The role will be Full Time and will involve assessing and providing support to clients whom have a range of mild to moderate mental health needs, you with work extensively alongside GPs and practice staff with support from the local mental health service.

The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, practice nurses and other Additional Roles Reimbursement Scheme (ARRS) workers e.g. social prescribers, health and wellbeing coaches, care co-ordinators, as part of the primary care Multidisciplinary Team (MDT).

They will work with primary care GPs to manage the mental health needs of their population at the point of contact. This includes those with lower level support requirements that can be met within primary care, working with the Primary Care Plus (PCP) team and GP practices. The post holder will be a member of the Primary Care Plus team. .This post has the responsibility for providing effective day to day mental health care and management to clients with mental health problems presenting in Primary Care. The PCP team provides a single pathway for assessment, referral management and care coordination on behalf of patients with more complex mental health problems presenting in Primary Care. The service sees adults with bladder and or bowel dysfunction with or without incontinence. The post holder will take a key role in the delivery of the service including assessment, development of individualised treatment plans, give advice to patients, carers, healthcare workers and care home staff, both in clinic settings and at domiciliary visits. We are currently looking for a motivated, professional, and flexible individual able to work 5 days a week, Monday to Friday day shifts, within a small stock count team, counting our Pharmacy Clients.

In this role, you are required to accurately count stock for Retail & Asset Solutions in various high street Pharmacies, this role is extremely important to ensure pharmacies are fully stocked with the medication required for local Doctors and Hospitals.

Please note all training is provided upon your first shift.

Previous stock, audit or retail experience is advantageous but not essential.

As an essential member of the Pharmacy Team, you will work under the direction of registered Pharmacists, to provide Pharmaceutical care for patients. The duties are varied. It is not simply counting tablets or picking a box off the shelf.

Apprenticeship roles at MKUH are Fixed Term for the duration of the training. For this role, the Term is 24 months.You will be paid70% of band 4 in year 1 and 75% of band 4 in year 2. Upon completion, there will be no set role awaiting you, however you will be in a position to apply for posts available that match your new qualification.

Versus our provider’s guidance, if candidates do not pass year 1 then they will not be able to go onto year 2, therefore impacting contract of employment. If year is not passed, then your apprentice contract will reflect this. You must also meet the MKUH standards and behaviours throughout.

The role

As a Functional Assessor, your main task will be to assess individuals and how their disabilities or conditions affect their work life. Drawing on your clinical expertise, you'll listen carefully and take down each person's medical history before performing a physical examination. You'll then produce concise, accurate reports for the Department of Work & Pensions (DWP) so they can decide each person's eligibility for benefits.

The role involves meeting people both face-to-face and via telephone/video. But whether you connect online or in person, you'll be sensitive, compassionate and able to ask the right questions while upholding best practice standards.

Rest assured that you'll have the time you need to make a precise assessment and give each person your whole attention. And while you'll work unsupervised, you'll be part of a wider multi-disciplinary team - support is all part of the package.

About you

• An HCPC registered Occupational Therapist with a minimum of 12 months' post-registration experience within the past five years.
• Continuous Professional Development present on your CV.
• Great oral and written communication skills, with the ability to negotiate and respond to a rapidly changing environment.
• IT skills and experience of using a range of software
• Self-aware and focused on professional development
• Honest and able to maintain integrity and customer confidentiality
• Motivated to contribute to quality customer service, demonstrating empathy, respect and sensitivity in assessment delivery.
• Able to work on your own initiative to meet deadlines and standards.
• Ready to work as a reliable and efficient team member and willing to support the multi-disciplinary team.
• Are eligible to work in the UK (Unfortunately, sponsorship is not possible.)
