Imperial healthcare (Hammersmith Hospital Site) are seeking experienced Band 6/7 nurse to work within our expanding inherited cardiac conditions department. The successful candidate will work Monday to Friday 9 -5, with occasional cross cover at St Marys site.

Providing specialist nursing advice to support and facilitate high quality, individualised care to patients with inherited cardiac condition (ICC).

The successful candidate will have some knowledge and an interest of managing patients with or at risk of ICC, as well as working to identify and support at-risk family members and facilitate family screening. Early clinical review and will ensure individuals are cared for in the most appropriate setting utilising their clinical expertise, leadership, management and educational skills to ensure seamless care from diagnosis to end of life, training provided.

The post holder will provide long-term support to patients with inherited cardiac conditions and their families from the time of referral and throughout the care pathway.

The requirement for this post is extensive cardiac experience (3- 5 years) at Band 5/6 level who processes excellent communication and organizational skills and the ability to work both independently and liaise with multidisciplinary team members within the trust adult and paediatric cardiology directorate as well as Clinical Genetics services and community services.

The Hand Therapy Team at the Chelsea and Westminster NHS Foundation Trust has an exciting opening for either an Occupational Therapist or Physiotherapist with previous experience in Hand Therapy to join our team.

The role includes a varied patient caseload of trauma and acquired conditions of the hand and wrist, from both Orthopaedic and Plastic Hand Surgeons. Hand therapy is carried out in closed trauma fracture clinics, on the ward, and in out-patient settings for post-operative and non-operative patients. The service will run 24hrs a day, 7 days a week for people residing in the catchment area offering open access to a range of psycho-social- medical Interventions at every stage of their wellness journey, this includes short term Hospitality stays for those in crisis, and community based offers that are embedded in the neighbourhood. We will work in partnership with Look Ahead Housing Association, the Tower hamlets VCSE Alliance, experts by experience, in a new fully integrated community mental health centre based in Bethnal Green and serving PCN1.

Integral to the role and ethos of the service is a commitment to co-productivity and the eradication of a “them-and us” culture, a removal of barriers that hinder good care, ensure compassion, empathy and relationship building as the primary focus.

St George’s is recruiting to the Principal Occupational Therapist role for the multidisciplinary therapy team in Senior Health and Medicine and the occupational therapy service in Cardiology and Renal inpatients.

We are looking for a dynamic and flexible occupational therapist with clinical leadership experience and advanced assessment and rehabilitation skills in the management of frailty and/or complex medical long-term conditions. You will provide clinical leadership and operational management of the multidisciplinary therapy team in senior health and medicine. As part of your role, you will provide clinical supervision to the band 7 OTs in senior health, medicine, cardiology and renal inpatients.

You will work closely with other Principal Therapists including the Principal Therapist in the ED and AMU, inpatient cardiorespiratory and neurosciences Principal PTs and OTs and Community Team Leads to ensure high quality evidence-based inpatient therapy provision and a safe and a timely discharge. This role also has strong working relationships with OPAL, the ward MDTs, neighbouring community health, social services, and the voluntary sector.

You will participate in the seven-day Enhanced Weekend Rehabilitation and Discharge service on the medical and senior health wards enabling safe and timely discharge (1 in 6-8 weekends).
With strong links with the Southwest London AHP Faulty and St Georges University, there are opportunities to promote the occupational therapy profession and contribute to developing occupational therapists of the future including apprenticeships, clinical placements, preceptorship and advanced clinical practitioner roles.

1. CLINICAL To be professionally and legally responsible for all aspects of your work, including the management of patients in your care. To promote the use of evidence based practise during the teaching and supervision of more junior staff. ORGANISATIONAL To be responsible for providing an efficient and effective medical rehabilitation Occupational Therapy service, by demonstrating good leadership, organisational, time management and prioritisation skills.
