To Deliver and evaluate an efficient and cost effective Health Visiting service for an identified population. Delivering the national and local public health agendas in response to identified health and social need. To liaise with the nurses, and carry out assigned duties as a member of the multi- disciplinary mental health team. To be prepared to work on own initiative as necessary and report to the Nurses. Participate regularly in clinical & managerial supervision sessions. Liaise with representatives from associated voluntary and statutory organisations and facilitate good communication processes. Co- facilitate groups for some of the NICE recommended treatment interventions for eating disorders. Behavioural Activation and Graded Exposure using the GOALS programme. Problem-solving skills. Improving sleep. Recognising and managing emotions. Confidence building. Support with delivering physical health monitoring pathway for eating disorders. Support with medicines management. To be responsive to service users needs and choices; and uphold their right to be treated with dignity and respect. To include carers and families in line with the service users wishes. To liaise with other health and care providers, including third sector agencies and primary care, to ensure continuity of care for service users. To communicate information concerning the assessment, formulation and care plans of service users in a skilled and sensitive manner in order to promote effective multi-disciplinary working and therapeutic outcomes for clients. To develop collaborative plans for relapse prevention. To deal with endings appropriately and safely with service users, families and carers. Demonstrate the ability to establish and maintain empathic, supportive relationships with people in significant distress, their families and carers. You will be aligned to PCNs in an allocated area in Lincolnshire, and will be part of the Adult specialist eating disorder pathway working across the ED-primary care service and specialist eating disorder service closely on a day to day basis with CBTE practitioners.

The post holder will work with the Infection Prevention Matron to deliver the infection prevention strategic direction and associated work plan for LCHS.


To support implementation of robust and effective systems and processes for infection prevention. To provide a visible presence in all service areas providing high quality, easily accessible clinical infection prevention advice and guidance.
Assist in the development and improvement of infection prevention and control standards in line with local and national guidance.
Under the leadership of the infection prevention and control lead nurse to lead delivery of the staff flu vaccination programme in teams.
Work alongside other specialists to deliver national and local programmes of work.
Develop and deliver programmes which improve or update practice aligned to assessed training needs analysis.
Embed audit as a key area of practice in teams and identify and support actions to improve knowledge and practice.
Facilitate implementation of hand hygiene practice and audit.
Support clinical quality structures across the Trust including representation at the service level quality assurance groups and / or deputising on behalf of the IPC Lead Nurse.
