-escalation rooms, along with enhanced staffing and specialist training for Colleagues in positive approaches to behaviour, psychosocial therapies, de-escalation interventions and safe restrictive practices. NUH is undergoing a wonderful change with the Maternity Neonatal Redesign (MNR) which will see Neonatal services grow significantly over at QMC. We have built a state-of-the art new centre which will open at the end of this year and we are so excited to move back and settle into our new unit. We are also able to offer experienced candidates the opportunity to work with CenTre transport as an ACP on transport caring for some of our patients while they safely transit around the network.

Both of our units at City Hospital and QMC are involved in teaching and clinical research with strong links with the University of Nottingham. We are also the lead Neonatal Unit for the North Hub of the East Midlands Neonatal Operational Delivery Network which allows us to link with other Neonatal Units in the East Midlands such as Derby, Mansfield, Lincoln and Boston. Working closely with these units allows all babies in the network to receive the care they require and, should the need arise, they can be transferred between hospitals.

The ACNP crosses the clinical boundaries between nursing and medical care, using advanced clinical knowledge and skills to work independently, with support provided by a Consultant Neonatologist for clinical supervision. This includes working out of hours with telephone support from a Consultant Neonatologist.

The post holders will promote a high standard of advanced clinical nursing and medical care by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of babies, parents/carers and families through evidence based clinical practice following agreed policies, protocols and guidelines.

An advanced clinical practitioner exercises independent judgement to provide all elements of clinical care using an holistic, family-centred approach, which may include assessment; planning; provision of care and treatment; diagnosis; investigation and referral; evaluation; transfer; discharge; follow-up.

ACNPs contribute to Tier 2 (Middle Grade/SpR/ST 4-7) or Tier 1 (SHO/ST1-3) rota and clinical nursing shifts, the work pattern of each individual being determined by their level of experience and competence. There will be the opportunity as experience is gained to progress from Tier 1 to Tier 2.

The post-holders will be expected to work on average 37.5 hours per week in a shift pattern. The shift patterns will be compliant with the European Working Time Directive and will involve shifts not more than 13 hours in length and will include working weekends and night shifts on a regular basis. Approximately 25% of the post holder’s time will be used for nursing development and service development, education and training.

Delphi Care Solutions Ltd is looking for a Registered Mental Health Nurse or Registered Learning Disability Nurse for a residential adult care, to provide care in accordance with best practice and legislative requirements in this community-based service in Nottingham, NG8 supporting adults with learning disability and autism. The role aimed at providing direct care and support to individuals with autism and mental health conditions / learning disabilities whilst delivering person-centred care that is bespoke to each individual.



As a Registered Mental Health Nurse or Registered Learning Disability Nurse, you will be responsible for:

Care Delivery - Ensuring Service Users are at the heart of direct care delivery and their wishes and preferences enhance their wellbeing.

Medication Administration - Administer medications and monitor their effects

Stakeholder Management - Developing effective working relationships with other employees within the service and work in cooperation with members of multidisciplinary teams to maximise opportunities for service users and implement care plans

Client Evaluation - Monitor service users' progress and adjust care plans as necessary

Stakeholder Engagement - Educate service users and their families about mental health conditions and available resources

Data Management - Maintain accurate and up-to-date service users' records





Requirements:

Valid NMC Registration - (Essential)

Bachelor's degree in nursing - Learning Disabilities - (Essential)

Minimum 5 years' work experience in mental health nursing/learning disabilities with relevant group of Service Users - learning disabilities, autism and challenging behaviour adults (Essential)

Excellent communication (written and verbal), and organizational skills.

Detail-oriented with strong analytical skills for problem solving.

Ability to work effectively in a multidisciplinary team

Compassionate and empathetic approach to service users’ care

Experience in managing service users’ acute episodes

Excellent knowledge of capacity assessments and DOL’s

Excellent knowledge about autism and LD

Excellent knowledge about reflective practice

Excellent restrain trained/knowledge – understanding of restrain reduction network

Excellent knowledge about communication programmes for adults with learning or communication difficulties

Expert knowledge about medication policies and procedures, related medication electronic systems and regulations

Experience in writing and updating care plans and risk assessments

Specialist speech, language, and communication assessments focusing on functional communication and what reasonable adjustments (Accessible Information Standards) should be in place to support the persons needs. Liaison, advice and recommendations that aim to prevent further communication breakdown and that aim to empower the person to express their thoughts and opinions to the extent they wishes. You will work in an interactive and a consultative way. The role will also involve assessing and developing the management of eating and drinking difficulties for individuals, where issues arise due to a variety of physiological, cognitive and environmental reasons. Co-working with your MDT colleagues, families, and carers to enable their inclusive communication practices in their relationships, care, and interactions with the individual. Attendance at meetings with and on behalf of Orion Ward and CIDT and the individual to plan for ongoing and accessible care.

• To provide personal care and support to every service user with a wide range of needs and abilities
• Undertake the tasks detailed in the Service user’s care and support plans using a person-centered approach, these include:
o Assisting Service users getting up in the morning and going to bed at night
o Assisting Service users to wash, bath and shower
o Assisting Service users to dress and undress
o Assisting Service users to look after their skin, teeth, hair and nails
o Assisting Service users with toileting, continence management and personal hygiene
o Assisting Service users with their medication at the agreed level of support and as detailed in their Medication
o Prepare food and drink for the Service user, being aware of the Service user’s choice, likes/dislikes, nutritional needs and cultural requirements
o Set tables and trays, serve meals and feed service users who need help.
o Provide light general household domestic duties, including housework and laundry, as detailed in the care plan
o Use manual handling equipment safely and correctly
o Provide companionship to the Service user, actively talking and listening to them about their interests
o Help the Service user to maintain contact with their family and friends
o Accompany the Service user on trips into the community
o Assist the Service user to manage their personal affairs
o Care and support people who are at the end of their life

• Have an in depth understanding of the care and support of each Service user you care for
• Provide input into the care and support plans of Service user by regularly feeding back to Field Supervisor and Care Co-Ordinator’s

The following are the core responsibilities of the Medical Secretary/Administrator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. Typing letters, reports and associated documentation as required b. Liaising with external agencies such as hospitals and community services, ensuring referrals are processed efficiently c. Managing all enquires in an effective manner d. Maintaining an accurate referrals database e. Actioning all incoming email f. Processing calling letters as requested g. Scanning of patient related documentation and attaching scanned documents to patients healthcare records h. Inputting data into the patients healthcare records as necessary j. Processing requests for information i.e., SAR, insurance/solicitors letters and DVLA forms k. Clinically coding data on SystmOne l. Answering incoming phone calls, transferring calls or dealing with the callers request appropriately. m. Managing all administrative queries as necessary n. Carrying out system searches as requested o. Maintaining a clean, tidy, effective working area at all times p. Supporting all clinical staff with general administrative tasks as requested In addition to the primary responsibilities, the Medical Secretary may be requested to: a. Partake in audit as directed by the audit lead b. Produce meeting agendas and record the minutes of meetings c. Support reception staff, providing cover during staff absences Complete opening and closing procedures in accordance with the duty rota
