Qualifications needed

It is essential that you hold an occupational therapy degree or equivalent, along side being registered with the HCPC.

Another vital requirement is that you have membership of the College of Occupational Therapy.

Working Hours

You will work 37.5 hours per week, part time hours will be considered.

This is maternity cover for a 12 month period.

We are looking for qualified High Intensity CBT Therapist to join us in delivering the best care we can to our patients accessing our NHS Nottingham and Nottinghamshire Talking Therapies service.The CBT therapist role is highly valued and seen as an integral part of what we do. Our therapists provide disorder specific assessments, formulation and evidence-based CBT protocols for people with anxiety disorders and/or depression. If successful, you will be join the community forensic team which sits within the Low Secure and Community Forensic Directorate alongside our low secure inpatient service provided by the Wells Road Centre.

The service provides care to people from diverse variety of backgrounds and cultures and we would encourage a diverse range of applicants to apply. The Services consist of a Multi-Disciplinary group of staff with a supportive and friendly environment.

To assess, design, implement and evaluate safe, creative and effective treatment plans in collaboration with the patient’s needs, their carers and other professionals.

Interventions will be evidence based and designed to reduce stress and anxiety, manage risk and aid recovery.

To maintain accurate, timely and relevant clinical records both written and computerised. To maintaining the safe custody of healthcare records.

Responsible for the safe monitoring, management, conveyance and administration of medication as required by Trust policy.

Demonstrate and be responsible for the safe application of relevant, up-to-date legislation including the Mental Health Act, Mental Capacity Act and Safeguarding.

To work with case managers, inpatient services, social care and 3rd Sector organisations to identify pathways and necessary living arrangements for sustainable community living.

To be able to provide and deliver physical health care interventions and to develop this role within the team.

On going professional leadership within a multi-disciplinary team.

Compliance with best practice directions (NICE) and participation in quality and safety initiatives.

To access and provide clinical supervision to maintain competency and deliver best quality patient care

To undertake any other duties that would be a reasonable expectation of the role

Our practice is looking for an enthusiastic, conscientious, Community Matron/Care Home Nurse. To provide case management to patients with complex needs. Working as part of the Community Support Team based in practice (Senior Community Matron, Care Home Nurse, Care Co-ordinators). We encourage and help with the professional development of staff within the organisation with training as needed. Please see the attached Job Description.

The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: Primary responsibilities a. To develop, implement and embed health promotion and well-being programmes b. To implement and evaluate individual treatment plans for chronic disease patients c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health d. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF e. To provide wound care (ulcer/Doppler etc.) to patients f. To provide travel medicine services g. To request pathology services as necessary h. To process pathology results as required i. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required j. To maintain accurate clinical records in conjunction with extant legislation k. To ensure read codes are used effectively l. To maintain chronic disease registers m. To deliver opportunistic health promotion where appropriate Secondary responsibilities In addition to the primary responsibilities, the practice nurse may be requested to: a. Support junior members of the nursing team, providing guidance when necessary b. Participate in local initiatives to enhance service delivery and patient care c. Support and participate in shared learning within the practice d. Continually review clinical practices, responding to national policies and initiatives where appropriate e. Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)

Your key responsibilities will be: To work as part of a team to deliver the T2DR programme face-to-face to individuals with type 2 diabetes, carrying out telephone consultations as well as communicating with clients via email and secure messaging service To use behaviour changes skills to support clients throughout each phase of the programme by following the guidelines and training provided Taking accurate measurements for instance weight, blood pressure and blood glucose Respond to clients using a variety of communication methods To liaise with supervising dietitian and/or GP regarding any nutritional or medical query as appropriate General day-to-day admin duties to ensure all data is captured in an accurate and timely manner To attend mandatory training and be an active participant in staff meetings To work as part of the team and on your own. Qualifications: You will be required to be a registered nutritionist or working to be a registered nutritionist You will be required to complete a diabetes management course of a minimum length of 6 months, funded by ABL Health.
